Also, you if you miss one of the Stanley Coleman letters in Silent Hill 3, it’ll despawn and you none of the subsequent ones with spawn either. On my first playthrough, I got all of them but missed the last one.
Pokemon Ruby and Sapphire: The Masterball It was hidden in the team aqua/magma hideout in an out of the way path. If you left without getting it, you could never go back in that far.
That's the one that comes to my mind you can go through there and not even see it it's off in a corner easily missable and you can't get another one unless you complete the Pokedex and go to the game designer
Black dragon priest staff can be acquired here as well from said dragon priest and you can keep it if you can kill him quickly after he pulls said staff and whirlwind sprint in the portal
Not only do you miss another mask, you miss a golden mask with tusks, which has a unique enchantment that not only gives the affects of the avoid death perk, but has a chance to summon a full on dragon priest to fight for you. Considering dragon priests are some of the toughest enemies in the game, that's kinda a big deal.
Theres another really good missable item in skyrim called Windshear, its a scimitar you can only get during a specific dark brotherhood mission and it has a 100% chance to stagger with every single hit, which makes pretty much every fight in the game easy as stealing a sweet roll from a small child.
When I saw Skyrim on the list, I thought they were referring to the Dragon Priest staff on Skuldafn, not the mask, cause you got to kill the dragon priest quickly grab the staff, then jump into Sovngarde before the portal closes
@The non gamer so, you use the staff to open the portal, and if you want to keep that staff, you need to open the portal, wait till it's ready, then grab that staff and yeet yourself into the portal before the interaction despawns.
Fo3: the medicine bobblehead can be acquired during a quest that takes you back into Vault 101 after one of the main quests is done. There's also a perk that lets you try a broken lock again.
Nathan Levesque An even better example is the energy weapons bobble head located in Raven Rock- the Enclave bunker you don’t get *any* second chances to return to. The medicine bobble head can be recovered by returning to vault 101, so at least you have ONE second chance to recover that one.
I found the sword, and promptly put it in my satchel and forgot about it. But that adventure is easily one of the best in the game... especially the swan dive from Rapunzel's room.
So I just got around to playing the third Witcher game and it’s DLC, and if I hadn’t watched this video three months ago (and somehow remembered it for some reason), I would absolutely have missed that sword, which would have driven me insane. So I am watching this video again as thanks.
Final Fantasy XII: If you open a random chest too early in the game, you'll miss getting Vaan's legendary later on in the game from the very same chest.

This is literally one of my fears. That's why I prefer when mini-maps show me where I've been in a certain building/dungeon by showing me my path. That way, I can make sure I didn't miss any side rooms or corridors that have important and/or valuable things.
Its why I loot EVERYTHING. I need things like TGM, Toggle God Mode, which removes overencumbered in Fallout and Skyrim. Maybe the others, but those two are the only ones I played.
Luke H I use inventory-weight-specific cheats in fallout and Skyrim to carry everything and still play the game somewhat “normally” 👌 “player.modav carryweight 1000”, if you’re wondering. Increases your maximum carry weight by 1000. Repeat as necessary
Mini maps are amazing if done right. It's very easy to turn the game into what could have been a fun exploration game into "follow the marker on the screen like a passenger on a bus ride". For critical quest items though, I don't see any reason why you would allow them to accidentally miss them.
Oh, oh, I have one! Sort of. In Final Fantasy XII, the only way to get the most powerful weapon in the game, the Zodiac Spear, is by NOT opening 4 treasure chests spread throughout the game's world. Except the game never tells you that at any point. There's no warning along the lines of "be careful which chests you open" and given it's an RPG, everyone opens every damn chest they find. Hell, one of the first chests is accessible right near the beginning of the game, long before you have any inkling that this weapon exists, unless you have a game guide or are reading a walkthrough. It's absolutely fiendish.

As soon as we started talking about dragon priests I expected there to be a mention of the fact that you could KILL ONE OF THEM AND WITH THE FINAL BLOW THEY'LL FALL OFF A CLIFF AS THEIR RAG DOLL COLLAPSES AND YOU CAN NEVER GET TO THE BODY.

Apparently there’s another personalised revolver you can get in RDR2 during the epilogue, so weird my game stopped a few missions before that for no particular reason
There’s another one in The Witcher 3: Wild Hunt. In the DLC Hearts of Stone, there is a Venomous Viper Silver Sword that you can only get if you challenge Gaunter o’ Dimm. It is protected by a shadow of a Leshen on a side path.

The Type-40 sword from Nier: Automata. You can only get this weapon through a side quest if you are still playing Route A (2B's pov). You need to beat the entire game to go back to Route A and do the side quest. If you missed the side quest, you will also miss some unique dialogue from operator 6O in Route C.

Do not judge them so harshly. They know not what they do. You can also get a special dialog if you are wearing a mask during the talk just before robbing the vault during the same DLC.

