What is Delusional disorder? How Is It Different From Schizophrenia?

  • Delusional disorder is very different from schizophrenia. The only thing they have in common is the presence of delusions. Schizophrenia is delusions, plus hallucinations, disorganized thoughts and behavior and other cognitive or thinking problems.
    A delusion is a fixed false belief. Despite evidence that goes against what you believe, you still believe it. Delusions can be a part of other psychotic experiences like mania, depression and substance intoxication or withdrawal.
    The delusions you see in schizophrenia tend to be bizarre in nature whereas the delusions you get with delusional disorder are non-bizarre and possible. An example of a bizarre delusion is believing that our insides are being replace while we sleep. A non-bizarre delusion is believing that a famous person is in love with you. It’s probably really unlikely, but it’s possible.
    The main criteria for delusional disorder is that you have a delusion that lasts at least one month. There are certain personality disorders where people can experience very short-term psychotic experiences that last minutes to hours. You can see this with paranoid personality disorder and borderline personality disorder
    But those very short, transient psychotic experiences would not be considered a delusional disorder.

    There are 6 types of delusions in delusional disorder.
    1. Erotomanic type. This is when you believe that someone is in love with you.
    2. Grandiose type. With this delusion, you believe that you have a great talent or insight that people just haven’t discovered yet or you’re on the verge of a big discovery.
    3. Somatic. It involves delusional beliefs about body functions or sensations.
    Common somatic beliefs are: you emit a foul odor, you’re infested by insects in your skin, you have a parasite in your body, or parts of the body are not functioning.
    4. Jealous type. With this, the person believes their partner is unfaithful. They may see minor things like you have disheveled clothing as evidence of the infidelity.
    5. Persecutory. With this type the person feels they are being conspired against, spied on or poisoned. The person may also feel like someone or some entity is holding them back from achieving a certain goal.
    6. Mixed type. This is where no one specific theme predominates. You may have features of a few of the types.
    These delusional themes can be present in illnesses that have delusions. For example mania can have grandiose delusions. With psychotic depression, people can have persecutory delusions.
    Delusional disorder may not be obvious to other people. Because the delusions can be compartmentalized in a way that you can have normal functioning in every other aspect of your life that doesn’t involve the delusion.
    Delusional disorder tends to remain stable over time, meaning it can intensify and cause problems or run quietly in the background of your mind. But, a small portion of people can go on to develop schizophrenia - not because it causes schizophrenia, but it may be more of what we call a prodrome to schizophrenia. A prodrome is an early stage of an illness where you can have some symptoms of a larger illness but not all of them. So in some people, delusional disorder can lead up to schizophrenia, but that’s not the usual.
    If the disorder doesn’t cause a lot of dysfunction, you may not seek professional help for your beliefs. Since it doesn’t make you globally psychotic and out of touch, many people will just keep to themselves about their beliefs to reduce conflict.
    Therefore, delusional disorder is something people can live with without seeking help. Sometimes the intensity of the belief can come and go such that you get less focused on it. Then under stress, the thoughts can ramp up again.

    When I was a child (in the 70s) I used to fantasize that in the distant future information like this would be readily available to me in an instant whenever and wherever I wanted. I didn’t know how. I had science books but never enough or access to enough books. I’m glad I lived long enough to experience the net and great channels on YT like this one. Thank you, Dr. Marks.

      @@anonymousmobster2444 Social media certainly has its downsides but there are may positive aspects, too. Social media tend to magnify dysfunctional or toxic behavior that is already there. OTOH, it allows people to keep in touch with family and friends, find people that were important to them that they have lost touch with, and find people that enjoy the same hobbies or have the same values as themselves.

      That's funny!! I too can remember wishing, in the 70s as a 13 year old at summer camp, that there was a way to capture all the fantastic nature and canoeing experiences I had as seen from my eyes in a moving format......did someone say GoPro?😉

    I think I suffer from persecutory delusions. I think people are constantly watching me, but not necessarily trying to harm me.

