The first 16 years essentially are just getting the marvel universe back together, then after, we can see storylines we know and love with ALL the characters.
The nature vs. nurture angle is a brilliant play. Tony was adopted into a wealthy family, so he didn't do anything heroic with his life until he was forced into a cave and had to fight his way out. If that same person was born into abject poverty in a war-ravaged country, oppressed by a tyrannical government, and watched his witch mother burned at the stake, that innate genius would've come out sooner. Victor's whole life could play out on-screen, where instead of 40-yo Tony building a suit, we see teenage Victor inventing Doombots to overthrow the evil baron who killed his mom. Decades later, he rules over a country, has a big family, lives in a castle--it's the same person on a darker path, leading another life. Plus, if Howard & Maria only adopted a Latverian orphan in ONE timeline, Stark ain't coming back from elsewhere in the multiverse.
We really need to consider just why starks parents were killed and just how they came across Victors family and his adoption!! If the Red Skull knew of the potential due to his access with the cube this would be mind bogglinly incredible
Realistically Kevin Feige and the MCU basically made Thanos, similar to Apocalypse in that Apocalypse believed in survival of the fittest and wasn’t just went about it terribly

One of the two. For the MCU, it makes 100% perfect sense to make him a DOOM variant instead. It harkens back to the comics, it serves the narrative purposes of setting up doomsday and concluding the multiverse saga, it's perfect. Though, I will say they could reverse the direction of this, and make DOOM an adopted tony. If I recall, DOOM also has some versions of his origin story where he was adopted, so they could easily swing the opposite. In fact, one one-of multiverse story was a case where they... was it a body swap or being swapped at birth? I don't recall, but I do know that they finished with a green and silver iron man suit, a red and gold DOOM suit, and birth-tony as DOOM winning.
I just hate that we're jumping straight to Secret Wars with Doom. It's lame that we have to piggyback off of RDJ to get people interested in Marvel's premier villain.

Who was surprised? There was no surprise. The father didn’t know how far along she was by months, only by size. That’s no indication of adoption.
