  • When a bone fracture in the femur, tibia, leg, forearm, arm, or pelvis, a metal implant is implanted. These are plates, screws, nails, and wires. They are titanium or stainless steel.     I had all the screws and plate removed from my ankle…wow, what a difference! No more pain or bone burn. Its like walking on air. I am finally walking with no limp and was fully weight bearing immediately, no crutches needed! There is hardly any bruising or swelling and l can truthfully say l havent taken a single pain killer since the recovery room! So glad l had the metal work removed…best decision l made! Two weeks on and with the stitches removed today, l am back walking my dogs, obviously only short walks on flat paths but l am not limping and in no pain…nobody can believe l have even had surgery!

    Why Fins remove implants routinely while UK doesn't? The reason is Finland has a better health care system. It is mostly about money.
    In US, implant removal is not strongly advocated because of the cost as well. If cost is not a factor, most implants should be removed to allow the bone to heal naturally. I hate doctors using cost and hospital time as reason to deny removal of implants.

    I just had a Bimalleolar fracture from my soccer game…
    My doctor recommended not removing the small plate and I think a screw!
    I take no pain medication. It feels good. Just had surgery eight days ago. !

    Thank you so muchfor the video. i just had a distal humeral fracture reduction surgery with plate and screws and I needed some info on the subject. I was worried about it so much and I ave radial nerve palsy as well and don't know how to handle it. It's just overwhelming

      You are very justified in your concerns. The radial nerve passes approximately 8 cm above the most protruding bone in the elbow, and this nerve can be at risk when implants extending to this area are removed. However, an experienced physician can remove the implants without creating a significant risk to the radial nerve.

    i got a arm surgery 5months back i just afraid to the hard wear to my cause of any kind of miner harm but I'm also afraid to operate again what should I do and after how much time i remove my hard wear they are on place right know the vedio is helpful ❤️

    I have removed my implants after 9 years even though my doctor not suggested as x-rays showed a picture perfect in side. I am eagerly waiting to remove my stitches by 6th July 24

      @@nadiaqutab1098 it seems fine now, but started normal activities, limbing walk now. Still pain inside. I had 2 plates, top and bottom of knees. It seems it will take 2-3 months for full recovery

      2/3 of patients who had problems in removing the hardware would do it again because for them the thought of a foreign object in the body is worse than the damage.

      Doctor, i will cut and realine my proximal falange pinky toe. After a malunion, i had an accident.
      I see that for keeping in place toes k wires are usually used.
      I don’t feel comfortable being drilled through the joints i must say.
      I prefer mini plate and screws.
      And then have them removed.
      Would you agree?

      First of all, thank you very much for your interest. Even the smallest plates and screws may seem bulky for the pinky toe. After the procedures they should be removed. K-wires are generally more suitable for that area. When removed after six weeks, nothing remains inside.

    Is it normal to feel a kind of freezing burn pain where the screw and plate is? I have a Trimalleolar Fracture. Thank you for your help and this video! I wonder if I need to ask to have mine taken out. I have 2 plates and 10 screws.

      In fractures of the malleolus, the implants used can cause discomfort because they are right under the skin. Your desire to have them removed can be quite reasonable. Since their removal won't be too troublesome, it is appropriate to request this from your doctor if not much time has passed.

    I have 6 pins and screws in my right ankle. I dislocated and broke 3 bones. Going on 3 yrs now. Im 69 female . Very active. I walk daily, usually 10, 000 steps. I slow run 5Ks . About 1 a month. Lately my ankle hurts. A lot with much swelling. I wonder if I should think about having them removed. My ankle doctor thinks I should . I'm not sure.

    I have femur fracture in 2021 and orif was done for intertrochanteric fracture for right proximal distal femur. Xray shows all good with bone but I always fee disconfort and physical therapy not helping well to get back to sports. Do we have a way to know if the screw holes will fill up? I believe there is some scar tissue buildup in leg . will they remove scar tissue from leg during hardware removal?