      That could very likely just be some kind of social anxiety. Feeling like people are looking at you is something a lot of people experience at some point when dealing with anxiety or low self esteem. It becomes delusional paranoia when it's a constant feeling (extending to in private areas such as in your home) and when you feel that the people that look at you are working together in a conspiracy against you

      Also, it sounds like you are too self-conscience. I have suffered from that off and on. Look up Jordan Peterson’s advice on increasing the meaning in one’s life. He describes how when people are voluntarily immersed in something productive and meaningful like helping others, their career, a hobby or even chores, they decrease their self conscience behavior and really start enjoying life more because time doesn’t drag and they feel more competent and confident.

      I worked on a goal of my choice which was trying to get better at golf so what that means is i started doing all the training and practising to get good and what it done mentally is build more confidence and which got got rid of the self awareness that comes with social anxiety. So its basically lack of confidence and self esteem. So set a goal snd work on it and you will gain confidence and your self awareness and social anxiety will not really bother you.

      It IS true that others are watching you, especially if you are presenting something or speaking. We're also all being captured on screens, ring bells, drones, satellites and CCTVs anytime we go outside. It's also true that people notice things about others - be it strangers on a subway or coworkers at a lunch table. "That's a gorgeous top, where'd you get it?" "Wow, tuna fish 3x this week, you're really hitting your new eating plan hard!" etc. This is just about survival and paying attention to our environment. It is NOT personal.
      Additionally, most decent people will ask gently if you're OK or need any help when you look like you've been crying or are overwhelmed.
      SO YES, WE ALL WATCH EACH OTHER TO A CERTAIN EXTENT.
      However, most people are preoccupied with their own image and their own goals more than they are with other peoples. For example, if you're at work and giving an update on what you did, people may be looking at you, but most people will be thinking about what they're going to say when it's THEIR turn. Even your boss, who asked for the updates, might be thinking about which items they will select to comment on, with concern or praise, rather than absorbing everything you are saying at that moment. So NO, people are NOT watching EVERY move YOU make and you are not the only one being scrutinized. Even when people ARE looking AT you they may not consciously register it: instead they could be looking right at you and zoning - thinking about what they have to pick up at the store later that day, etc.
      Knowing all this may help you feel a little less uneasy, I hope. 💞 It might even make you feel okay that you don't have to be perfect. Try this for a challenge: on a Saturday, think about your boss. What color shirt did she/he wear every day last week? How far back can you go back with certainty? What about your best friend? See? Even with the best memories of those we spend so much time with we tend to let go into the fog of the past (unless of course it, if it was a special occassion, OR traumatic event). You see, we are all the center of our own universes, in a way. 🤔 🌏

    I had a friend in high school who seemed "normal" in every way but one: She was convinced that David Bowie (the musician/actor RIP) was in love with her. Given the chance, she would tell anyone and everyone this. She also had a collection of ongoing lies around this theme; for example, she missed school because David had asked her to join him in England and help him out of a song writing rut; or she couldn't go out because David was going to call and she had some suggestions on a specific "top secret" release that was about to drop. If someone innocently said something like, "Oh yeah, I just love David Bowie!" She would say something like,"Really? Would you like to meet him? I can arrange that you know." Sometimes when she'd pull out an album of his to play, she'd hand the inner sleeve around and gush about "how exciting" that it was that David had "finally" recognized her in the thank yous or writing credits. When someone would point out that her name was not there, she'd respond with surprise, examine it closely, and say something like,"Oh yeah, you're right....this is the American release. They wouldn't let him put it on there for copyright reasons. But he still managed to get it on the UK release, which is all that really matters. Sweet, sweet David, always thinking of me." Everyone in her family knew she had this delusion but they thought it was funny and that she was just kidding around or "pretending." I don't think she ever got treatment for it.

      Thanks for sharing this. Someone close to me had something very similar about a succession of singer/songwriters--never more than one at a time, I think. Like little 'relationships' that took place in dreams; and then they would sometimes tell me about secret messages to them in the lyrics. I was very young, and found it all very confusing; was so surprised that there was a term 'erotomania' for this oddly specific thing.

