Resin cast from an actual milk cap, de-threaded. Dremel small holes for magnets, super glue in place. thrust roller bearing can be found in eBay.
Make your own. Wood would be cool too.

I have several of these gambits from billetspin and while they are all awesome, the interior springs are all different, you can get the spinning coin effect with a looser spring but the coin might fall out if you carry it with half a mind on it. I have one where the coin is completely stuck and wont spin, I have to smack that one to pop the coin out.

The milk cap name is just from the shape I think and all the other terms I think are their type of milk brands maybe. It can be a bit confusing. As for the sound all magnetic sliders/coins are similar in the fact that pressure will eliminate most of the sound and you need to loosely push to get the loud clicks. I got one of the cheap plastic ones myself just to try it out and see if I like them, $100-$200 was a bit steep for an unknown for me. I did recently get a Pebble from Magnus and love that, but I knew I would as I love smooth Titanium. I may try a CP-2 next, maybe all SS or Zirc and SS (I dislike copper so the Ti version is sadly off limits for me).

Are they durable, like if you drop them will they get scrated easily?
Yes they will get dinged up if u drop them or put in ur pocket with keys. But breaking them would be difficult

Yeah I got a clicker recently as well. Not like this one, but same concept. Its a good thing to get if you dont have a knife with good flicking action, or a click pen.
They are pricey tho, not a lot of people make em and those that do will charge a fair bit for a good one.
I think the best looking so far I've seen is the slider with the ace cards. Think that ones like 200 bucks tho... if you can get it.
