Does it affect the results if I use DEM instead of DTM in HEC RAS?

Yes, the software will interpret each tree, car, as a big obstacle (like a hill) and this will definitely change the behavior of the water flow. You can eventually keep the houses. But those might be flown away while flooding in the reality