      In radiological examinations, what we see is not just the healing of the bone structure, even if it appears to be healing well. A fracture is considered fully healed not only when the bones have mended but also when the alignment, rotation, and length of the fractured bone have been fully restored to their normal anatomy. Meanwhile, the damage caused to the soft tissues by the fracture or surgical procedure is a separate issue. While you can remove the scar tissue that forms in these areas, it's highly likely that similar scar tissue will reappear in the same spot.

    The fact that 2/3 of people with removal complications were happy with the procedure means their old implants were causing them serious problems!!
    Is there proof that titanium plates cause less osteoporosis than steel plates?

      Yes, stiffer metals are considered more risky in terms of osteoporosis because they reduce the load on the bone. So titanium is considered less risky in terms of osteoporosis.This is an accepted fact.

    Hi. So I just got surgery yesterday’s afternoon of ORIF in my fibula. 1, When do you think is the best time to get the hardware removed? Cause I’m thinking of removing it 2 years after. 2, Would removing the hardware lead to Arthritis in the later future? 3, Would the holes from removal fill it’s self back up?

    Thank you for a helpful video. I am due to have the plate in my wrist removed 12 months after injury. I am 77 years old and broke radius and ulna when I had a fall playing table tennis! Five months after my accident I have intermittent pain, and supination is virtually impossible despite lots of physio. Barring complications, I wonder how long it should take to recover from the surgery i..e. how long I might be in hospital (in the UK), and whether I would be in plaster again or have a splint.

    I had a really bad head injury from a motorcycle wreck where I went into some rocks and bricks head on without a helmet in 1971. I'm 67 now and it's been 40 years or more now I seem to be getting what feels like something trying to come out of my skull at the same place where the rocks went in and it hurts sometimes I just wonder if there's something still in my head driving me crazy

    Amazing and very insightful video. I have an internal fixation carried out on my right elbow ulna as I had a fracture of the olecranon process of the ulna 7 years ago, currently feel a discomfort around the region which is likely caused by either a pin, wire or plate which is pinching my skin from the inside. Also there is a noticeable bulge on the elbow, do you suggest to remove the implant after 7 years?

      Thank you very much for your interest. Implants used in fractures around the joint and especially in the elbow can cause discomfort when putting the elbow on the table. It is often necessary to remove the implants in this area. Seven years is a long time. Removing the wires does not cause any problems, but the plate and screws used may cause problems. It is useful to discuss this issue in detail with your doctor. Maybe if the plate and screws are in a position that will not cause much discomfort, only the wires can be removed. Have healthy days.

Had no issues carrying the implant until recently that I noticed a bruise on the tip of my elbow where the internal material pinches the skin and felt it might be time to recheck the fixation to determine how to reduce the discomfort.       Of course, I don't know your x-rays and haven't examined your condition, but as enough time has passed, I am sure it won't cause a significant problem, so I assume it's okay.

    I have a humerus fracture due to an accident in 2020, but the bone did not heal after 6 months, and the screw in the plate was loose. My doctor suggested doing a second surgery and putting much bigger and stronger metal in 2021, and now the bone has already healed, but I always feel pain and discomfort. The pain feels like my first fracture, and I feel like I want to remove the hardware because every week I will feel pain. Do you recommend it, doctor? Your answer is much appreciated and will help me a lot

      Thank you very much for your interest! I can't know the details because I haven't examined your X-rays or conducted a clinical examination. However, a fracture that hasn't healed in six months doesn't typically heal on its own. Six months is the necessary time to say that a fracture hasn't healed. If there is still pain, it is possible that it hasn't healed, and if the source of the pain is indeed the unhealed fracture (If the source of the pain is not your shoulder), it may require surgical intervention to mend the fracture line. Nevertheless, the final decision can only be made by your doctor after conducting your latest examination and reviewing your tests.

    I had a right collarbone titanium implant yesterday. Rn in a very uncomfortable situation but hope it heals soon. I'm asked to keep the implant but what's your view on keeping a collarbone implant? Please do reply

      Plates and screws placed in clavicle fractures can be left in place if they do not cause any discomfort. The risks of implant removal in fractures of this region are the same as described in the video. On the other hand, since the collarbone is just under the skin, it may be uncomfortable for the patient and in such a case, the patient's request for removal may be reasonable. The current complaints are related to the recent surgery and all of them will be gone in a few days. I think you have at least a year to make a decision. You will have time for evaluation.

