A fault in the auxiliary supply used to power the controller chip - the resistor in series with the auxiliary winding was open . 2 . A diode fault in the feedback circuit ? Is that correct ? What was the root cause of this fault ?

these are blocking voltage which drop across the diode, it shows us the transformer and diode's health. no need to use reference for voltage measurement across a working diode
