We can see the actor looking for her and being worried while she's seeing him as the young him. At least that's how I see the MV
The story of ONLY basically is that she has Alzheimer's/Dementia. She doesn't remember whats going on in her current life( where she is at, falls asleep somewhere outside her home and wakes up elsewhere) but remembers things she loves doing like dancing and her husband and she still sees themselves as when they were young as thats the happy memories she DOES remember even though she has Dementia/Alzheimer.
I love Lee Hi. I've been following her carreer since she was in kpop Star (Never dissapointed me tbh) and I could say she's my favorite female soloist (i'm sorry Queen Ailee 😭). Even though I always liked her music, I feel she was able to do the music she wanted to do the moment she steped out of yg and went to AOMG and OMG THE IMPROVEMENT. Really her latest album is art and I do really recommend You to give it a listen. She also did a live performance of said album that's masterly as well.

X or XI or O - all Code Kunst tracks (a legendary Korean hiphop producer) all extremely good if so best take them in order one of them has a video, but again she doesn't feature in it. and finally if you want to go deep into most things K-R&B check out Chancellor's Automatic Remix, it has a verse from Lee Hi and many other prominent R&B acts from Korea well basically just search Lee Hi and pick whatever it is all very good.