    Should I remove my clavicle plate if my shoulder muscle feels tight? I cannot tell if it’s because of the plate of if it’s my post injury muscle problem. I have a plate and screws implanted on my left clavicle after a bad fall injury from basketball, 9 months after the implant surgery now.

      If the fractured bone is painful because it has not fully healed, or if it causes discomfort because it is too close to the joint, different treatment methods can be considered. This may include removal of the implant. It can also be removed for cosmetic purposes, but sometimes if there are such problems due to the damage caused by the trauma itself to the shoulder, implant removal surgery will not work.

      Your doctor is absolutely right. Anything can happen after ten years. In one of our patients, we had to cut it in half and leave it in place because it did not come out more after the nail in the canal was halfway out. Moreover, the operation, which we planned for 15 minutes, took 4 hours. If there is a problematic situation, let's make a profit and loss calculation, you can still try to remove it, but all the facts in the video apply to you

    My Ortho really really did not want to take all mine out but they bothered me especially considering I raced motorcycles and could easily brealk legs again which I mean could be super bad if hardware bent etc... so he relented and agreed it should all come out I had 22 screws 2 plates and a rod removed my two legs tibias felt much better after and have not been fragile. But I was youngish I think 35ish? The only thing still left is a broken drill bit the original surgeon possibly did not even realize he broke? Or else just neglected to document for whatever reason...stange looking xrays now with this drill bit just suspended in the bone Anyway the only bad thing about hardware removal was dr wanted me back on crutches for some weeks ...which mybe was overkill idk but I tried to obey somewhat i took a week or two vacation from job in trades and after that mostly just used a cane ... it all felt good to me but I tried not to take chances....eventully I was back racing and regularly crashing 500cc Moto Cross never broke legs again knock on wood

      This is intersting to hear. I am nearly 35 and just had tibia plates put in at the bottom near at the ankle. I race motocross and want it all taken out in a years time or whenever. I have read som much on peoples comments and this one sounds very close. I work a manual job outside and want to continue doing so. I just want my ankle as back to normal as possible. God I;m gonna exercise that hting back to life if i can get it out. Doctors so far want it to stay in as im UK and they dont want another surgery but i am more than happy to take the risk, omg. I dont want it in there when im 70 +

    Hi Sir, How about calcaneus implant. Whether it should be removed or stay? It's a titanium. But Swelling and pain is there.

      In calcaneal fractures, pain and swelling can occur due to arthrosis in the subtalar joint, which is between the calcaneus and the talus above it. Generally, these symptoms are not due to the implant itself. However, the most accurate answer can be obtained from your physician.

    Sir, will it be a problem for a drill-bit to be in the bone after surgery. Doctor told me that there is no need to remove it.

      Yes, such things can happen. If the drill bit is inside the bone and does not cause any discomfort, it can stay there.

    What if I had a fractured tibia and will need a TEP in 20 years? Will it be a problem to remove it in 20 years or should it get out soon?

      20 years is a very long time, and anything can happen. However, my recommendation is that it's not quite right to think about whether you should remove it someday in case you need a Total Endoprosthesis (TEP) if there's no current need. Such a need should be considered if and when it arises.

    I currently have two titanium plates separated by an 8 mm polypropylene spacer in my right ankle---.Yikes, I'm a cyborg! This surgery was preformed in Jan of 2019. I've been told that eventually this (For the lack of a better wording.) artificial joint will fail. Is there any truth to that? If it fails I would almost certainly be facing surgery number five on my right ankle. I do not relish the thought of another surgery.

      What you describe sounds more like an ankle arthroplasty than a plate. Unfortunately, arthroplasty of the ankle using our experience with knee and hip arthroplasty does not give as long term results as knee and hip arthroplasty. It is impossible to know the duration. Although there may be a possibility of wear within five to ten years depending on the activity level of the patient, the process may take 20 years or more depending on the success of the procedure, the activity level of the patient and the quality of the implant used.

