Usually the main stress would be on 'shirt' so if you stress 'Tilly' more then yes, that's contrastive stress. You would do that to ensure the listener understand that it's Tilly and not another person.

to emphasise the word, do you use a high volume, lengthen the word and use a higher pitch?

The implied meaing is really hard to recognize in some situations😭
