  • Side effects of steroid shots in the knee

    Osteoarthritis is the leading cause of disability in adults and affects over 50 million people in the US. The knee is the most common source of disabling pain caused by arthritis.

    One of the hottest topics of discussion with patients is the use of steroid (cortisone) injections. People always want to know how they work? Is it safe? What are the side effects?

    Pain relief from steroid injection is thought to work by reducing the inflammation in the knee.
    A common recommendation by providers for people with moderate to severe arthritis is that injections can be performed 3-4 times per year (if they are successful @ reducing pain). Keep in mind this is not recommended for people with healthy normal cartilage. So if you are a young athletic patient, this is not an Injection that should be used frequently.

    Several studies have shown that steroid Injections have been successful at reducing pain in the short term. The exact duration of pain relief remains UNKNOWN. Several studies showed a definite improvement at 1 week, but there is no evidence that these injections are demonstrating benefit past 6 months. Additionally, there is evidence that in obese patients the injections are less effective.

    Steroids are not recommended in people with the following conditions:
    allergies to corticosteroids
    an active infection
    difficulty clotting
    Compromised skin
    Fractures
    Presence of knee implants
    Labile diabetes
    or if you are having a knee replacement within 3 months (higher risk of post op infection)

    Some Adverse effects include :
    Skin pigmentation changes
    Fat or skin atrophy
    Residual injection site pain
    Facial flushing
    Increase in blood sugar
    Infection (studies range from 1/7000 -1/50000)
    Note: Studies demonstrating a toxic effect to cartilage have been shown in laboratory studies but not clinical studies. The clinical data showing local anesthetic has effect on chondrolysis is shown in continuous infusion of GH joint, not one time injections.

    I have been in pain for well over 6 - 8 months in my right knee with OA. I will have a knee replacement within the next 6 months. I received my 1st cortisone shot yesterday and WOW the pain that I have been living with is 95% gone! I am so happy that I received this shot and I can say that my life has changed!! We are going on vacation in 10 days from now and I will be able to do the things I want to do!!

    I had an injection in my knee this past week. I’m almost 71, had the other knee replaced 14 years ago. Doctor said we’re buying some time before the knee is replaced. It does feel less painful. I’m very active, gym 6 days a week, walking every evening.

    Thanks for bringing up this topic! I am 61 yrs old and just had injections in both knees two days ago. I am experiencing chills and kind of fever feeling in my face and arms are very hot. The pain have reduced a bit but I was told to give it a 3 weeks to kickstart. I am feeling hopeful, thinking that after 3 years enclosed cause by the excruciating knee pain I may will be able to finally start to get out of the house now to get on with life. 🙏🙏🙏

      Well I just got an injection steroid shot in my left knee 3 days ago & I'm in just as much pain now as I was then. So many people get relief right after they've received their shot, and I'm happy for them, but I'm so bummed for myself. I thought this sht would really work.

    I have resisted cortisone for all these years. Last week I finally gave in and got an injection in my knee. The first two days were wonderful by Monday I wasn't doing so well. By Tuesday I had to reschedule two appointments because I was in so much pain that my nerves were shot. My knee was full and tight and I could hardly even bend my knee. Before that I was getting around but had pain but it did not bother me at night. Now I can't find a comfortable position that has no pain. My doctor did not ask if I was diabetic. After watching this I will go check my sugars and see where I am. I can say that I am not a happy camper and there will be no more cortisone in put in this old body.

      I just had exactly the same thing. By day 4 the pain was worse than before my ACL surgery 5 months prior. 3 weeks later it's still agony to put weight on it and exceptionally swollen. Did yours end up getting better?

      @@kathklphotography It took quite a while but that terrible pain has subsided. Not that I don't have pain anymore but it is the normal pain of arthritis and I am able to sleep, aside from my lifelong insomnia, the pain doesn't keep me awake. I just started prp injections. That is platelet rich plasma. Look it up to learn more about it. It is very expensive but I finally found a doctor who actually is a doctor so she could help people and she keeps her prices super low. It will take a while before I know if it is working but I don't want to get the replacement if I can help it so I keep trying alternatives. Best to you.

    Super helpful, thank you! As a rheumatoid arthritis warrior, I’ve had a few of these over the years. One knee was done a few months ago and yesterday both were done. This is the first time I’ve had side effects! Last night I had a cortisone flare in one knee and today I have the facial flushing. Fun times! Lol

    I had my first Cortisone shot, I am Thankful, It really really Helps, Well it Helped me alot , Although I need a Knee Replacement sometime in the Future, But besides that This shot is Awesome, Just hope it last a long time, Thankyou Dr. Calvin Oishi.