      So I have this childhood friend she has been having this severe delusion of men falling in love with her (including supervisors at work, married men, men have kids) for few years. For example, she thinks her previous married manager was falling in love with her because he gave her a lot of projects and tasks to do...... I recommended her to try therapist once. After she talked with the therapist, she said it’s no use and she felt like she was CURING the therapist. I felt like it’s the same kind of delusion with your friend?

I remember she was confessing her love with one of the guys (that guy has a girlfriend and he told her about it), the guy panicked and said he thought they were friends and if in anyway he made her getting the wrong idea he's sorry. Then the guy stopped talking to her. She told me she can feel that the guy was so much in love with her but due to some irresistible cause he can't tell her....... So I agree with you, that can get really bad for those men.

Yes, when they have this Issue every single word you talked to her would be considered as “flirting” on her side. That friend of mine thinks when a guy talks to her, even if it’s a simple hello, has deep meaning behind it🤷‍♀️

    I had a somatic delusion while I was pregnant with my first son. I was convinced I had a parasite growing inside of me. I only told my husband and a good friend about it. She even joked after I gave birth that he wasn't a parasite after all but a perfectly healthy baby boy. I can laugh about it now, but at the time I had so much anxiety over it. It kind of makes more sense, now, because that's when all of my mental health issues began. About 6 months after he was born, I developed severe anxiety and depression, bad enough to be diagnosed with post-partum depression.

      I’m pregnant and started having thoughts some people are out to poison me while pregnant and it’s stressful having theses thoughts because I can’t let it go .

      Pregnancy is such an incredibly tough time. Your hormones just explode and it can have all kinds of side effects.

      From a certain point of view you did have a parasite growing inside you, a parasite is an organism that is benefitting from another organism at the hosts expense. A baby is an organism benefitting from its host at the hosts expense. So by definition you technically had a parasite growing inside you 🤣

    I went through psychosis and it was horrible. People always say about thoughts and voices and visual hallucinations, nobody ever tells you they add up and up and everything can add to it, then people dont realise that your thoughts could tell you little bombs are being implanted in your body by things out of sight but your body and brain can generate the actual feeling of needles going into the skin and little bombs going off or nerves and muscles being pinched and the sounds too, it hurt, nobody told me it could be that bad ever

      @I got yo roaches in my backseat with eren jaeger😚 when I went on medication it took a while for it to even out but I get blips every now and again where I start to slip into it, its an odd feeling and then the thoughts start going a bit off and it works its way up but so far I have managed to get it relatively under control but stress can trigger it which is unfortunate but its no where near as bad as it was thankfully, got to take it as it comes. how are you doing with it all because its defo not a nice thing to have

    a good subject for a video would be determining OCD from delusional disorder, contamination vs infestation etc. I see many patients with either psychosis or severe OCD that ride the line between obsession/delusion.

      Not a doctor, but went to therapy for OCD. OCD sufferers tend to be aware that their fears are unrealistic, but can't help the extreme anxiety that accompany them (hence compulsions). It's the awareness that these thoughts are not realistic, or that are likely false, is what determines whether they're delusional or not. An OCD sufferer can become delusional if there's poor insight and they firmly believe that their fears/thoughts are true beyond a doubt, but from what I know it's less likely, since in OCD it's commonly observed that sufferers are in fact aware of how "ridiculous" their fears are, or that their fears are just a possibility rather an the absolute truth. People suffering from delusions tend to firmly stick to their delusions and think everyone else is wrong, whereas typical OCD sufferers are desperate to seek proof that their fears are not true.

I have severe OCD (contamination and harm) and the most maddening thing about it is that I know I am crazy but cannot control my bodies triggered actions or functions or tell the thoughts NO once they get going. Yes I can logically see why a 4 hour shower is unnecessary and yet knowing that does not enable me to leave that shower.

