The inhumanity of it all screwed with Kiryu so much, he actually nearly killed someone (and almost breaking his policy/record) and straight up dissociated from reality. It's likely that Kiryu was thinking he hit Iwami, his representation of a cruel Kamurocho, what it has done to him and his loved ones.

the difference is that he had a choice when it came to shibusawa. there were no nonlethal measures to take while escaping his men. except, y'know, shooting out tires. but honestly with how fast they were going that could've wound up fatal too.

The first guy managed to leave the helicopter as it got hit by the rocket and landed safely outside the explosion
The second guy was thrown out of the chopper as it hit the wall away from the explosion, saving his life

Anyway, the Kiryu called the ambulance right before stabbing the guy, so that they could save him and put his lost blood back
The last three guys never happened
