    Thank you, doctor. Now I understand how this happened in a split second, very painfully, and with no traumatic cause. I have tested positive with the rheumatoid R-factor but do not have digital deformation (like both my parents). I am disabled (scoliosis and COPD) and somewhat sedentary for he last several years; although I did physical work during my careers, I’ve never been a boxer or seriously injured my knuckles (although I’ve broken just about all my fingers but for my thumbs). I guess the rheumatoid arthritis is showing up in my senior years. Thnx again.

    Mine is an interesting tale. Still working on diagnosis and proper treatment. On 4/6/23, I was rear ended at a traffic light. I was fully stopped and another vehicle hit me at speed. Broke my steering wheel off at 10 and 2 with my hands/arms. I had a SLAP tear in my right shoulder that required a biceps tenodesis and SAD repair. My left hand, dominant, had a partial ligament tear of the MCP joint of the index finger on the radial side. A steroid injection on the radial side was the initial treatment. At 2 months after the injection, I was still having pain in the MCP joint. A second injection was done, this time into palm section of the joint. Three days after the injection, I was lifting a baking sheet out of my oven and something popped in my joint. I immediately notified my doctor. His answer was to wait 4 weeks and let the steroid work. There was increasing instability in the joint, an odd deformity on top of the knuckle and constant pain. I could not pinch items, as the index finger would deflect easily to the ulnar side. I began to experience some locking of my finger in extended position. This led to my doctor ordering another MRI to tell me it was muscle strain. And a trigger-finger release surgery. 6 weeks after surgery, I'm still experiencing pain and swelling in the joint. I'm am currently doing PT for the joint. However, I am seeing some subluxation of the tendon towards the ulnar side. I am now suspicious that the steroid injection caused the sagittal band to rupture and am now seeking another doctor. You're video was most informative at helping me understand why my finger has not been healing. Thank you.

    Stinging pain in my middle knuckle only when I hit something. I know 100% it’s from punching hard objects for no reason.
    Although, there aren’t any deformities on my knuckle, and it doesn’t hurt to move it in any way. Wtf

    After a lifetime of MMA with no problem.. I was simply sitting on my yoga mat this moring to stretch, put my knuckles on the mat to simply move positions and "pop" ... Thank you so much for explaining what this is... I couldn't then hold coffee pot to pour a cup of coffee. Tendon back and forth over knuckle as I make a fist and release. It is tollerable typing here but HOPE it resolves?? Dunno yet :)

    Very helpful . A brilliant presentation. I’ve got a mild pull I think . Starting from middle knuckle extending to thumb other fingers and around wrist . I was knocked over and I fell backwards . Head injuries .treated at hospital. Hand mentioned and coccyx but they weren’t concerned about hand .

    I injured my hand, popping my knuckle. The Dr assigned the Saggital Band Rupture after ruling out a broken knuckle. He did this based on where the pain and welling was. I got a Yoke splint and 4 dinky OT visits.
    Based on this video - there’s no way I injured my saggital band. I do not have any of the symptoms, nor any of the mobility restrictions. There is absolutely zero subluxation if the extensor tendon. There is subluxation of the MCP. I’ve also experienced, now, what feels like carpal tunnel pain, which I’ve never had in my entire life. And I don’t work at a computer that much anymore - and esp not since the injury.
    I’m 11 weeks out from initial injury and there’s no improvement in swelling (soft tissue swelling between MCP 2&3) nor in the pain associated with specific movements of the hand. There is some moderate subluxation of the MCP itself.
    What should I do? New doctor, 2nd opinion? (He’s the local hand superstar. Highly respected. Don’t want to offend him.)
    But I think there’s more to this injury than snapping or tearing or spraining the saggital band. He only took a few x-rays. Should I ask for additional imaging?
    Maybe things aren’t improving because we’re not treating the whole injury?

    From I was young the tendons my my middle fingers always slipped and I could actively move the tendons in my pointer fingers too, just not as loose. I haven’t had any injuries of the sort. I can flip a middle finger but when I turn my hand downward I noticed I need to use more force to flip the middle finger out. Is there any way this could be some type of birth defect?

      It has to be, because I’ve had it since I can remember. Except my ring finger on my right hand the tendon will go completely behind my knuckle and if I try to straighten it I can get the tendon to snap and it sounds disgusting.
      I need to find a solution to this as I play guitar and drums and it makes it hard to do certain things when my hands will lock up mid song if I play something a little quick. I’ve been able to build the dexterity in my right hand quite a bit, but my left is lagging behind.

      The worse part is that they surround it with a mystic aura and make it seem normal.
      While the best punchers on the planet are saving their hands from injury, the best kickers always wear protection in training and the best lifters train safely without pushing over their limits.