    I can relate to the somatic delusions. I started having heart palpitations when I was 19 and since then I cant focus on anything other than that. I'm just waiting to drop dead. I'm 35 now, and that alone has completely eexhausted me.

      If I'm correct, that alone is a form of hypochondria, now called Illness Anxiety Disorder. Don't worry, I feel you. I've had palpitations since I was 15. It's been 2 years and nothing has changed.

    Your video was most helpful, Dr. Marks, in clarifying points about delusional disorder.
    We have found that many psychiatrists do not have good training in identifying and treating DD, especially those who did their training over 20 years ago. But it is so important to read a lot on it and help get the diagnosis right.

    What a great explanation of paranoid delusion. That’s exactly how my mum is. Doctors and others initially thought it was early onset dementia but I put two and two together and realised she was always paranoid, but covid iso precipitated her symptoms and magnified them m’as she didn’t life to forget the issues into. She is now wrapped in her stories… very sad to witness. At least understanding is very useful for children and supporting family members.

    It can seem like an embarrassing situation revealing a delusion to your doctor. I told him about what ended up being called paranoid thoughts. I thought neighbours were planting listening devices and listening in to me. Being suspicious of other people. He made it very comfortable to disclose. I’m on a new medication. (I have Bipolar 1 disorder) I didn’t realize these were bad symptoms or symptoms of anything until they slowly got worse and worse. Please don’t be embarrassed to tell your doctor anything, and be open. This was something I have been dealing with for a while it seems, I just never saw it as a problem. I pride myself on my insight however this one I missed and dismissed. It was hard to accept what it was but what can you do. Keep smiling everyone! 🙂

    I’m glad you explain clearly in these videos. Everybody has intrusive thoughts..mood swings..sometimes miss hears a voice thinking it was a ‘ghost’..feeling family are against you.. I guess eveyone would want at least 1 celeb they fancy to like them back of course 🤣😳it’s nothing to worry about it’s all normal but people like myself with anxiey can sometimes over think maybe this is me and when watching these videos (I’ve binge a lot of your videos for education) you can see that your not going insane,crazy or ‘not normal’ and anybody who has these symptoms can feel safe in your videos and seek help.
    I can’t imagine what it feels like to have BPD, Psychosis, ect I hope eveyone who watches these videos seeks help if you need to and always remember you’re human who is still worthy of love !

      I totally agree; I'm 43 and only heard of it recently while I've been doing this since my early teen years. For me, it is a positive experience, helping me get through my many disorders and demons. I keep reading it is something bad.

I find it helpful to have the list of all the types of delusions someone may have to not just fixate on one or tell people they're crazy when I'm not a licensed professional and don't know how many things it could be for them. But I do know it really doesn't make things better to give a possible label to an insecure person. Situations get tense and I want to shout the label because my anxiety is up and I'm hoping that knowing what the behavior is caused by leads to empathy and de-escalation of conflict. It's not an insult, but an already weak person will feel insulted and keep attacking. It's harder to ask them questions to help them look at how measurable is the reality of their delusion, especially when a jealous or erotomanic type of delusion is being shown and someone already wants to hurt me because they saw disheveled clothes or their favorite person said hello to me. Worst of all when there is also ASPD, BPD, or schizophrenia. Being told there is a point past which the police will arrive is convincing for most but not everyone.

    a clinical psychologist wrote on a paper I had all of these delusions, but also wrote that I was mentally 'fit for trial' , no one cares that I am getting harmed and then the abusers say its a delusion

    Thank you for this video. My brother is having a mental health crisis where he claimed people were trying to harm him in ways that didnt make sense to me. Hes getting mental health treatment in a shelter now. Im not sure what hes been diagnosed with, but i hope that he feels safe and will be able to live the kind if life he wants soon.

