There was a top-tier gymnast at the Olympics and I cannot remember which one it was, but it was China or possibly the one before… who was doing some very difficult front flips on the beam and she came right down on her head on the beam. And then came off altogether, of course. Worst fall I’ve ever ever seen.
If you guys might be confused with the second one, look closely. Her arm is bent and not in the correct position so she could have had a very bad fall if she didn't stop.