    Very helpful info, I have pain in my right knee, doctor says is benign and very small sclerotic mass and suggested cortisone shot, I've been thinking about it and just been taking Ibuprofen to ease the pain in thethe last 3 months, you video definitely helps, and my question is, what happens to the cortisone injected inside the knee after a period of time, does it stay there and cause any future problem or does it just disappear over the years. THANKS AGAIN!

    I have bone on bone OA. I got a cortisone injection in December 2022. It worked within hours and was gone in 10 days. I have an appointment next week to discuss surgery.

    Enjoyed the ease of the video. When you say that they last 1 week to several months is that the effect of them or the medication is still in the systems. That is the part that confuses me.

    I'm considering the shot following an accident. Cycling 2+hrs my knee is already feeling swollen inside, sharp pain. My miniscus has a tea, im 90 lbs overwright and 6 months away from active duty. I guess my plan is to get this shot, begin training and my knee should feel better after losing all the access weight following the accident....

    They work around 3 months for me, I have RA and the shots work really great. Although I feel like I took speed for a few days after getting a shot.

    I had a Cortisone shot today. The X rays revealed Baker Cyst caused by torn meniscus. In a few weeks knee surgery. Right now my knee is hurting worse

    I think they should also be avoided for tendons because they weaken the tendon and have a risk of rupture later on.

    I've had relief that only lasted 1 week and another shot that lasted about 4-5 shots. It's frustrating not knowing how long it will give me relief.

    what i find weird is i am reading others say their knees hurt for a few days after, and got worse or better with time. i got the shots in both knees today and other than fainting after and being a bit out of it mentally for maybe an hour or two afterwards, i feel fine. no pain and honestly i don’t feel like i’m very wired up. i feel quite normal actually other than some inflammation after receiving the shots which has already slowly gone down. however, i am a lot younger than everyone i’ve seen comment on their reaction to the shots. (i’m 17)

    I had the flushing for a couple days on my face , arms and chest. It reduced my knee pain immediately and has lasted for 2 1/2 weeks so far. I can sleep again, which was the goal, so I’m thrilled.

    I dislocated my hip when I was 13 years old and when I did that I started falling on my left leg on purpose. Now I did have two screws in my hip when I was thirteen and they were removed when I was 14. By the time I was 38 I got a total hip replacement and I'm almost 47 now. I was diagnosed with kneecap arthritis 2 years ago but I knew about arthritis in my knee 8 years ago when I got my total hip replacement. And believe me I'm in pain. I can't stomp very hard with my left foot if I stop hard it will make my knee hurt. If I miss step then I will have to use my crutches because I won't be able to take any weight on my left leg for a good six or seven or eight hours. I used to get cortisone shots in my heel because I used to be obese. Those cortisone shots in my heel only lasted three weeks. I'm not obese anymore. I am overweight. 15 pounds will get me to 136. I can tell a difference because I went from 207 pounds to 127. I'm 152 now. I'm good at 135.
    And I had my first cortisone injection in my left knee in October 2021. I was very leery of getting this because I heard they do damage to the ligaments and tendons in all it does is mask the pain. I want something to address more than just symptoms. But I was impressed. That cortisone shot in my left knee helped my knee pain go away for about 10 weeks. I'm going to count the weeks this time. And then the next time I think I need to get the Ozone gas therapy in my knee

    Had a cortisone shot about a month-and-a-half ago took about a week to work. The cortisone injection only works for about two-and-a-half weeks now I'm back in a lot of pain in my knee unfortunately. The doctor said I can only get a cortisone shot every 4 months. I feel like the cortisone shot was a waste of time for me.

    Long-term affects are nnot mentioned...and seem to be SIGNIFICANT! Reports are that cortisone actually ACCELERATES joint deterioration. Why is that not mentioned in your warnings?

    You mentioned it might not be a good idea if you have a broken bone. Can you please elaborate on this? I have a broken ankle and was just given a cortisone shot in my knee. This is why I am curious about wht you said. Thank you.

      I believe they are not good for acute injuries because the inflammation and immune response heal the wound. Steroids reduce your immune system and inflammation.

    Thank you. I had cortisone in my outer hip area yesterday. Today I feel like my hips broken lol I can hardly put weight on it. So to hear flare is a side effect is good to know. Hopefully it settles down soon. Thanks again.