    I have a close relative and I believe has these delusions. It started with her believing she had parasites eating her brain. She lives alone in NY and we live in the south. She has been to several doctors that have explained to her she has no parasites but she won't listen. I want her to move back home but she won't. She always thinks some one is plotting on her behind her back conversations go on and on about all the people that are jealous of her and trying to get her. I don't know what to do to help her. I can't drive to NY and hit her over the head and drag her home....NY is full of people that have no one and end up on the streets... I dont want that for her....I dont know what to do....help.....😒

      Ok I know this sounds crazy but why don't you find some one to pretend to be a doctor or see if i doctor will lie to her say she dose have parasites give her some sugar pills to make them go away i know it probley wont work but at this point a placebo might work best

    Hi Tracey! Your videos changed my life!
    I was wondering if you could do a more detailed video on Somatic anxiety disorders especially what causes it.
    I have experienced many years of child emotional stress and I believe that at one stage my body couldn't handle the stress and I developed or triggered this disorder. In the past two years I was back and forth to the doctors for standard blood tests, and I even saw a specialist to check if my symptoms was related to cancer. Of course the doctors didn't find anything wrong with me which is why I'm convinced I have somatic anxiety.
    I experience the following symptoms almost every single day even when I'm not anxious:
    - dizziness (like how you described it in one of your videos where you aren't necessarily spinning but you feel like you are about to faint)
    - fatigue
    - blurry vision
    - derealisation or brain fog
    - poor concentration
    - occasionally breathlessness
    - occasionally sore chest and abdomen
    - Occasionally a choking feeling
    - Occasionally heat coming from my head
    - sweating hands
    - unexplained shakiness.
    When I am anxious I don't worry about normal things. I more overthink about my past and the actions and words of others even if they aren't bad.

    great explanation, I have a sister border line cluster B, my family tried to get an idea what was wrong with her since childhood, and then more recently ( she is now 73) all this disorganized view of the world is much more studied. We cannot handle this, but at the same time we tried both deal and keep her at distance.... She keeps thinking she is a very smart person, and can do whatever she pleases. So tiring .... we never knew why she decided Never follow the minimum rules. No medical approach in the '50s and anyway she didn't cooperate with doctors.

    My mother is 63 now. She's been holding to this belief that her neighbors are watching her every move and impeding her from leaving her life at its fullest. It does seem to come and go in intensity, and only the closest family members know about it. My mom has been exposed to abandonment and trauma many times in her life. I just wish I could help her, but she doesn't want to seek help nor accept it. She became a lot like that after getting a psychology degree at the age of 57 and having a heart attack at the same year. The woman is hilarious, yet messy. I love her anyways. 😂

    i have schizophrenia, and having delusions is one of ongoing symptoms since i have a lot of delusions and they happen almost all the time - and most of the time im not even aware that what im having is a delusion
    it was interesting learning about delusional disorder, and how it's different than my disorder. and some of it made me think about stuff. this was very informative and somewhat hitting, thank you for the video!

    I think i might be suffering from delusions😭 I always experience the following delusions: Being watched through cameras, people plotting against me, things i say will be used against me and the things i do on my phone being monitored and sent to my mom one day💀

    Ex husband had a few of these in different categories. He could not even consider that his accusations weren't reality. But he was on a really bad drug binge; he seemed to know his beliefs were socially unacceptable and would hide them, before the drug use got bad.

    My late husband was delusional after his TBI. The subject of the delusion changed from hour to hour, but they never really went away. He once told me that the nurse at his assisted living facility came into his room the night before and replaced his eyeballs with a new set, and this new set just wasn’t as good as the originals. It was horrifying and heartbreaking.

      Yash Sharma honestly it took YEARS AND YEARS before I found out. I have a psych, and he gives me meds to control the disorders I have. I rarely take them. I only take my sleeping meds. I will pray for you and your brother!

    I'm so glad you uploaded this video Dr. Tracy. I have a patient who I believe suffers from Delusional Disorder. I believe she is a somatic and a persecutory type (mixed). She is always worried that she is sick and always make appointments to check her chest, her heart etc and she is also obsessed with thoughts that people might hurt her or that they re making a conspiracy about her. Two years ago she was prescribed with Villamos. I would love to know your opinion regarding treatment.

