    Assalam o Alaikum. Sir
    Have a question at 4:03 min after extraction of 2 teeth the lower left lateral Incisor were present, also after healing left lateral Incisor was present at 5:57 min.
    U said 6:06 4 teeth are missing what happens to left lateral Incisor? That tooth was sound i think.

      Wasalam Asad.
      The lateral just looks sound in the extra oral pictures. 32 had Mobility between 1 and Grade 2 due to bone loss. If you check the initial xray, you will see that there radiolucency between 32 and 31 which means there is probably no bone mesial to the lateral. This was confirmed intra orally after extraction of the central. On top of that the bone on the distal side covers only about 30-40% of the root. Had planned to a do follow up after 2 months (when bone fills the socket). The mobility sometimes gets better and so might not have to extract it and if it is still mobile (II), will remove and place an implant in the extraction socket to give a predictable, stable treatment. This is what i did in this case.
      That being said, even if you had extracted the lateral at the first appointment along with the centrals, thats ok as the prognosis isn’t the best anyways. I don’t want any problems next to my implants. Whether it is a cavity on the neighboring tooth or a pending rct. I solve those problems first before getting to the implant area for good long term prognosis.

      No bone augmentation required. If the extraction was atraumatic and the wall of the socket are intact, bone will fill insha Allah :)

    We saw significant bone loss from the start but when you placed implants it looks like there's bone present
    Did you use any sort of bone to restore mandibular anatomy? As far as I know bone doesn't grow where there's periodontitis.

      Wasalam Farrukh. As long as the walls are intact, bone will fill in the socket, more or less, till the height of the walls, regardless of the size of the socket. All you have to do is remove the problem (in this case the teeth) and leave the socket to heal for atleast 2 months.
      In short: Body restored the anatomy itself through healing because the walls of the socket were intact.

    Very well delivered . Really appreciate the details you explained regarding the treatment. Also I have one question. After placing the implant and before the placement of healing abutments, was the patient given a temporary denture the one he was given in the start? What other options can be given in the healing time for the replacement of teeth as this is a highly aesthetic area.

      You mean after the healing abutment and before the final bridge ?
      Yes ofcourse the same denture was trimmed to accommodate for the healing abutments.