About the Skyrim mask thing: it is important to note, that if you miss just 1 mask, you will not be able to obtain the golden mask "Konahrik" that you can get by going to Labyrinthian, placing all the masks in a certain way, which will teleport you back in time to a place where the last mask can be found.
Heavy rain, After the fight with Normie Jayden and the Masked man, you pull his pockets and he drops gas station receipts. If you forget to pick these up (as I did), Jayden will either DIE because he spent too much time in the ARI or be forced to quit by the player, and is locked out of the perfect ending.
What about The elder scrolls 4 oblivion? Hatred's soul and Hatred's heart, these items are inside certain oblivion gates and once you close them POOF they're gone for good
DANNY 355 Yeah it was pretty hard to find those. Especially since the entrance to where they are is hard to find, mostly because the tower is completely submerged in lava which makes people not even know the weapons exist because of that. I didn’t know they existed until I looked it up on the wikia and thankfully I had just started my third play through so I made sure to obtain them then. Also in Oblivion Calliben's Grim Retort is also a weapon you can easily miss and can only obtain once as well, especially since you technically took it from a crypt in the Imperial City that’s only open once.
Viridiann It’s in Oblivion the game before Skyrim. Both the Hatred weapons are a mace and a bow respectively. With special hatred arrows as well for that bow. While the Grim Retort is a mace.
For those who remember Jeanne D'arc for the PSP, the second gem for Jeanne's second Armlet. I didn't even realize I missed it until I read a FAQ. Then again, It's in a well hidden place (near the back of the map), during a boss fight (Luxuria), on a time limit. Still, even with all of those, I think it falls on the list.

There's a lot of missable guns in RDR2, the Rare Rolling Block is only found on sniper in a mission where you have to get Joshiah back. If you don't switch it out when you kill the gun man in the barn then it's lost forever.
almost everything is miss able in that game depending on the person's curiosity and observation skills. Not to mention there are also a lot of miss able missions themselves if you go through the main story too much or always sleep and never play through night time. Rockstar really made this game for the people who really want to immerse themselves otherwise a lot of people are going to miss a whole lot of shit.

An infamous example from a recent game is the Scythe of Sariel from Fire Emblem Three Houses. This the Death Knight's signature weapon and you can only get it if you have both Mercedes and Caspar in your house (Easier in Black Eagles and Blue Lions since you start with one of them on your team, harder in Golden Dear because you have to recruit both of them), then you have to do defeat the Death Knight in the Mercedes and Caspar with Caspar specifically.
Light arrows, in the Minish Cap. Fuse kinstones with the right person, portal appears, head in, find old man with ghost, suck up ghost with gust jar. Later on, you can get the arrows. Also, the medicine bobblehead can be obtained in the return to vault 101 quest.
But if you don't get it during the return quest, and you leave it is unobtainable unless you use console commands on PC. If you played on console, you have to reload a previous save or kiss it goodbye. I can attest to this, I forgot to get it both times.
Absolutely. Add in it is a *specific* rare red shape kinstone you can only get in a handful of spots before you hit no return and this causes me to stress out the whole first few levels of the game.

I’m surprised that with Skyrim in the list the Unusual Gems weren’t mentioned. Similar to the bobble heads in Fallout you sometimes only get one chance to pick them up, and worse, they are quest items so if you miss one you now have a permanently uncomplicated quest and a bunch of gems you can’t drop.
I had tons of undroppable quest items and broken quests. The game was buggy as hell. I gave up on it after losing twice a days worth of gaming jsut to find out that an important quest doesn't work...
Quick, which would you rather read, The Necronomicon, the Tome of Eternal Darkness, or an Elder Scroll? Necromancy, Rune Magik, or Clairvoyance...all seal your doom and drive you insane....tough choice... I'd probably go with the Tome, though you are doomed, whoever finds it is written into an important part of the history of the epic interdimensional conflict as it seems to choose who finds it by appearing after random room transitions...

Fallout New Vegas’ Dead Money DLC full of stuff you can’t get after leaving, considering you can’t return to the Sierra Madre once you walk out the gate. However, the gold bars in the vault under the Sierra Madre casino can only be obtained if you grab them prior to locking Elijah in said vault forever or after killing him. Once you leave that room you can’t enter it ever again. Have we mentioned that the gold bars are as heavy as you think, and make leaving with them a nightmare even if you’ve stored all of your stuff somewhere before going to the vault?
To add insult to injury the Nahkrin mask is required to get the ninth mask in the base game(if you don't count the wooden one then it is the tenth), which by the way might be the best one really.
Not gonna lie: when I saw "Baldur's Gate" in spoiler list, I expected the entry to be about Big Metal Unit. I mean that is one of the hardest items to obtain in the game (waaaaaay harder than the Flail) and it's so ridiculously easy to miss that I imagine most players never got it: not only do you have to import the Golden Pantaloons from the first game, but you also need to collect the Silvers ones (very easy to miss) and than the Bronze... than take it to Kerrick near the end of the whole saga where he will turn them into the most powerful armor in the game. Miss even one pair of Pantaloons and you're not getting the Unit. Also: not an item in a regular sense, but in "Planescape: Torment" it is EXTREMELY easy to miss recruiting one of the game's NPC, who is basically an animated suit of armor, so... :D The character name is Vhailor and he can only be met during the player's escape from Curse's prison where in stead of going through the escape portal - like the vast majority of players probably did - you need to go beyond it and touch the armor found in the chamber. This will awaken Vhailor and you might be able to get him/it to join you... might, because the conversation may STILL end up forcing you to kill him in self-defense :D
Came here to say this. Nobody ever misses Flail of Ages, but Big Metal Unit is hard even when you know what you're doing - at least once I had to scrap my ToB progress because I forgot to pick up the silver pantaloons!