    I had cortisone shots in both knees I could hardly bend my left knee the shots are Purdy much painless and they worked wonders on my knees I felt like I was 18 again after the shots I have been getting them for the last 5 years every 3 months but these last shots didn't work its been over 2 weeks since the shots and I can hardly walk my left knee is killing me it hurts standing sitting or lieing down . I am going to try Gel shots next time if I can wait 3 more months. I am thinking about having my legs amputated that would stop the knee pain 😫

    I got the shot in my knee yesterday and woke up nauseous with a dull headache. I’m only 40 but I keep getting really hot and just walking into the next room my heart rate goes up as if I ran. I was watching the video for side effects but the kind that doesn’t exactly mean at injection site

    The cortisone was life saving for me (no more thoughts of suicide) had been getting them 6 to 8 months apart and now they no longer work (59) now looking at the hyaluronan injections.
    28.11.20 - Update my Doctor won't' refer me for the hyaluronan injections because he said they don't work so until I get the 2 knee replacements I'll soldier on until the suicidal thoughts return and then demand the surgery.

    Mine lasted 1 day. 2 days post shot was a slight improvement. 3rd day was fantastic. 4t day back to intense pain. Not a fan.

    I received a cortisone shot in my knee for pain three months ago. That night, I didn't sleep and my knee throbbed all night. The next day it was bruised about three inches around the site of the injection. There was a small pinprick where the needle went in. Over the next few weeks, the bruising radiated out and surrounded my kneecap/knee area. The bruising was circular and extended out about six inches around on both sides. There was bruising beyond that six inch circle, and the bruises radiated out like striations towards the sides of my leg and trying to go around the back of the knee. The pinprick started opening and turning into a scab that was the size of a pencil eraser; eventually the scab grew bigger and bigger and was about the 3" across the knee. My knee throbbed and there were days I could barely walk on it. It felt like it was going to explode from the inside, out - so much pressure. The knee was hypersensitive and I could not stand to have even a puff of air on it. At night I didn't sleep from the pain, and I developed a severe issue above the kneecap which swelled and was extremely tender, to the point I couldn't stand to have a sheet over it at night. I literally thought I was going to go crazy from lack of sleep and being exhausted. Went back to the doctor, who said "Yes, it's an allergic reaction!" and took no further action. She sent me on my way with advice to let it heal on it's own. I've been nauseous for 3 mos intermittently. The healing process took another four weeks; so I've had this for almost 12 weeks and it is just now starting to heal, with the scab decreasing and the pain getting better. I would encourage people to consider carefully before getting a cortisone shot - what's in it? What are the risks? How can it be treated if you severely react? etc etc....! I've had other cortison shots in the past with no reaction. My doctor could not/would not explain why it happened; and had no treatment for it. My doctor was SO disinterested in helping me. Why?
    What was in the injection she gave me? Did it come from the Wuhan Lab in China, or the Drano Lab in Mexico? I'll never know. Now I'm scared to death to get **any** shot for **any** reason.

      @@detectiveyoshi Let me add that before I received a numbing anesthetic injection and then the cortisone shot, I had received my Covid booster the week before. I'm not convinced now, that I didn't have some sort of reaction because of the booster. OR to the anesthetic injection. Frustrating b/c there is literally no medical professional who can or will respond with information.

      @@VABmo ah yes that might have played a role, your immune system was already weakened.. still doesn’t explain the allergic reaction tho 😳 and imo you were not helped enough with that.. I have a very swollen, painful knee and it’s really limiting me in everyday life. Also just kinda scared what will happen to me when I’m older since I’m not that old yet. They did a mri scan and in two days I get the results, hopefully they will figure out what it is.. I hope they won’t recommend cortisone injection because I probably don’t want to do that. I also read that for some people the area of the skin where they have injected cortisone had sunken in and it remains like that.. did that happen to you too?

    I just had both my knees I injected today and I have a horrible migraine. My blood pressure also shot-up and I am sweating. Why is that?

      Maybe you are like me and didn't get along so well with it. First two days my knee felt great. A steady decline in great. Today, one week later I can barely walk. Never again.

    Hello, I had a steroid shot 2 days ago, and my knee got very swollen and painful. Is that normal? The doctor said the shot would help me get better, but it actually got worse.     How do you know if you've got an infection? It's been 3.5 days since the injection and I'm in more pain now then I had seen I went to the doctor. I can't bend my knee at all. And pain Radiates down my calf.

