The fire on board the vessel Conception was a completely preventable, utterly horrific tragedy.
Let's hope the NTSB recommendations--all of them, including requiring SMS--become fully implemented.

She is on fire, and for good reason.
Keep up the heat on the coast guard!
Don't get me wrong, the Coast Guard personnel is very underrated and underpaid and the crew are nothing short of brave heroes.
The heat is instead directed to the management.
As chairwoman of the NTSB she does not like being repeatedly ignored, and the only thing that gets done years later is a "community survey/requests for comments"
This is appalling.
(Even the last Chairman of the NTSB did not mince words and said what needed to be said, but this chairwoman is taking it to the next level)