Remember, ornate engraving serves no tactical advantage whatsoever on your handgun, unless of course your attacker is so enamored with the pistol's aesthetics that they lower their guard just long enough for you to shoot them unchallenged. And if you happen to love a well made handgun as much as Big Boss does, that may very well be the outcome.
You can also collect that dragon staff before you go to Sovngarde. The portal closes quickly, but if you manage to kill the dragon priest just after he picks up the staff you can loot it off of him and try to make it into the portal before it closes. If you run at him when you get to his area he'll go for his staff. If you can quickly get to him before he reaches it you can try to block him from getting his staff long enough for you to almost kill him, he won't start fighting you until he has the staff. When he picks up his staff then you kill him and quickly loot him then get to the center of the portal as fast as you can and you can still progress without having to use the staff. It may take a few tries, but it's worth it if you want the staff. Save before you go up the stairs that leads to his area. He won't trigger until you get close enough.

If you don't grab the Masterball from the Lilycove hideout in Pokemon Ruby and Sapphire, you can't go back and get it after the main story is done. The only way to get one after is to win the lottery, which is rarer than a shiny Pokemon at 1 in 10,000 odds
The Chinpokomon. The first replay of South Park: The Stick Of Truth, I'd gotten so into playing the game that I forgot to pick up one of them AGAIN on my second play-through.
None of the Orichalcum+ are missable (actually there's one you can't get until the very last moment of the story). But they are limited to exactly the number you need. Which is half of what the game says you need because you're expected to use an Energy Crystal.
The entire venomous viper set of the DLC is missable. The armor pieces are only sold by a woman during the auction and the Steel sword can only be looted in the vault
Hollow Knight: I’ve got two (technically four). The first is the unbreakable charms, if you banish the Grimm Troupe. Divine will disappear, and therefore, those amazing charms would be unable to be obtained in an unbreakable form. The second is a fourth Arcane Egg. If you get the Lifeblood Core charm in the lifeblood area of the Abyss before you get an Arcane Egg there, you will lose the ability to get that egg forever.
Every piece of every gun the Nemesis drops if you stay and beat him, instead of running away, are one time only. I only found out he drops them after I suffered through The Mercenaries long enough to get the infinite ammo revolver, and as anyone would, I went on a whole new playthrough fuelled by a rageful need for revenge. Ah... Good times :D
The fact that we've been conditioned to do this, always frightened of being just slightly imperfect, is a damning indictment of some game designers, though.
More on the Nahkriin part: You can also keep the staff, which you're supposed to use to open the portal and lose forever, if you're fast enough in killing and looting the priest.
Neverwinter Nights had that too, if you killed the Drow Matron you could get some good stuff off of her, including the robe you'd normally have gotten when deciding to join her.
In Skyrim, you also can very easily miss the amazing blade Windshear at the end of the Bow on the Katariah at the finale of the Dark Brotherhood Questline. There's a lot of missable items in Skyrim, but Windshear is among the best in the entire game for it's very helpful effects..
Another one is the necromancers amulet in windhelm, because of what I assume is a bugged questline you can only get it by following a guide and doing specific parts of the quest in the correct order. Such a pain
Skyrim: this is tied to nakrin's mask. You can get a mask that heals you if you get all if the masks. And also the mask you get from the college of winterhold questline.
Actually, with the Skyrim one, you need all eight masks for a secret. If you take the eight masks, Rahgot, Nahkriin, Vokun, Volsung, Otar, Krosis, Morokei, and Hevnoraak, to the dungeon of Labyrinthian, outside will be a little barrow with a wooden Dragon Priest mask inside. Wear it inside the little barrow, and it will transport you to a little room that's virtually identical, except for nine podiums in the center of the room. Place each mask on the respective bust of each Dragon Priest, and the dungeon cleared theme will briefly play as the ninth podium opens up, to reveal the unique mask Konahrik. Rather than having a mouth slot, it has two large tusks and the forehead bulges out a little. While it doesn't have the combined effects of the other eight masks, it does give you a unique ability- when your health is below 1/3, much like the Dragon Aspect shout from the Dragonborn DLC, it summons a being from the lore to aid you (a spectral Dragon Priest), or it absorbs the health of nearby foes to heal you. Pretty dope if you ask me, and a great reward. And of course, if you didn't grab Nahkriin, you don't get Konahrik.
How about Duster’s special shoes that make him OP in Mother 3? When you have to go all the way back to a previous boss castle before Tazmily village changes
In Chrono Trigger, there’s a sword that you have to complete a ton of side quests to create (think it’s called the rainbow blade or something). It’s got a 70% crit rate and is easily the best and most missed weapon

