Just because the result was surprising to fans does not mean it was not supposed to happen. NJPW planned to have Okada win there. The fanbase might not have been ready yet, but Gedo had that planned. A better example may be Jay White. He would not have become IWGP champion if Omega did not leave the company to join AEW. As a matter of fact, the Tanahashi run in 2019 from Wrestle Kingdom was also a surprise due to Omega leaving.

Since Ibushi was a flight risk and Omega was gone, Tanahashi won the G1 and the title. He transitioned it to White after. The original plan, in my opinion, was Omega holding the title until MSG and losing it back to Okada. That would have been much longer than most recent first time IWGP championship reigns. Also, I am of the opinion that Evil was an unplanned reign due to wanting to reset Naito's run after the shutdown NJPW went through. This is not to say that White and Evil were never going to become IWGP champions. The timing was changed for both.

Guys who haven't won it yet that will at some point are Ibushi, Sanada, Ospreay, and possibly Sabre and Kenta. I personally would like Goto to get one to make up for his "always a bridesmaid" reputation. He has been a high performer in G1 and New Japan cup but can't win the big one. It always seemed like NJPW had a storyline that would get in the way of him getting the belt.
An honorable mention would be Seamus' 1st title when he beat Cena in a tables match. Another honorable mention would be heel Sgt. Slaughter in his traitor role during the period around the Gulf War.

I know we tend to think of Bob Sapp as a joke these days (because he is), but he was 5-1 at the time, with the one loss being a last-minute submission against PRIDE HW Champion Big Nog in a fight he would've won by decision, so I think it's safe to say he was "supposed" to win.

Can't really say anyone isn't supposed to be a champion. People get signed with no intention of being a champion at the time of signing, then they get over so big that it changes everything. If you think about it, only a few people were signed to be a champion from the start.
Stone cold wasn't, the Rock wasn't, the Undertaker wasn't, HBK wasn't, Brett Hart wasn't, AJ Styles wasn't, etc. It wasn't until they became larger than life that the plans changed and they were to become champion. Also depending on the organization. 1 person could be signed with the intent of being a champion in 1 promotion, while being signed in another with no intention only to eventually become the champ.

Speaking of IWGP Champion Kazuyuki Fujita and NJPW's MMA connections, how about the fact that he got his second reign for beating then-champion Bob Sapp in an MMA match? I know we tend to think of Bob Sapp as a joke these days (because he is), but he was 5-1 at the time, with the one loss being a last-minute submission against PRIDE HW Champion Big Nog in a fight he would've won by decision, so I think it's safe to say he was "supposed" to win.