      I had to look up the Villamos because it’s not used here in the US but the closest thing to it is Olanzapine which is an antipsychotic medication. In my experience it’s been fairly hard to treat, meaning it doesn’t seem to respond as well to antipsychotic medication. I think sometimes people can improve but it seems like it’s pretty difficult to make the delusions go away completely. That said, I have not treated a lot of people with delusional disorder because they often don’t come in for treatment because their delusions typically don’t cause a lot of problems for them. But when people have a lot of somatic delusions they can become pretty preoccupied and impaired. I hope the olanzapine at least helped her focus less on her delusions

    I worked for nearly half-a-century treating those with severe Mental Health and Social Functioning issues including chronic serious Criminality, and Addiction. I specialized in working with Personality Disorders and other severe cases such as treatment resistant Depression, Anxiety, Bipolar, etc. I was retired "early" due to medical condition, so for the past couple of years, I have a lot of time of my hands. I started watching "Court TV." Not the Reality shows, but real Court Cases. While the general theme routinely revolves around drugs and alcohol, * * Personality Disorders (PD's) are on full display.* * These cases are also good demonstrations of why, when it comes to PD's, Dimensional Assessment is the Gold Standard.

    I did have for a short time , very hard time, persecutory delusions, yet at the same time i was very aware that the emotional stress was the reason for those thoughts. It was so strange, feeling acutely that people was "talking about me on my back" , and at the same time telling myself "that is not true Clara". This lasted about 2 years. I cured myself ....

      @@allypat3002 I had to tell firmly to myself that it was just a suggestion coming from my mind. It was not "nice" , i needed to be disciplined. But not too hard. The reason I was in that state, was much worse. Sometimes I would feel like "drunk" or "unbalanced" . But I do think it was much more how i perceived it, not really external, because I used a bit of heels .... so it was not "physical" , it was all perception, That was also the time I had dreams and then things happened , total nuts . The great fire downtown Lisbon , for instance ... 1988

    You just solved my 8-year-old 'journey' in psychiatry. I might be dead now if I hadn't looked up my illness on the internet. Years of antipsychotic medication yielded no results. Before finding that video, I studied enough to understand. I've been on antidepressants for half a year now, and I feel better than I have in the past 10 years. Psychiatry is terrible in my country (ua).

    I have a neighbor who just suddenly decided she hated me. I used to drive her everywhere and help her out whenever possible. Then one day she just stops talking to me and won't even look at me. I thought of asking her, but her behavoir was hostile, and she refused to say why to a few of my neighbors. I have no idea why she was angry, but I also think that that was a good thing because she always talked badly about nearly everything.
    I feel sad for her, but I also feel like she's miserable because she just likes being miserable. She constantly tries to turn any pleasant conversation into a complaint about something. I do feel sorry for her, but I'm also a little relieved that I don't have to talk to her anymore. I wish there was a way to help these people not feel that everyone is out to get them. It's heartbreaking to see the effect it has on their lives.

    I know you can’t get through all questions but I’ll still give my situation a go.
    I started to have a lot of irrational fears since the time of watching a scary video a few of my classmates in 5th grade coaxed me to watch. Since then I was afraid of a certain being that would hunt me and try to kill or posses me or my family. Because of that I wasn’t able to sleep on my own and I needed to stay near my parents for comfort because when I was alone I believed the being was near me. I couldn’t go to certain places of my house like the kitchen because I was convinced he was there. I would get panic attacks and call my family members to come over because I was just that afraid of smth I don’t even see.After about a year of not sleeping on my own I was finally forced by my parents to sleep in my own room which led to me finding distractions to preoccupy myself until 5 am which I believed to be the time that the being wouldn’t touch me because it was too bright and early. This ruined sleep continued until now , I’m currently 15. I still am somewhat afraid of the being but now I’m convinced that someone is following and stalking me. I hear weird sounds and always check to see what’s wrong and get very anxious and leave the area and sit in my room. I am afraid the person I think is following me is looking at me through the window and recording me. I see running shadows from the corner of my eye thinking it’s the stalker and I also see hallucinations of bugs which I sometimes swat and realize they disappear before I swat them. I have intrusive thoughts of ppl I see on the street , of them doing smth to me, for example if they open their car door I’m convinced they do it so that they can strangle me and kidnap me. So it’s weird I don’t know if this delusion is just part of me being a teenager or something. I would like to know what is going on. Thank you