      The plates and screws are most likely not platinum but either titanium or stainless steel. Titanium is more inert, but nickel in stainless steel is the most allergenic metal. A so-called delayed allergic reaction may occur due to the implants. It is very, very rare. These patients may also be allergic to metals such as earrings and watch cores. However, if this is the case, removal of the implants may cause the symptoms to disappear.

    Great informative video, i had dynamic hip screw surgery 4 years ago, am still experiencing sharp pain in quad/thigh area just off centre to front, also occasionally in groin area and outside of hip( close to implant) could this be nerve damage after all this time?

      No, there can't be any significant nerve injury during the insertion of DHS plate screws. On the other hand, about a dozen problems can be considered as causing pain. You can revisit this issue with your doctor during your follow-up appointment.

    After 20 years, I’m getting my surgical screws removed. I’ve been sick those 20 years and it started 12 months after they replaced. I became chronically ill because I was allergic to the metal.

      Very, very rarely there may be allergic reactions to the implants used, especially to nickel. They can even occur in areas far away from where the surgery is performed. Nevertheless, there is no clear opinion because the standard for this is not very clear.

      Once the fracture has healed, it is safe to carry on with your daily tasks. The healing process may continue for up to 2 years after surgery, during which the bone undergoes remodeling to adjust to the stress placed on it. After this period, the bone will function almost like a normal one. However, the screws at the far end of the implants can cause increased stress on the bone. As a result, I recommend that you put slightly less weight on your operated arm than you normally would.

After orthopedic implant removal, the body does start a process to fill in the screw holes left by the hardware, although it varies from person to person based on several factors. The process is called bone remodeling and involves both the removal of old bone and the laying down of new bone. This process is usually facilitated by weight-bearing and load transmission through the bone. However, the bone may not regain the exact same structural properties as it had before the initial injury and surgery. Sometimes the areas where screws were located can remain a little weaker than the rest of the bone. As for your specific case of pain, while lifting heavy weights, it's essential to remember that bone healing and remodeling can take time. There may be some discomfort as the bone continues to heal and adapt. On the other hand, it's not sufficient for the fracture in the bone to have just healed. It should have healed in its normal anatomical position. There could also be problems later on with intra-articular fractures. If such a situation exists, the pain that arises later on may not always be due to the implant, and complaints may continue even after its removal. As you return to the gym, gradually increase the weights you're lifting to allow the bone to adjust to the increased load. Ensure you maintain good form to avoid undue stress on your joints and muscles. Also, consider working with a physical therapist to guide your rehabilitation and strengthening program, ensuring a safe and effective return to your pre-injury activity level.

      @ i’ve had my implant since september 2022 so i’m growing frustrated - i don’t know what’s going wrong but i’d take a chance on implant removal down the line if you can tell me that the holes can potentially strengthen to the point where it’s strong like pre-broken bone, i feel i can heal strong in my mind, i just don’t like the tension the plate and screws are giving during consistently going heavier in weightlifting - it’s not like excruciating but it ruins my workout as i can’t complete it sometimes as it feels janky and sore near the joint where the screws are

      The success of surgical procedures naturally depends on the experience of the physician and the method used during the operation. If the surgery was performed gently and without damaging any tissue, it usually takes up to three months for the bones to fuse and regain their strength. However, in a scenario where everything goes wrong, the process will be much longer.

    Great video. Had a distal radius and ulner fracture with volar plate and screws.
    Now one year out. Still experiencing numbness and pins and needles in thumb and index finger. Experience pain on wrist….at base of thumb. Feels like something is blocking my motion with pain.
    CT shows all the screws are interosseous with the exception of the medial styloid. This is posterior at the radial styloid probably by about 2 mm. Dr. Said screw could be irritating tendons. He has only removed one patient’s hardware before. I would like to get hardware out, but am afraid of more damage? Perhaps I should seek a doctor who is experienced removing plates? Please advise.

      First of all, thank you very much for your interest. The numbness you described could be the result of the nerve that senses this area being strained or injured during the surgery. A protrusion of two mm from the bone usually doesn't pose a significant problem. Sometimes, after such a procedure, a condition known as "trigger finger" can occur. You might be experiencing something like that. Nevertheless, I don't know the direction of the screw around the styloid, but yes, it might need to be removed if it could harm the tendons. Your doctor can make that assessment. Everything I've mentioned here is not medical advice but merely a remote observation. The recommendations of your physician are much more valuable.