yes. it requires FOUR sidequests. two involving the "rainbow shell" and two involving the "sun stone". note that the second "rainbow shell" sidequest involves going BACK to the castle (where the Chancellor had Chrono arrested). you MUST have Marle in the party. also, the first time i did the Sun stone quest, i messed up an obscure task: you need to by some "jerky" in 1000 A.D., then GIVE it to someone in 600 A.D. NOT sell it, GIVE it away FOR FREE.
In fallout 4 theres an unique legendary gatling laser from elder maxson that you can only get after killing him in the institute ending, if you dont pick it up its gone forever.

Two nice things that you can miss: Mercenaries 2: The Hunting Revolver. An extremely powerful, pinpoint accurate, one of a kind handcannon carried by one of the Big Bad's right hand men. But you can only get it if you kill the guy (not capture him alive), and if you don't remember to pick it up before you blow the place to smitheroons, it's gone for good. Ellen Rose: Her 'People Like Grapes' t-shirt. You can't get those anymore. :(
I would like to point out on the Flail of Ages +3 in Baldur's Gate 2 that you could have more than one opportunity to finish it, but if and only if you made your character a fighter, or included fighter in a multi-class character, and said yes to Nalia when she asked you if you wanted the De'Arnise keep as your stronghold. Just FYI.
The first time I saw a flail was in Tekkit Classic, I also always thought of it as a mace, but the head was on a chain and you swing it around a lot more
The first thing I thought of when I read the title was the FFXII Zodiac Spear. Open any one of four chests that are completely forgettable and you miss your chance to get one of the ultimate weapons.
This one was just unforgivable. It made no sense logically - why should opening 1 of 4 perfectly normal looking chests in a number of different geographical locations, prevent you from opening one somewhere else? They also had chests which only spawned once in a blue moon and were the only way of getting specific weapons (I'm looking at you Excalibur).
tthe zodiak spear was farmable, in a chest the, while does respawn, it does with a >10%, and while using the accessory that chnaged the intem in the chest, with a 10% of it being the spear. the genji equipment was missable...
This sort of thing drove me nuts when I was playing through The Surge. Each boss had a special unique weapon that tended to be rather good. You only got them however by making sure to do something obscure during the fight. You could always look them up to make sure you do it properly but then you spoil the boss fight. :/
9:00 Forgot her scroll of icarian flight.... Three words concerning missable items: Sword of Kings, 1/255 chance from a single enemy in a single room in Earthbound who disappears when you clear the two room dungeon, which happens to be filled with enemies who self-destruct AND heal other enemies....I've gotten it twice in over 20 playthroughs, mostly through grind city...and it's the only weapon the character can equip so if you don't get it, the incompleteness of your kit drives you mad... How bout the Mantorok rune in Eternal Darkness. You have to know to backtrack to where a shield wall was after hitting three switches that opens up another shield wall that leads forward, then use a control trapper to crawl through the small opening to get into the room that holds the rune... It's technically missable (but so are a lot of items in Morrowind) the black hand dagger. You basically have to traipse all over Vardenfell murdering people just to get the quest, or stumble upon the dungeon and kill the mark to be instantly given it by the quest giver, that pissed me off to no end spending hours doing the Morag Tong questline only to read on the wiki you can just kill the mark without the mission and get it....best weapon in the game....very frustrating....
That moment when you blow up megaton city was do exciting at first...but then when u realize that it was the most convenient town to trade and use, you get a feeling of regret especially after it's just a crappy amount of caps for doing it...but man that explosion is epic! Lol
What about basically ANY of the special items from vampire the Masquerade bloodlines? There are so many items that you can find obly that one time you get in a map you will never visit again! In the museum, the creepy house, etc...

And my spare keys were also locked in a security lockbox, which opens with a key attached to my main keys, that were locked behind a locked door in my house.

Well that's just stupid. Why would you keep the key to the spare box on your main key ring? If you ever need the spares, it's because you don't have the main keys...
The Problem Solver: Fallout 4. One of the best auto weapons in the game, and you only get one chance at one speech check ONLY if you pick the right speech options beforehand. And yes, I know The Splatercannon does the same thing, but that cost, like, 12,000 caps with maxed charisma.
I missed Emmett and Flaco's revolvers my first time through RDR2 because I thought they were normal guns. And I accidentally managed to blow up the Calvary Dagger when messing with the Gatling in the civial war fort when bounty hunting.

Also, you if you miss one of the Stanley Coleman letters in Silent Hill 3, it’ll despawn and you none of the subsequent ones with spawn either. On my first playthrough, I got all of them but missed the last one.