    I would love to speak to you on this subject. Can trauma cause temporary delusions? I was delusional for over a year and forgot who my family and fiance were then randomly one day I "got my mind back" but by then, my life had already been ruined.     Hello Dr. Marks :) I've missed the premieres of your last (two, I wanna say) videos. When you post these videos I often hear you say something to the effect of "This video is based on a user question." I have one :) We don't hear much as clients, at least in my experience, about serotonin syndrome and those at risk for developing it. Can you talk about what it is perhaps, and what comorbidities and concomitant therapies increase concern about developing it... is it an emergency? and/or associated with NMS or, differently, metabolic syndrome -- anything at all that covers it would be really cool to learn about. Thx.

    Very interesting video! Can delusions be about anything or do they have to fall into these categories? You mentioned mixed type, but are those just features from more than one of the other types? Sometimes when I'm depressed I can get convinced that everybody hates me and that I'm the source of everything bad in the world, but that's not a delusion is it? At least not like these ones. Or take body dysmorphia, where you have false perceptions of your body that very negatively impact your life. That's kind of in the same vein, but not classified as a delusion (I don't think?). Where does the line go?

    I think a video solely on Cognitive-Behavioural Therapy and Rational Emotive Behavioural Therapy (might be the same thing?) would be helpful for me. I have severe health anxiety; I obsess over my physical state and health nearly all the time. You made a great video about this disorder, but I hardly understand how CBT will work for me, considering how my disorder manifests itself.
    For example, I fear having a heart attack and facing an untimely death. Thanatophobia is a big factor for me as well. In response to my fears, I am heavily restrictive on my diet, avoiding all fats and cholesterol for fear of giving myself heart disease. I avoid excessive movement, because the increased heart rate freaks me out too much for me to handle.
    How would a professional even begin to tackle these problems with CBT or REBT? It seems so impossible at times for me.

    Can you post a video about Bipolar Psychosis specifically? Including both clinical symptoms (positive & negative), cognitive & functional impairments. I was told by my psychiatrist that delusions and hallucinations of BD are more based in reality and that they are different in some ways than Schizoaffective or Schizophrenia psychosis. I also liked the maladaptive daydreaming idea.

I would love to hear your thoughts but I understand it's a very big topic right now and very hard to explain for anybody to actually want to understand.

      I think to call racism a mental illness trivializes mental illness. Hate and bias are emotions and preferences, not illnesses.

      @Dr. Tracey Marks It also seems to me, as just an observer and not a clinician or doctor, IMHO, that hate and bias are learned and can therefore be unlearned, which is hopeful. Mental illness, on the other hand, has an organic component to it. Certainly traumas, learned behaviors/modeling can contribute to its acuity, but the organic components can only be solved through either ECT, neroplasticity reprogramming techniques affecting the symptoms
      (e. g. Thought stopping, deep relaxation, etc.) and medications, especially in severe disorders like psychosis, dissociation, and depression. Thoughts?

      @@notpub although I kind of agree. I believe most mental illnesses are caused. Especially in the case of dissociation. I understand a lot of the biological factors that can contribute. But! I believe most of the main factors are external from the environment that can be retrained or even mostly healed without damaging the brain further.

