Once epic verify your documents , you have to create AMC account, upload documents there. Then you come back to epic account and upload your lady credential and select to share it's report with amc . Once done , wait , amc will send mail and you will also keep checking your AMC account , they will mention properly , that you are eligible.
On my medical degree my name is - Pamidimalla Swapnith Reddy and on passport my Surname is - Pamidimalla and First name is Swapnith Reddy will this be a problem?
Epic account formation takes 03 days , real time is taken by notary verification. Try to do online meeting instead of appointment for notary. Online meeting will happen as soon as slot gets empty , it will take some effort like you keep on looking after every few hours. And with amc account formation it also happens in 4-5 days .
Use names as mentioned on passport , the surname is family name used and given name is your name part. E.g my name is Qurat ul ain , so Qurat ul is my given name and ain is my family name or surname according to passport
When you create epic account , ecfmg provides you temporary username and password. It's other than your Gmail password. Later you can set it accordingly.

What is student identification number??
It's your university registration number. In my case , my university is UHS , so I entered registration number provided by university.
Is this epic verification forever or it has to be renewed?
Yes this verification is permanent unless some novel rules take over