It was hidden in the team aqua/magma hideout in an out of the way path. If you left without getting it, you could never go back in that far.
That's the one that comes to my mind you can go through there and not even see it it's off in a corner easily missable and you can't get another one unless you complete the Pokedex and go to the game designer

Black dragon priest staff can be acquired here as well from said dragon priest and you can keep it if you can kill him quickly after he pulls said staff and whirlwind sprint in the portal

Not only do you miss another mask, you miss a golden mask with tusks, which has a unique enchantment that not only gives the affects of the avoid death perk, but has a chance to summon a full on dragon priest to fight for you. Considering dragon priests are some of the toughest enemies in the game, that's kinda a big deal.
Theres another really good missable item in skyrim called Windshear, its a scimitar you can only get during a specific dark brotherhood mission and it has a 100% chance to stagger with every single hit, which makes pretty much every fight in the game easy as stealing a sweet roll from a small child.
You can go back and get it anytime after that mission. It will always be there.

When I saw Skyrim on the list, I thought they were referring to the Dragon Priest staff on Skuldafn, not the mask, cause you got to kill the dragon priest quickly grab the staff, then jump into Sovngarde before the portal closes
@The non gamer so, you use the staff to open the portal, and if you want to keep that staff, you need to open the portal, wait till it's ready, then grab that staff and yeet yourself into the portal before the interaction despawns.

Fo3: the medicine bobblehead can be acquired during a quest that takes you back into Vault 101 after one of the main quests is done. There's also a perk that lets you try a broken lock again.
Just pick it up during the G.O.A.T quest. Easy.

An even better example is the energy weapons bobble head located in Raven Rock- the Enclave bunker you don’t get *any* second chances to return to. The medicine bobble head can be recovered by returning to vault 101, so at least you have ONE second chance to recover that one.
You wait until you have almost perfect perk to collect the bobbleheads. Boom. All 10s and 100s when you level up all the way.
This is one time I'm quite glad to have a game spoiled for me, otherwise I know I would've missed the Witcher 3 items

I missed the spectacles at the auction because I had no idea they were DLC... really wanted them too.
I found the sword, and promptly put it in my satchel and forgot about it. But that adventure is easily one of the best in the game... especially the swan dive from Rapunzel's room.

So I just got around to playing the third Witcher game and it’s DLC, and if I hadn’t watched this video three months ago (and somehow remembered it for some reason), I would absolutely have missed that sword, which would have driven me insane. So I am watching this video again as thanks.
Final Fantasy XII: If you open a random chest too early in the game, you'll miss getting Vaan's legendary later on in the game from the very same chest.

This is literally one of my fears. That's why I prefer when mini-maps show me where I've been in a certain building/dungeon by showing me my path. That way, I can make sure I didn't miss any side rooms or corridors that have important and/or valuable things.

Mini maps are amazing if done right.
It's very easy to turn the game into what could have been a fun exploration game into "follow the marker on the screen like a passenger on a bus ride".
For critical quest items though, I don't see any reason why you would allow them to accidentally miss them.
Oh, oh, I have one! Sort of. In Final Fantasy XII, the only way to get the most powerful weapon in the game, the Zodiac Spear, is by NOT opening 4 treasure chests spread throughout the game's world. Except the game never tells you that at any point. There's no warning along the lines of "be careful which chests you open" and given it's an RPG, everyone opens every damn chest they find. Hell, one of the first chests is accessible right near the beginning of the game, long before you have any inkling that this weapon exists, unless you have a game guide or are reading a walkthrough. It's absolutely fiendish.

There’s another one in The Witcher 3: Wild Hunt. In the DLC Hearts of Stone, there is a Venomous Viper Silver Sword that you can only get if you challenge Gaunter o’ Dimm. It is protected by a shadow of a Leshen on a side path.

The Type-40 sword from Nier: Automata. You can only get this weapon through a side quest if you are still playing Route A (2B's pov). You need to beat the entire game to go back to Route A and do the side quest.
If you missed the side quest, you will also miss some unique dialogue from operator 6O in Route C.
The dialogue is super emotional in route C, almost made me cry.

Do not judge them so harshly. They know not what they do. You can also get a special dialog if you are wearing a mask during the talk just before robbing the vault during the same DLC.

1. If you're playing a platformer and miss a platform while going vertically up, you will, in your anxiety to land on the platforms below, manoeuvre your character in such a way so as to miss literally every other platform, and land at the very bottom.
2. Power cuts will occur ONLY when the game is autosaving.
3. You'll always have too much of in-game currency, except for when you actually want to purchase that expensive item you absolutely need.
4. You'll do extremely well on your first attempt against a difficult boss, die, and then fail to recapture that success for the next hundred attempts. (By ViddyOGamesForFun - this applies to difficult levels/sections in general)
5. In RPGs where your choices affect the ending, you will inevitably obtain the worst ending on your first playthrough.
6. (By Filou I) Online connection issues are most frequent when the match has just started.
7. (By missy prime) In an FPS, it is most likely that an enemy shall appear at the exact moment you begin to reload.

