Thank you for this little piece. I just hit 2yrs post-op on my right and in 6 mos I'll be 2yrs post-op for my left! Starting weights in my gym and i wondered this very thing! I still follow your posts as they are so informative and helpful through my recovery ... which has been great! 😊

I had an RTKR a little over 3 months ago and started using the leg extension machine on leg day at the gym at the end of week 2. I started with the lightest possible weight, the least flexed setting, and the rollpad above my ankle bone level; starting with 3 sets of 10 reps. I'd always do the exercise with both legs, trying to distribute the weight evenly ... and my "good" leg serving as a "spotter" in case the TKR'd leg should struggle or encounter pain. Over the weeks, I was able to incrementally increase the weight and the number of sets and reps. I believe my quads really benefited a lot from the use of this machine. I typically would use the leg press machine after the leg extension machine and I think that sequence works great.
Thanks for your clips - super helpful. Viewing them has been my early morning constant. There are many other very good PT posters on RUclips but I find yours the best by far. Thanks a bunch!!

Yeah, I had LTKR a year ago. I do mainly bodyweight squats onto a collapsible stool, going lower each time. Then I do light sets of leg extensions - sets of 30, and then 25 for 4 sets. If you do them heavy I believe it's not good for your knees. I'll increase the weight, but only if i can keep getting 25 reps.

Excellent video, and timely as well. I am 8 weeks out from TKR and just wondered about this very topic last week and will be heading into the gym this week. Thank you.

Thanks Tony, a timely reminder to include squats in my exercise routine, for some reason I dislike them and keep 'forgetting' them . . . . .

I had both my knees replaced 1-2 years ago after 25 unsuccessful knee surgeries. I am struggling mightily activating the quads. Straight leg raises don’t do jack for me, I can do 50-100 but all they do is aggravate the area below the knee and make me hate life, no matter how hard I concentrate, feel minimal quad activation and maximum joint pain. I’ve finally decided to ditch them completely, am gonna try partial range of motion knee extensions instead. Any advice on how to get the quads firing? The only exercises that don’t seem to hurt are deadlifts bench squats and glute bridges, along with calf/tibia/hip stuff

Thanks for that info as I was struggling with the extension machine. Will ramp up leg press now as it will strengthen my other muscles which definitely need it.

Yes , bilateral, although I don’t use the weight I used to , but not because of knees, I just don’t want to do anymore damage

What are the best weight machines to use after 5 months of TKR surgery. How much weight should be the limit? Louis

My knee stiffens up after doing knee extension or flexi on exercise in a chair or standing up, but I handle the leg press just fine. Any ideas on why that is? I can do straight leg raises just fine.

Hi Tony, I am at 4 weeks and saw Ortho yesterday and my range of motion is very good. He has released me to do recumbent bike and some squats at home. I'm wondering if a gym membership for strengthening, as opposed to range of motion would be better.
