It’s not really a stereotype, Brummie is just someone from Birmingham. Anyone with a particularly regional accent is ripped on in the UK for not speaking RP, it’s a ‘dumbass hick’ thing in a classist society. Edit to add, the joke is that someone from Birmingham is saying Astarion lines that only really work in his theatrical accent.
I'm a Brummie, and our accent is consistently voted as the worst in the UK and regularly ridiculed - although a lot of people mix up our accent with the Dudley accent (a neighbouring town). It's often believed that we sound stupid, but that predominantly comes from outdated classist views (as Brutishandshort said). Historically, Birmingham (and the West Midlands as a whole) was a very industrial area, where most of the jobs used to be factory or manual labour based, so that heavily influenced how we were/are treated or depicted. It's not quite the frat boy/valley girl kind of "stupid", but probably more akin to the hillbilly/redneck stereotypical "stupid".
It's just the dichotomy between Astarion's upper class High Elf appearance, matched with the perceived "lower" working class Brummie accent. Class is historically a big deal here in the UK, and accents are intrinsically tied to that. If you're American, it's like how people from the South are treated like they're stupid just by virtue of their accents.