About the Skyrim mask thing: it is important to note, that if you miss just 1 mask, you will not be able to obtain the golden mask "Konahrik" that you can get by going to Labyrinthian, placing all the masks in a certain way, which will teleport you back in time to a place where the last mask can be found.
Heavy rain, After the fight with Normie Jayden and the Masked man, you pull his pockets and he drops gas station receipts. If you forget to pick these up (as I did), Jayden will either DIE because he spent too much time in the ARI or be forced to quit by the player, and is locked out of the perfect ending.
Bro I still got the trophy for finishing the game with all 3 playable characters alive even though I died with Jayden.

What about The elder scrolls 4 oblivion? Hatred's soul and Hatred's heart, these items are inside certain oblivion gates and once you close them POOF they're gone for good
DANNY 355 Yeah it was pretty hard to find those. Especially since the entrance to where they are is hard to find, mostly because the tower is completely submerged in lava which makes people not even know the weapons exist because of that. I didn’t know they existed until I looked it up on the wikia and thankfully I had just started my third play through so I made sure to obtain them then.
Also in Oblivion Calliben's Grim Retort is also a weapon you can easily miss and can only obtain once as well, especially since you technically took it from a crypt in the Imperial City that’s only open once.

Viridiann It’s in Oblivion the game before Skyrim. Both the Hatred weapons are a mace and a bow respectively. With special hatred arrows as well for that bow. While the Grim Retort is a mace.

For those who remember Jeanne D'arc for the PSP, the second gem for Jeanne's second Armlet. I didn't even realize I missed it until I read a FAQ. Then again, It's in a well hidden place (near the back of the map), during a boss fight (Luxuria), on a time limit. Still, even with all of those, I think it falls on the list.

There are two more heads you can pick up later in the game, giving it 5 total.
There's a lot of missable guns in RDR2, the Rare Rolling Block is only found on sniper in a mission where you have to get Joshiah back. If you don't switch it out when you kill the gun man in the barn then it's lost forever.
almost everything is miss able in that game depending on the person's curiosity and observation skills. Not to mention there are also a lot of miss able missions themselves if you go through the main story too much or always sleep and never play through night time. Rockstar really made this game for the people who really want to immerse themselves otherwise a lot of people are going to miss a whole lot of shit.
There was also guns that you unlocked by playing GTA5, I'm not sure if they are in RDR2 elsewhere, but they could be somewhat rare.

An infamous example from a recent game is the Scythe of Sariel from Fire Emblem Three Houses. This the Death Knight's signature weapon and you can only get it if you have both Mercedes and Caspar in your house (Easier in Black Eagles and Blue Lions since you start with one of them on your team, harder in Golden Dear because you have to recruit both of them), then you have to do defeat the Death Knight in the Mercedes and Caspar with Caspar specifically.
Where are literally all the special weapons from Final Fantasies? I once quit a playthrough of 7 when I realized I had missed Barrets best one.

Light arrows, in the Minish Cap. Fuse kinstones with the right person, portal appears, head in, find old man with ghost, suck up ghost with gust jar. Later on, you can get the arrows. Also, the medicine bobblehead can be obtained in the return to vault 101 quest.
But if you don't get it during the return quest, and you leave it is unobtainable unless you use console commands on PC. If you played on console, you have to reload a previous save or kiss it goodbye. I can attest to this, I forgot to get it both times.
Absolutely. Add in it is a *specific* rare red shape kinstone you can only get in a handful of spots before you hit no return and this causes me to stress out the whole first few levels of the game.

I’m surprised that with Skyrim in the list the Unusual Gems weren’t mentioned. Similar to the bobble heads in Fallout you sometimes only get one chance to pick them up, and worse, they are quest items so if you miss one you now have a permanently uncomplicated quest and a bunch of gems you can’t drop.
Are those the gems need for the Crown of Berenthia? I got all of them after doing every side questline and story. None are missable anymore.
Plus, once you start the quest, the gems don’t stack. My misc inventory on my switch save is 80% stones of barenziah at any given time.
I had tons of undroppable quest items and broken quests. The game was buggy as hell. I gave up on it after losing twice a days worth of gaming jsut to find out that an important quest doesn't work...

Fallout New Vegas’ Dead Money DLC full of stuff you can’t get after leaving, considering you can’t return to the Sierra Madre once you walk out the gate. However, the gold bars in the vault under the Sierra Madre casino can only be obtained if you grab them prior to locking Elijah in said vault forever or after killing him. Once you leave that room you can’t enter it ever again. Have we mentioned that the gold bars are as heavy as you think, and make leaving with them a nightmare even if you’ve stored all of your stuff somewhere before going to the vault?
To add insult to injury the Nahkrin mask is required to get the ninth mask in the base game(if you don't count the wooden one then it is the tenth), which by the way might be the best one really.

