      Thank you for this video. I'm not 100% sure this is the cause but I have no other explanation so I offer this as a cautionary tale: 6 months ago I misplaced my house slippers for a few days, so I was walking around barefoot at home. That includes on our stone kitchen floor. After a couple days I started noticing discomfort in both feet and figured it would go away when I found my slippers--I have two young kids so things get misplaced all the time. Occasionally I would carry our baby (around 20 pounds at the time) in the kitchen, so there was extra weight on my feet. I found the slippers after a few days but the pain never went away and actually got worse over the next few weeks. It was first diagnosed as metatarsalgia and plantar fasciitis (I think the injured forefeet caused me to walk in a way that hurt the arch and heel of my feet). Eventually I got MRIs on both feet and discovered torn plantar plates in both.

    I go to a minimalist/naturalist podiatrist. She helped me fix my plantar plate strain (from accidentally stomping a piece of sharp gravel on a trail run) with exercises and massage and going barefoot as much as possible to strengthen the muscles, tendons, and ligaments that support the forefoot structure. Worked like a charm. Back to running 30 miles a week in barefoot shoes. No special shoes or orthotics necessary. Just my case, though.

      I like this comment. We aren't meant to walk in weird thick shoes or our feet would be that way naturally. I'm watching this wondering how in the world this is going to work with Barefoot me. Maybe orthotics is the pharma of podiatrists.

    I have flat feet, left bunion and long second hammer toe. The other night I felt like a marble was under my sock but it was inside in this area. Had been waking in the nights prior with fascial pain and was diagnosed with pl. fascitis last month and in my youth. 5 prior spinal surgeries make walking difficult but live alone, 60 y.o. female, no car. Ice got the lump down but in serious pain. Right foot plate pain, flat, no bunion. Scared about money as I work 3 hours a day as disability is not enough. On my feet. Help!

    Question:How do you diagnose a tear or stages of such? I have major pain on the balls of my feet - sometimes going to the edge (mostly left foot but now starting to feel it on the right foot).
    Background:
    Ive had xrays and ultrasound and the results were "No Morton's Neuroma Normal second and third MTP joints with no effusion,synovitis or erosions.. The visualized plantar plates and tendons are grossly unremarkable." Believe it or not I actually tried the duct tape therapy from one of your other vids and I dont know if it is placebo or not but I actually feel some relief - though its only been one day.

    I got my 2nd toe pain from spending 4 months in Puerto Vallarta and walking 8-10 miles each day. The flat areas were easy, though mostly hard surfaces. But much of my walking was on hills and staircases. I came back to the US with a sore spot that just never healed, despite getting orthotics, metatarsal pads, and rubber wedges to put between the first and second toe. I also cut back a lot on my walking. Finally got an MRI after 14 months of pain, which revealed a partial tear of that ligament. I'm having surgery in a couple weeks, including shortening the 2nd metatarsal by a quarter inch. I'm hopeful and intend to follow the doctor's instructions completely.

      @Roccarocks at the last minute, my insurance denied the claim and wouldn't let me have the surgery. I can't afford to pay for it myself or to fight the insurance company, since I'm homeless and living in my van. So, I've been continuing with the pads and toe separator. My limit on walking seems to be about 3-4 miles per day, or I'm limping badly for a day or 2 afterwards. I'm 62 and may have to wait until I am 65 and have Medicare, before getting the surgery. It's a bummer and the pain is always there, to a certain degree.

    Very informative! Is it possible to have a plantar plate tear on your third toe? so that it begins to move away from the Big and 1st toes and pushes up against the 4th toe? or is this some other sort of injury or condition?

    Is it unusual to feel more pain in the big toe with a plantar plate injury? I'm a dancer and was diagnosed with a spiral fracture of the first metatarsal. After 8 weeks in a boot, I returned to dancing and still had significant pain. An MRI showed a bone contusion, osteoarthritis, and a suspected partial plantar plate tear. I think my sesamoids are probably inflamed as well. All my pain is in the ball of the foot under the big toe. So far no treatment has helped with pain. And quitting dance is not an option. Any helpful taping methods for the big toe?

      i've had the same issue ! I am a dancer and was recently diagnosed with a supposed fracture in my metatarsal bone under my second toe as well as shaving off a piece of the bone. However, i got an MRI and it said i have a tear/ruptured my capsule in my metatarsal(where the supposed fracture was) but i feel the pain more in my big toe. Giving up dance isn't an option for me either and i've so many performances coming up in 2 months !! Let me know how your foot is feeling as of recent to hopefully give me hope and i also had a few questions !
      -How was the process of getting back into dancing and did you do PT?
      -Around how long did it take to heal?
      -What did you do during the injury to help it heal other than rest?
      -Were you scared of re-injuring it by rushing to start dancing again?