    I got the shot in my both knees and after 2 hours I started to feel a lot of pain . I am in my third day after the shot and still have pain that is really bothering me .
    Please advice

      I don't really think personally is not normal. I've had cortisone shots for years put in. Yesterday I had them put in both knees my left one is the worst usually it feels a little better but my right one got way worse so I don't understand I'm seeing the doctor today God bless you good luck with your journey

    i had a cortisone injection recently. 2 hours laterit was if a bolt of lightening hit me. i couldnt walk as in excruiaciting ain and screaming . had to sit for hours in terrific pain could not even get up for a drink . then my face inflamed and went red from chest up neck to cheek. leg not painful now but im taking antihistamine regulary as still a bit puffy and burning up. no support to help with this . i have had 3 anaphalaxis this year . im not saying this was one but certainly triggered a reaction and concerned next could trigger an even worse reaction more quickly. why would this happen?

      Yes! Mood swings, depression and light psychose in some rare cases.

    I had a cortisone shot in my knee yesterday and have weird swelling in the ankle. I iced it but still swollen.

      I had 3 in my leg yesterday....Do yourself a huge favour and DO NOT look at the needle( had to lay on a stretcher because I fainted )

      This is not official medical advice, but Cortisone is a potent anti inflammatory and doesn't solve the underlying issue. It can actually make the problem worse by degrading tissue since it shuts down the body's inflammatory response. Be careful if you ever decide to do another injection.

    I have a suprapatellar fat impingement in my left knee. I've tried PT, got an MRI, toom antiinflammatory meds...now I guess I'm signed up for the shot

    I use to shots in both knees every 2 months. Now just the left knee cause zero help for right knee. And just 37 years old. 17 years of the coal mining industry.

    You left out hiccups. Each time I get steroid injections to both my knees I always have uncontrollable hiccups as a side effect. Not easily treatable for me.

    Frequent cortizone steriod injections can degenerate the bone and cartilige. Steriods do have side effects. Anesthesia for joint replacement is dangerous for seniors and temporiarly or permanently affect brain function. My brain is more important than my knee.

      That's ridiculous. If you're really a doctor, you probably should take the time to educate your patients on the risks of the injection rather than send them to RUclips. Cortisone isn't a joke, doesn't fix the problem, and if you were my doctor, I'd walk out of that appointment if you told me to watch a RUclips video after I took time out of my day to have an appointment.

    My shots only lasted 2 days. Not the dr wants to do gel injections. If that doesn’t work the next step is surgery

    What about people who have been living with chronic lyme disease
    The injection was supposed to be for a crows feet tendinitis. It did nothing but caused me more problems. Its been 1.5 month and haven’t been able to have a proper night of sleep since i got the injection. I have terrible hot flashes day and night but more specifically at night, insomnia, weird heart palpitations (chest pain), dizziness, it has affected my vision and speech (forget words) The first few days i had terrible nightmares and hallucinations (that i had fleas)… they never warned me of anything before getting the injection and they didn’t give any info. It has clearly fucked up my system. My knee and tibia still hurt like crazy. I cannot walk more then 15 minutes. Don’t get this injection

    My doctor did the injection without lidocaine. I didn't see him aspirate. But when he put it in I screamed like a banshee never felt anything like that before. I have sciatica is done nothing for the sciatica I don't know why he did the knee. Because the knee was affected by sciatica the back of the knee the back of the leg the back of the calf my hip the front of the knee did not hurt. I'm pissed. I won't have him do my lumbar injections. And when he took the needle out it was bleeding all over the place

    I don't trust this presentation. Why? It did not deliver any information at all about Cortisone shots causing liver damage. I'm more concerned about liver damage than all these other side effects they mentioned (skin tone etc) But I've seen this before. I haven't found ANYONE on RUclips that HAS addressed this issue. It's almost as if it is a part of some agenda or something? It's like they won't let you on RUclips if you talk about it. And liver damage is a serious side effect. I've heard about it, but I want to know more.

      Worst experience ever! I had injections in the past so I asked my rheumatologist to do both knees. My knees were numb for 2 days. I reported it and they said that can happen, to let them know if it didn't get better. 10 days later, lower half of knees numb and it felt like I had rubber bands on my knees. I had many Kaiser (uggg) diagnostic tests including nerve conduction on one leg, x-ray and m. r. i.
      Three months of not being able to walk well made leg muscles weak. It's been 11 weeks and I have not recovered. Never again!

    Long-term affects are nnot mentioned...and seem to be SIGNIFICANT! Reports are that cortisone actually ACCELERATES joint deterioration. Why is that not mentioned in your warnings?