    Thank you Dr.Ehraheim for explaining so thourghly. I have a Nontraumatic subluxation of extensor tendon at metacarpophalangeal joint, all from flicking my middle finger. When I flicked my finger, pain to my knuckle n then swelling. My dr. suggested surgery. I am going to ask if I can try the conservative treatment first.

      me too! well, i cant move the whole thing, but when i make a fist it moves too the side. i dont think i have arthritis so i've been wondering why my hand does this. i've been searching this up but i dont really see any results that arent "youre injured" or "arthritis"

      I also have had this my whole life, it's not a painful thing and my fingers just lock up when it happens but it doesn't cause pain.
      The only problem I find is when writing for long periods the area gets very fatigued.
      I mostly just use it to freak people out since it makes a loud noise.

    I have been in constant pain for over a year but due to the covid pandemic my doctor will not even allow me an appointment I have had x-rays that have shown negative and been to a physiotherapist who also hasn't helped. How do I explain to my doctor that I actually have an injury that needs medical treatment?

      You could try to splint it at home. If the swelling goes down you know you are on the right track. I have this problem and I just got home from the doctor. Xray was negative. Surgery is a last resort. Hope you are healed up soon. Anti inflammatory medications may help as well.

      @@sevchyk I'm still waiting for treatment, universal health care is a bit slow to non emergency procedures. I tried splinting and taping, stretching and soaking, heat and cold, all kids of things with little relief. Simply not using the affected hand for few weeks improved the strength and mobility, but the problem is still there albeit less affected . I try to keep my hand flat as possible when resting to prevent the ligament from slipping down the knuckle and that was key. The rest of my hand was contorting to compensate, improved by 40% more or less.

      @@sevchyk it does bother me at night before bed, so I take an anti inflammatory and position my hand under my asscheek to keep it flat until I fall asleep.

    Thank you. I just got diagnosed with RA after having surgery to repair the extensor tendons of the middle finger in my left hand; one side was almost entirely torn away and the other side was almost there too. Your video explained what happened to me clearly. Unfortunately, I'm feeling similar twinges in my right hand. I'm wondering if using the brace you suggested will help me avoid surgery on the right hand, or am I doomed to repeat what happened to the left hand because I have RA? Thank you.

      You need surgery to repair the damage. Things are just floating around and that's not good. Luckily I live within walking distance of the best hand surgeons on the planet.

    Hey Doctor, great video I found it very informative. I just had a few questions. Can there be swollen tissue on or around the knuckle that could feel like the band is misplaced or be mistaken for a rupture? and is there a vast difference in the pain and ache between hitting an object getting bruised swollen knuckles and hitting a solid object and rupturing the knuckle(s)? In my case I’ve hit solid objects multiple times in my life, I never considered really treating or icing every time it happened I just let the swelling and scabs(scabs came from breaking skin nothing deep at all just scrapes) heal naturally and I’m not sure if Ive developed a pain tolerance in my knuckles and can’t tell the difference between a rupture and swelling and bruising.

      @@Michelle-t311 the doctor also added that it could just be fluids, fatty damaged tissue that “doesn’t matter” more specifically he said something like “you may have just damaged some stuff that doesn’t matter” this was at an Urgent Care called EXCER

      @@mikeoxlong726 surgery wont help if you keep doing the same thing that caused the injury. Splint the finger, take some anti inflammatory drugs and see how it looks n feels after 2 weeks.

      @@PrettyUglyR6 True, but in my case I got attacked by 3 toxics, it was self defense, so I resolved something to an extent cause I managed to get back home with minor bruises caused by them, but I injuried my strong hand seriously, and I'll probably never recover 100% again..

    I got this from punching my ex in the face ... he was a boxer so we were just boxing and I went to the dr and he said oh tht will go bck to normal in 2 weeks... it's been 15 years now . Wish I tried to atleast used a splint for my finger maybe it would have healed. Ik its going to cause arthritis

      I asked that same question of my hand surgeon this morning but a tendon is NOT inside a joint so the answer I got was no--unless there were other factors at play (for example, you have rheumatoid arthritis already, etc.)

      My hand was fine until I flicked some dirt off my fingers in the air really hard and now my tendon just started slipping off my knuckle and it's all sore. I'm freaking out 😭

    I Hit My Hand On My Beds Wooden Frame And I Did Feel Pain For Hitting My Hand And The Pain Instantly Disappeared A Day Later I Was Holding A Pen And Noticed That My Middle Fingers Tendon Moves To The Side When I Closed My Hands And i looked At My Other Hand And closed It and That Was Fine I Still Have It A Year Later
    Do I Need To Seek Medical Attention??

    I ve been hitting the heavy bag too hard with my left hook, which is an absolutely devastating hook.
    The stronger I hit it, the worse my SAGITTAL Band Injury gets.