      @@pachecoisabella5574 Since posting this comment, I have since visited two more podiatrist and gotten different diagnoses from both. I have continued dancing off and on since March, 2022 but had to take a couple of breaks off and on due to excruciating pain. Most recently, I've been out of dance since October and don't plan to go back until April or May. The 2nd podiatrist I visited diagnosed me with capsulitis and administered a steroid injection. He also did a chielectomy (removal of bone spur from my big toe) in July. I left that doctor and got a 3rd opinion in October because the pain was not improving. My current podiatrist diagnosed me with Morton's Neuroma between the 2nd and 3rd toes and administered a series of 8 sclerosing injections to treat it. He has given me shockwave therapy for inflamed sesamoids, and performed a 2nd chielectomy on my toe this week. I wish I could say I was hopeful that all of these treatments are going to work and that I'll be able to dance pain free again, but it's hard to stay optimistic when I've been dealing with pain for almost a year and have been through 3 different doctors so far.

    This has been so helpful!!! I’m dealing with this issue currently. The specialist I was referred to is booked until December! I’ve had an X-ray and it’s showing arthritis. My second toe (next to big toe) and third splay and form a V! Painful! More so at night. I also had an mri on my hip and I have a tear in my gluteus minimus. Both injuries are from running. I’m 50 year old woman and could also stand to lose about 40lbs! Ugh! Getting old sucks! lol!!!! Great video! Already tapped my toe like shown.

    Hi, great videos. My Doctor doesn't explain much to me so you are definitely a help. What is the name of the shoes or slippers? I have a total tear and am in a boot. Also, I sleep on my belly, does that affect my feet more because they stretch out?

    Excellent video. I'm a PT who needs a refresher course in the foot. Unfortunately, some of us are close to hypermobile and shoes with elevated heels aggravate my back and there is a high probability I might have overstretched at times. Because I own half of those products. But good points and I appreciate it thank you

    great info. I'm working (therapy) with 88 year old lady, RA her Dx and her 2nd toe on BLE are crossing over greater toes, but RLE is the worst and the toe is pretty stiff. I will try the stretching and taping to see if that helps a little. I will advise her to talk to her podiatrist reg plantar plate issues. Thanks!

    Just got diagnosed with plantar plate strain from my podiatrist after pain for a couple months. They gave me a met pad and waiting for my orthotic. I’ve always had longer second toes that curl up in shoes and been trying different shoes for work (stand a lot) hopefully I’ll get better with the new orthotics that should help my arch and offload weight… Thanks for the video explained it clearly!

      Hi J Bryant, we like the carbon plate for turf toe. A soft insole can be combined with a carbon fiber insole. Generally, a supportive insole is more important than carbon fiber in these cases, but combined can be even better for some people (but a carbon fiber stiff insole might not add many benefits)!

    Im researching. thank you for this...I have Full-thickness degenerative tear of the first toe plantar plate. Additional tearing and
    retraction of the second through fourth plantar plates. What should I tape? which toe? waiting to hear what the surgeon wants to do at this point..not sure what to tape now..You are right, misdaignosing. THat is what happened to me. Fall of 2020 it felt like I was walking on rocks. I got an injection, PT, orthotics

    Thank you for your instructional video! I do have a question. Do near complete plantar plate tears heal on their own… once you ice, rest potentially Immobilize and it stops hurting? How do you know when you’ve healed?
    After an MRI (foot pain for 6 months and 2 podiatrists so far, looking for a 3rd) I have been diagnosed with:
    Near complete tear of the plantar plate of the third metatarsophalangeal joint
    Small intermetatarsal bursitis of the first through four web space.
    High grade strain of interossei muscle of the third webspace
    Moderate first metatarsophalangeal joint space narrowing with minimal osterphytosis and joint effusion.
    I’ve had PT (wrong type), shots once (cortisone, b12 and something else), a soft pump up cast boot I’ve worn for a month (realizing it’s too big of a boot - just bought a new one today), metatarsal pads prior to the boot. I’m using red light therapy, icing it a lot, and soaking it in an Epsom bath. Still having problems with swelling and pain.
    MY QUESTIONS:
    1. I’ve been instructed to tape 2 toes together - I see you only taping one toe. What is the difference?
    2. I’ve been instructed to use kinesiology tape, but have seen several RUclips podiatrists say it’s too flexible. Is that why you use duct tape? Is kinesiology tape too flexible in your opinion?
    3. What do you think about stem cell therapy?
    4. Have you heard of molecular hydrogen for healing and have you seen any help with the hydrogen foot baths or H2 gas treatments help?
    My hubby’s a Michigander 🤚 but we’re in Los Angeles, so I’m looking for a podiatrist who won’t treat and street me.