      Wow, I have almost the exact same issue and diagnosis but I’m 12 years post implant install. I have removal surgery scheduled 4/30 and one surgeon is ok with removing it, another stated crossing tendons could be the reason for the shooting pain, and won’t do the surgery due to “high risk and not making anything in the end”.
      @carolegates6480 did you end up having it removed?

    How likely will your immune system all of a sudden fight a dormant implant that has been there for decades? Like fibrosis or fibrosis encapsulment happening out of the blue on a scoliosis implant in the spine that has been there for 40 years?

    My father of 50 years had a surgery due to bike skid accident, in which his ulna and radius of left arm are broken. During surgery he was fixed with plates and screws, after that he was fine. After 8 years, we planned to go for removal of plates and screws. After removing, even after six months, he couldn't open all fingers fully. Mainly, he couldn't move and open the thumb. What could be the solution for this? Please reply .

      I do not recommend the removal of any implant, whether titanium or steel, that does not cause discomfort. Still, the decision on whether there is a need for removal should be made together with your doctor.

    Hi my husband had a fall and his patella was fractured. He underwent a surgery with a titanium plate implant. Within 7 days he was infected with a bacteria called klebsiella pneumoniae. Again underwent another surgery for draining the infection. After antibiotics he was fine in 3 weeks. And was able to walk with full weight. After a month his leg was twisted while walking and again the same patella was fractured. Underwent another surgery and two screws from the plate were removed and anchor sutures were fixed.. After a week again infected with the same bacteria and another surgery is done for wound debridement and removal of the implant is done. Will this cause a problem?

      This is definitely a problem. The natural process of the procedure did not work and the method was changed before the result was obtained. It is impossible to predict the outcome.

      It would not be very accurate to say a specific age because each child's growth is completed at different ages. In fractures before the completion of growth, the implants should be removed. In general, it is necessary to remove before the age of 14 for boys and 12 for girls.

    Hello sir i had clavicle surgery and titanium plate is installed its been 20days of my surgery...And i m feeling tightness and a bump on the clavicke bone...is it normal?...and do the titanium plate dissolves with bone because doctors said it doesn't need to be removed...Please help sir I m a student And i m very much worried

      Tightness and swelling may be normal and will resolve over time if nothing is wrong. On the other hand, the implant, even if it is titanium, does not dissolve and stays there. The details about removal are explained in the video. My personal opinion is that it should not be removed. However, since the clavicle is a subcutaneous bone, it may need to be removed if it causes discomfort.

      I had clavicle injury and surgery, it takes a little time to move it normal again but it should work. After your initial healing, your physician probably will teach you to do some movement exercise or do physiotherapy to gradually move the left arm normally again

    Watching 6 days Post Op hardware removal for a bad tibial plateau fracture that I originally got surgery on 18 months ago.
    The kind of pain I was feeling is different than the pain that hardware usually causes so they were skeptical for a long time that it would help. After lots of tests and scans and finding nothing wrong with my surgery that everyone said looked like it was done very well, I found a few orthopedists that were all for taking the hardware out.
    Still recovering, but I'm very optomistic!

      barring infection or you doing silly stull before it all heals? It should be much better without that crap in there if it bothered you IMHO based on my own experience having 2 plates a rod and 22 screws removed from tibias including ankle

      There is no scientific basis for what you're saying. Nowadays, many people in their sixties and beyond are living with knee, hip, shoulder prostheses, or dental implants. If these were to be removed, they couldn't continue their lives, and these are made of metal.

      @@SimplifiedOrthopedics Of course there is no scientific basis. The science is corrupted. The medical industry would never implicate itself. When there is a problem, they won't admit it, they quietly make changes. That's why they moved away from stainless steel in favor of titanium. They are currently trying to move away from titanium with materials such as carbon fiber, ceramic, and even dissolvable implants to mitigate the problems of foreign objects in the system. Metal on metal hips are being replaced...why is that? How about amalgam fillings? So many dangerous medical procedures and medicines throughout history, but the good doctor here thinks everything is finally sorted out....not likely.