Not gonna lie: when I saw "Baldur's Gate" in spoiler list, I expected the entry to be about Big Metal Unit.
I mean that is one of the hardest items to obtain in the game (waaaaaay harder than the Flail) and it's so ridiculously easy to miss that I imagine most players never got it: not only do you have to import the Golden Pantaloons from the first game, but you also need to collect the Silvers ones (very easy to miss) and than the Bronze... than take it to Kerrick near the end of the whole saga where he will turn them into the most powerful armor in the game. Miss even one pair of Pantaloons and you're not getting the Unit.
Also: not an item in a regular sense, but in "Planescape: Torment" it is EXTREMELY easy to miss recruiting one of the game's NPC, who is basically an animated suit of armor, so... :D
The character name is Vhailor and he can only be met during the player's escape from Curse's prison where in stead of going through the escape portal - like the vast majority of players probably did - you need to go beyond it and touch the armor found in the chamber. This will awaken Vhailor and you might be able to get him/it to join you... might, because the conversation may STILL end up forcing you to kill him in self-defense :D

Came here to say this. Nobody ever misses Flail of Ages, but Big Metal Unit is hard even when you know what you're doing - at least once I had to scrap my ToB progress because I forgot to pick up the silver pantaloons!

Remember, ornate engraving serves no tactical advantage whatsoever on your handgun, unless of course your attacker is so enamored with the pistol's aesthetics that they lower their guard just long enough for you to shoot them unchallenged. And if you happen to love a well made handgun as much as Big Boss does, that may very well be the outcome.
You can also collect that dragon staff before you go to Sovngarde. The portal closes quickly, but if you manage to kill the dragon priest just after he picks up the staff you can loot it off of him and try to make it into the portal before it closes. If you run at him when you get to his area he'll go for his staff. If you can quickly get to him before he reaches it you can try to block him from getting his staff long enough for you to almost kill him, he won't start fighting you until he has the staff. When he picks up his staff then you kill him and quickly loot him then get to the center of the portal as fast as you can and you can still progress without having to use the staff. It may take a few tries, but it's worth it if you want the staff. Save before you go up the stairs that leads to his area. He won't trigger until you get close enough.

The event items in the Pokémon games. I remember getting an action replay just so I could get certain items to obtain mythical Pokémon.
This, I always hated this mechanic, made the game exclusive and it didn't make any sense.
If you don't grab the Masterball from the Lilycove hideout in Pokemon Ruby and Sapphire, you can't go back and get it after the main story is done. The only way to get one after is to win the lottery, which is rarer than a shiny Pokemon at 1 in 10,000 odds

None of the Orichalcum+ are missable (actually there's one you can't get until the very last moment of the story). But they are limited to exactly the number you need. Which is half of what the game says you need because you're expected to use an Energy Crystal.

The entire venomous viper set of the DLC is missable. The armor pieces are only sold by a woman during the auction and the Steel sword can only be looted in the vault

I’ve got two (technically four).
The first is the unbreakable charms, if you banish the Grimm Troupe. Divine will disappear, and therefore, those amazing charms would be unable to be obtained in an unbreakable form.
The second is a fourth Arcane Egg. If you get the Lifeblood Core charm in the lifeblood area of the Abyss before you get an Arcane Egg there, you will lose the ability to get that egg forever.
Every piece of every gun the Nemesis drops if you stay and beat him, instead of running away, are one time only.
I only found out he drops them after I suffered through The Mercenaries long enough to get the infinite ammo revolver, and as anyone would, I went on a whole new playthrough fuelled by a rageful need for revenge. Ah... Good times :D
I've never been so glad that I compulsively check for items everywhere in games and steal everything not nailed down.

The fact that we've been conditioned to do this, always frightened of being just slightly imperfect, is a damning indictment of some game designers, though.
You've obviously missed the crowbar to pry up the nails. (And are therefore going to be missing more items.)

More on the Nahkriin part: You can also keep the staff, which you're supposed to use to open the portal and lose forever, if you're fast enough in killing and looting the priest.
Neverwinter Nights had that too, if you killed the Drow Matron you could get some good stuff off of her, including the robe you'd normally have gotten when deciding to join her.
'No neon under-lighting' that's fine, you can just cast Yrden.
In Skyrim, you also can very easily miss the amazing blade Windshear at the end of the Bow on the Katariah at the finale of the Dark Brotherhood Questline. There's a lot of missable items in Skyrim, but Windshear is among the best in the entire game for it's very helpful effects..
Another one is the necromancers amulet in windhelm, because of what I assume is a bugged questline you can only get it by following a guide and doing specific parts of the quest in the correct order. Such a pain
Skyrim: this is tied to nakrin's mask. You can get a mask that heals you if you get all if the masks. And also the mask you get from the college of winterhold questline.
Actually, with the Skyrim one, you need all eight masks for a secret.
If you take the eight masks, Rahgot, Nahkriin, Vokun, Volsung, Otar, Krosis, Morokei, and Hevnoraak, to the dungeon of Labyrinthian, outside will be a little barrow with a wooden Dragon Priest mask inside. Wear it inside the little barrow, and it will transport you to a little room that's virtually identical, except for nine podiums in the center of the room. Place each mask on the respective bust of each Dragon Priest, and the dungeon cleared theme will briefly play as the ninth podium opens up, to reveal the unique mask Konahrik. Rather than having a mouth slot, it has two large tusks and the forehead bulges out a little. While it doesn't have the combined effects of the other eight masks, it does give you a unique ability- when your health is below 1/3, much like the Dragon Aspect shout from the Dragonborn DLC, it summons a being from the lore to aid you (a spectral Dragon Priest), or it absorbs the health of nearby foes to heal you. Pretty dope if you ask me, and a great reward.
And of course, if you didn't grab Nahkriin, you don't get Konahrik.
How about Duster’s special shoes that make him OP in Mother 3? When you have to go all the way back to a previous boss castle before Tazmily village changes