    I had an emergency incident at work, I got up to run to see what occurred, there was a cable on the floor, I tripped with the cable across the top of my foot, with the forward movement the cable across the top of my foot broke my 2nd metatarsal. My foot was swollen, black and blue but I could manage to walk on the side of my foot. The owner of my company said if it was broke you couldn’t walk, so I continued to work. After a week the pain became so bad I took a few days off and upon returning feeling better, I went up the stairs and did a push off and felt a crack and then felt my foot cracking whenever I walked, we called workmen’s comp, and two days later I went into the doctor, and I was diagnosed with a fractured 2nd metatarsal. After 6 to 8 weeks in a boot, I am still having pain and swelling in the ball of my foot. I am scheduled for an MRI next week and just looking for a video to potentially help with my pain and swelling.

    I am findtaping impossible because the 2nd toe is webbed with the 3rd. So I can't put the tape correctly.

      @@DrTomBiernacki yes, my podiatrist has made a temporary one but also wants me to tape it which is impossible and makes it more painful. Any other means to treat this problem would be incredible. I have a follow up appointment in about 2 weeks but I can't deal with the taping and wearing the temporary orthopaedic soles together because it aggravates the pain. And because I'm on my feet and wearing steel cap boots for work, I don't know what else to do.

    Well, I’m a runner and I have been doing a lot of eccentric heel drops for Achilles problems also. It didn’t seem to happen when running but I had pain on the ball of my left foot at the 2nd metatarsal. Clearly I put too much stress on that area. Initially I thought it was a neuroma problem or metatarsalgia but… after two months of it not getting better I saw a podiatrist and he did some x-rays which looked good and he examined my foot. He diagnosed it as a plantar plate tear. He recommended the taping technique shown here. He recommended a shoe with more of a rocker shape like some of the Hokas. Things have gotten a little better but I’m still working on it.

    First, I want to thank you for all the information you have provided on plantar plate issues. I am a runner and started experiencing pain in the ball of my foot mid April. It took a while to diagnose, the pain was very minimal with no swelling or bruising and only when I ran, but I feared that I may have a stress fracture. The podiatrist first thought "stress reaction" and suggested that I back off my running for the next two weeks, I was training for a half marathon that took place May 1st. I ran my half and have not run since. After multiple trips back to the podiatrist an MRI showed the following:
    There is a partial tear of the medial plantar plate of the second MTP joint with hypointense pericapsular soft tissue extending into the second intermetatarsal space.
    IMPRESSION:
    1. Partial tear of the plantar plate of the second MTP joint with perineural fibrosis rather than an neuroma of the second web space.
    2. No occult fractures.
    3. Intact myotendinous structures.
    4. Preserved articular cartilage.
    I have been in a walking boot since June 7th and have been fit with custom orthotics. I have a follow up with my doctor June 13th. He gave me zero instructions regarding taping, icing, stretching, etc. I do believe that the causation of the tear was increasing my mileage to quickly and a really tight calf muscle. Does a tear actually heal? What are your thoughts about stem cell injections. Any thoughts or questions I should ask my doctor would be greatly appreciated.

      I've had a very similar diagnosis - partial tear with preserved structures surrounding. My MRI was mid May, and I had the same treatment to follow - put in the moonboot [walking boot], but the injury occurred Nov 28, so 2021 has been incredibly frustrating.
      I had already been issued the orthotics in March, as a catchall intervention.
      The sports doctor to whom the podiatrist referred me, I questioned the same things. I directly asked about stuff like PRP - not necessary at this stage. Just stay off your feet as much as possible, and use the moon boot when you need to. Can do hot and cold therapy, but it won't likely do anything apart from some potential general pain relief.
      After about 6 weeks, depending on severity and their assessment of your injury, can move on to a rocker shoe [i.e. Hokas EARLY [not late stage] rocker] shoes. I bought those a few days ago. Do the taping though if you do it correctly, it certainly doesn't hurt.
      No pain, but as I learnt just prior to my diagnosis, no pain after 6-8 weeks of essentially immobilizing is simply enough to repair the ligament, but not get the strength back. So exercising as soon as you are pain free will be like expecting that a glass sculpture of say an aeroplane, and an actual aeroplane, to be equally as tough. No, you need to stay the course.
      Before my diagnosis, I had managed to repair the ligament [i.e. little to no pain when walking] but it was functionally very weak. If you think about running somewhere or doing some exercise that'll overload it, you'll be back to square one. It's a 12-18 week thing, minimum.
      When cleared to you can start doing the calf stretches as well as other physio stuff to increase intrinsic strength of your foot to help assist / prevent the issues happening again.
      Here's hoping we can get back to our sports / running in the near future..It certainly sucks.

