He knows what the last round is, otherwise he always goes 50/50. There even is a bug where if he use a magnifying glass to look at the next round, then drink a beer to eject it, he will still believe the round is what he saw with the magnifying glass.
It doesn't know the last shell, it only knows what the last shell is when it is on the last shell of the shotgun, otherwise it would be playing like this 30:10 and wouldn't be playing like this 32:59

In short its worth taking the 60% chance of death over the 75% chance of death here.

Beer can be useful for "stalling his turn" basically say like if u have 2 Live left, use the beer to waste one, and use the other to shoot, which then starts a new round and you can still start your turn

Honestly I found Full house the much more difficult achievement. For the 1 mil cheevos there's at least a measure of strategy involved. Full house on the other hand is 100% RNG as you need to entirely fill your inventory with 8 unique items one of which being adrenaline and have the one unique item you don't have be owned by the dealer so you can steal it from him

For those wanting to gain the achievements, you actually don't have to go 18 rounds. The key is the scoring system: you start with 70k and you linearly lose 4k every minute. If you manage to beat the game in 9:40, meaning having left at least 31250 points, you'll only need to double it 5 times, therefore achieving 1 mil in 6 rounds.
If they nuke the Trash Medicine, or make it 70/30 to heal you and make rounds between 3-5 lives the game will be infinitely better because the dealer couldn't randomly get you whit a Saw or Cufs when you start a rownd whit 2 life and sh@@t items.
You know, in order to load 8 rounds into this shotgun you would need to rack one shell then put another shell in, but ig it would be less confusing if it just puts all 8 in instead of 7, rack, and then the 8th

I don’t think the items not refreshing after the third round when you double it and move on to another first round is a gitch. Some players call it the “carry-over” round because it allows to to carry the items to the next round, so the strategy is to move on while using items generously in the first two rounds and use them strategically during the third round to give yourself an advantage after you double. The developer probably designed it that way to make it harder each time you double it if you’re too focused on winning the third round and used most items.

The expired medicine has been changed to be a 50/50 now instead of 40/60
And the dealer will still gain hp every time he takes it bc this game is rigged

there were a few times i decided to take meds with only one charge left and it always brings me back to 3 charges, never died to meds so far