    Dr Marks,
    I’m off of the vraylar. As you said in another video that blocks age1 receptor and some other receptor for telling you when you stop eating mine were clearly malfunctioning because I gained 30 pounds on vraylar. I’m not taking Lamictal chewables 25 MG and I cut them in half once again crazy side effects. But does not have the appetite problem the video did about the antipsychotics with the receptors was the best video I watched you ever did . Thank you for pointing that out did not know about that I’ll try to find the video and comment on it.

    Thanks for this on the Somatic. I was wondering if there is a link between Somatic Narcissism and OCD? I notice a lot of these types seem to have extreme OCD. But another article described Narcissism as 'Whole System OCD'? Helpful video. Thanks ; ) K

    Dear doc, in the past you have brought up about how THC can be a culprit for psychosis. As someone with unipolar depression and PTSD, I personally feel THC helps me cope with symptoms of both my depression, helps prevent flashbacks and can stop them when they happen. So what I guess I'm asking is what is your opinion on THC/Marijuana, can it work somewhat as an anti-anxiety and/or an antidepressant, and in what ways does it lead to any possible psychosis? I also take an antidepressant everyday and a benzodiazapene for moments where I can't just be lighting up smoke around people. Thank you for your videos they are very informative!

    I have a friend who is a girl and, (I think), has this.
    She is always grieving some "relationship" that hardly ever happened or was very brief, and cannot let go.
    Im really sad for her , tonight i told her that i cannot take it anymore, because, (and this i didn't tell her), it is just too sad to see someone like this, and to see that no one is doing anything about it.
    She told me she will be fine, but i know she is hardly ever really gonna get better.
    Before that i had never met anyone that had this. It is shocking to see. Plus society doesnt seem to detect this type of thing.

      This is currently what I am going through with my friend right now and I really need her to get help because it’s becoming hard to support her as a friend especially when I have so much things going on in my own life . I am sad for her but I can’t do it anymore

      @@emmanuella8269 In my case, my friend was "saner" than I originally thought.
      She IS now with the guy i thought was only leading her on. I still think he doesn't care much about her but yeah.
      So yeah I wasnt totally right.

    I’ve experienced this pain for a while now but I’m on a cocktail of drugs to quieten things down...I’ve been diagnosed lately with eupd and COVID-19 has interrupted my day to day routines...life is hard enough without Covid urgh! Too much time on my hands and sleep disturbance doesn’t help with my comorbid diagnoses...😕 dr marks’ vlogs on here are very helpful especially at this time of confusion and uncertainty as I’m unemployed but watching these uploads weekly actually gives me a sense of hope that Recovery really is possible and building a life worth living is within arms’ reach...do I make any sense at all today or am I a bit delusional? All replies are welcome 🙏🏻 cheers!

    I've really been appreciating your videos they give me quite a bit of insight into Mental Health. Does everyone in the world have some sort of chemical imbalance that makes them have some sort of disorder or are there people who actually live without chemical imbalances?

    i thought i might had delusional thoughts, but i cant really relate to any of the categories. im depressed and when i have bad days where i will break down and cry for hours, thats when i experience delusional thoughts or whatever it is. i am then convinced that everyone hates me and that my life is horrible and i will never be able to get better. i will also be convinced my boyfriend wants to break up with me and i will now be alone forever. many other things too but i have a hard time remembering what i think when im in that state. it maybe happens a few times a month where i have delusional thoughts for maybe some hours or a day or an evening. but last week i had been really really down and without even noticing before i got out of it, i had been stuck with my delusional thoughts for more than a couple of days and unfortunately i took some poor actions because i did not realise i wasnt thinking straight. other than that i am constantly, and not just sometimes, thinking my mom can read my thoughts. whenever im in a room with her and she isnt speaking to me im convinced its because she is reading my mind so i will repeatedly think in my head 'stop reading my thoughts, stop reading my thoughts, stop reading my thoughts'. i dont know what any of this means and i am gonna talk to a therapist but the waiting time is one month so i thought i would share here for some reason....

    My first manic episode was very very hidden. I was convinced i had Alzheimers, which my doctor thought was just anxiety. What made it a delusion was the fact that I believed it and didn’t know why.
