Well, the whole Romeo and Juliet concept works because the song is about being in love with drugs or alcohol, and it slowly killing you. Romeo and Juliet died from being in love with each other. Even though the drugs are killing the Patient, he still does them knowingly, which fits "lather the blood on your hands".
Its starts off building tension and then holds it in and releases the tension, making for a very satisfying pre chorus, especially it's right before the chorus
I feel like there's one line in the song that just explains it. "Juliet loves the beat and the lust it commands". The music is just so raw and lustful and sexual (without actually being sexual) if that makes any sense. It kind of tickles that animalistic urge in us and the chaos ensues. Its just a beautifully written song.

This song is about the patient being a rockstar in his past life. ( this was an artistic choice as gerard, being a rockstar who underwent a drinking problem could relate to) The patient was inspired by the marching band when he was young, leading to his life of music, which hasnt treated him well at all. He follows the black pardade in the afterlife because the parade is very meaningful to him.