In Chrono Trigger, there’s a sword that you have to complete a ton of side quests to create (think it’s called the rainbow blade or something). It’s got a 70% crit rate and is easily the best and most missed weapon
yes. it requires FOUR sidequests. two involving the "rainbow shell" and two involving the "sun stone".
note that the second "rainbow shell" sidequest involves going BACK to the castle (where the Chancellor had Chrono arrested). you MUST have Marle in the party.
also, the first time i did the Sun stone quest, i messed up an obscure task: you need to by some "jerky" in 1000 A.D., then GIVE it to someone in 600 A.D. NOT sell it, GIVE it away FOR FREE.

In fallout 4 theres an unique legendary gatling laser from elder maxson that you can only get after killing him in the institute ending, if you dont pick it up its gone forever.

FF3s Odin fight is the only chance in the game where you can steal Gungir the arguably best Lance in the game.
Two nice things that you can miss:
Mercenaries 2: The Hunting Revolver. An extremely powerful, pinpoint accurate, one of a kind handcannon carried by one of the Big Bad's right hand men. But you can only get it if you kill the guy (not capture him alive), and if you don't remember to pick it up before you blow the place to smitheroons, it's gone for good.
Ellen Rose: Her 'People Like Grapes' t-shirt. You can't get those anymore. :(
I would like to point out on the Flail of Ages +3 in Baldur's Gate 2 that you could have more than one opportunity to finish it, but if and only if you made your character a fighter, or included fighter in a multi-class character, and said yes to Nalia when she asked you if you wanted the De'Arnise keep as your stronghold. Just FYI.
The first time I saw a flail was in Tekkit Classic, I also always thought of it as a mace, but the head was on a chain and you swing it around a lot more

The first thing I thought of when I read the title was the FFXII Zodiac Spear. Open any one of four chests that are completely forgettable and you miss your chance to get one of the ultimate weapons.
This one was just unforgivable. It made no sense logically - why should opening 1 of 4 perfectly normal looking chests in a number of different geographical locations, prevent you from opening one somewhere else? They also had chests which only spawned once in a blue moon and were the only way of getting specific weapons (I'm looking at you Excalibur).
tthe zodiak spear was farmable, in a chest the, while does respawn, it does with a >10%, and while using the accessory that chnaged the intem in the chest, with a 10% of it being the spear.
the genji equipment was missable...
This sort of thing drove me nuts when I was playing through The Surge. Each boss had a special unique weapon that tended to be rather good. You only got them however by making sure to do something obscure during the fight. You could always look them up to make sure you do it properly but then you spoil the boss fight. :/

Three words concerning missable items: Sword of Kings, 1/255 chance from a single enemy in a single room in Earthbound who disappears when you clear the two room dungeon, which happens to be filled with enemies who self-destruct AND heal other enemies....I've gotten it twice in over 20 playthroughs, mostly through grind city...and it's the only weapon the character can equip so if you don't get it, the incompleteness of your kit drives you mad...
How bout the Mantorok rune in Eternal Darkness. You have to know to backtrack to where a shield wall was after hitting three switches that opens up another shield wall that leads forward, then use a control trapper to crawl through the small opening to get into the room that holds the rune...
It's technically missable (but so are a lot of items in Morrowind) the black hand dagger. You basically have to traipse all over Vardenfell murdering people just to get the quest, or stumble upon the dungeon and kill the mark to be instantly given it by the quest giver, that pissed me off to no end spending hours doing the Morag Tong questline only to read on the wiki you can just kill the mark without the mission and get it....best weapon in the game....very frustrating....

What about basically ANY of the special items from vampire the Masquerade bloodlines? There are so many items that you can find obly that one time you get in a map you will never visit again!

Well that's just stupid. Why would you keep the key to the spare box on your main key ring? If you ever need the spares, it's because you don't have the main keys...
The Problem Solver: Fallout 4. One of the best auto weapons in the game, and you only get one chance at one speech check ONLY if you pick the right speech options beforehand. And yes, I know The Splatercannon does the same thing, but that cost, like, 12,000 caps with maxed charisma.
I missed Emmett and Flaco's revolvers my first time through RDR2 because I thought they were normal guns. And I accidentally managed to blow up the Calvary Dagger when messing with the Gatling in the civial war fort when bounty hunting.