      @@MrVibhas Thanks for your input. Yes, it is very frustrating. I went back to the podiatrist this week and he basically said I need to decide when the foot feels good enough to begin walking in the orthotics. I mentioned the taping, metatarsal pad and the exercises. He said the custom orthotic takes the place of the taping, but I they are made specifically for my running shoes and I cannot wear them to work. I wore the orthotics all day, one day and both my feet were miserable by the end of the day. So, I am going to wear the boot and slowly break in the orthotics, wearing them a couple of hours a day until I feel my foot is completely healed. I has already been 10 weeks and I definitely do not want to reinjure it, so I am ok with waiting a while longer. Thanks for the advice!

      Stem cell actually doesn't have stem cell in them but rather mesenchymal, unless you are using your own fat cells autologous transfer. In either case this doesn't work, just get a plantar plate repair without an osteotomy from an orthopedic surgeon.

    So I was diagnosed with a tear by ultrasound after a year of pain. It was fascinating how the pain moved from the 2nd metatarsal joint to most recently the end of the outside of the toe 😳 very horribly sharp pain that sometimes literally came on out of nowhere (obviously it flared when I trained on the track or did plyos or went for a hike) ... I thought it was either a stress fracture or a neuroma all of this time. Turns out I tore the plate. Just wish I did the ultrasound earlier, could have saved myself a few months of pain me thinks...

      I had a large bursa under my second metatarsal and just figured Ihad burnt my foot on our hot deck. Quickly the situation devolved into 2 hammer toes. Went to Podiatrist who said I had a plsnter plate tear with subfluxed metatarsal phalangeal and a far progressed bunion and all this made me glad I worked in ophthalmology. The doctor told me I would need my foot reconstructed but it was too much work. I spent all day running around soshe figured it would not be worth because she didn't think I would stay off feet and it would fail. That was before Covid. I was made to retire early and also have a torn meniscus in that knee that has worsened so I have learned to stay off my feet and use a walking stick. I now have Medicare and need all this fixed - is it too late? I can barely feel my two middle toes......

    Hi Doc. My foot pain came on 5 years ago in both feet after increasing mountain bike racing at 54 (clip in shoes stressed balls of feet and racing tightened leg muscles). Self-diagnosis was easy at first, extreme heel pain getting out of bed and walking on cold tile floors felt like pebbles under ball. I went to podiatrist who diagnosed metatarsalgia and made flimsy custom orthotics ($500) that didn't do anything. He begrudgingly gave me a walking boot after I begged and cried about the pain! A year later, still in pain, I went to a larger/well known orthopedic foot specialist. He took x-rays told me nothing was wrong, said I could tape if I wanted, but nothing was structurally wrong. Very discouraging. Since then, 2nd toes have slightly hammered up. The pain comes and goes. Left foot felt better after 6 months of night splint. Right foot got worse. I've been wearing an open night boot (Braceability brand) 3 years ago. Still aches/burns if I lay down without boot.
    New symptom appeared a couple months ago (after minimal jogging on sidewalk)...my 2 & 3 toes starting separating into a V (both feet, but the right foot is worse and very painful). My #2s also appear to be turning sideways and moving toward my big toe! Omg, my feet were always PRETTY! Now I've got crazy toes and I'm limping a little to avoid the pain! Sounds like the old injuries have worsened to planter tears. I have an appt with new podiatrist in 2 days, thank God! He specializes in minimally invasive surgical repair of planter tears. I've been using a wide thick rubberband laced around 1, 2 & 3 toes (clinching 2 & 3 closely together strapped less tight to the big toe). I like not have tape-goo all over my feet. I'm laying flat on my back to type this, the the sharp burning in the ball of my right foot at 2nd toe is awful! That's my story so far. Bright spot is that I can still ride my mtb! When the balls of my feet ache or burn, I unclip and shift back on the peddles. Hoping doc can fix my feet and give me back my pretty toes!

      Did you have a Plantar Plate Repair with an Osteotomy and if so how did it turn out?

    There's no "easy" way to fix this. Humans have not evolved to fix ligaments, nerves, and tendons. These injuries lag and lag and always come back until you get surgery. Orthotics, steroids, NSAIDs, shockwave, it's all an endless money pit that prolongs surgery.

      100% disagree with that, I strongly expect it to get better without surgery almost every single time over 1000s of patients. I think this push for surgery comes from doctors that have ownership in surgery centers or have deals with device manufacturers. I must be getting lucky with easy patients I guess

      @@DrTomBiernacki Okay, but how long is too long for these injuries? Chronic is defined as 3 months. Are you surprised when it takes 3-6 months or longer to heal a ligament the size of a fingernail? It seems utterly incomprehensible that it could take so long. It certainly seems like the body did not do the job.
