As far as I know there was an airplane mode but it'd stop your in-game progress until you'd put your phone in normal mode back again. Still, quite a neat feature and proves GTA IV was ahead of it's competition and even future games.

In my case, I've noticed that if you keep enough distance between yourself and the dealer, he will not be alerted by your phone ringing and will continue walking as usual. I also second your opinion, It's amazing how many tiny details I still find out about this game considering the amount of times I've completed the story before.
Spaced out over 16 years it equals about 50 minutes per day. Some days you would play 3-4 hours and other days you don't play at all. It's still a massive amount of time but not impossible.
There's also another possibility where Niko whispers rather than shout, and you're able to continue the mission without the dealer being spooked.
