Those are true healers to take so much time and effort to help... These are the types of people who deserve the best the world has to offer in life. The large nipple was probably gagging him a bit as a baby, just as your own jaw moves if you try to suck down a straw full of liquid at the back of your throat - that's what was happening. I remember being upset when my kittens peed on me and my mom said, oh those little kittens' pee is so clean at that age that it won't be a problem and won't stink. She was right. Great job for the most part.
The nipple's are very hard to find smaller than that. That was probably the smallest available. The vet gave me the same size for the kitten I nursed to health. It takes a minute for them to figure it out.

It's not easy raising a kitten that young. My current kitty was about that old and small when we rescued her from the street. Time flew by, and she really grew. It's been 13 years now, and she is a beautiful calico girlie. The light of my life. Blessings to you and this beautiful kitty. ❤❤❤

I forgot to mention the vets in my comment. They did an amazing job with the little orange kitten especially getting all the fly eggs and parasites off him when he first came in. That was a true labor of love ❤️
