  • This video will look at How Do I Lower DHEA Levels via Cortisol. Many people are asking me, how do i lower DHEA levels? This is a first in multiple articles on how to go about lowering DHEA levels. We will talk about specific tactics you can do to lower your DHEA levels in each video. Many times high DHEA levels are a sign that you have other things going on If you haven't been checked out for PCOS and things like this, you should be evaluated further for this.

    The approach to lower your DHEA levels will vary based on the reason that it is high to begin with. Your body is a complex system of overlapping layers of feedback inhibition. By feedback inhibition we mean one thing causes something which then feeds back to inhibit the same thing that caused it. Understanding your particular system takes patience lots of testing and clinical experience. With that in mind we will narrow the focus and I will explain how i would approach lowering DHEA. Also not that when i say DHEA i'm referring to DHEA sulfate (DHEA-s).
    Overall the idea here is that there's either too much DHEA-s coming in or not enough coming out. Either one can offset the flux of DHEA leading to higher DHEA levels. In this article we are going to look at the different things that are known to influence that which is coming in. Other videos will look at the things that influence what goes out. One important note about high DHEA levels is that congenital adrenal hyperplasia, which is a genetic disorder often presents with elevated DHEA-s levels. With this condition the enzyme that breaks down DHEA and turns it into androsteindione is defective. This enzyme is referred to as 3 beta-hydroxysteroid dehydrogenase. There are some other (non congenital adrenal hyperplasia) things that can go on with this enzyme that leads to elevated DHEA levels. So depending on how high your levels are you may need to look into seeing an endocrinologist. This is a good idea in general to see what they have to say. The first thing that we want to talk about is the amount of DHEA that's coming in and where it comes from. One of the things that influences this is stress. You may have suspected this or heard this and here we are going to go into more detail on how this occurs and what you can do about it. Each time cortisol is produced DHEA is also produced. This is because in the brain there's a hormone called ACTH which stimulates the adrenal glands. At the same time it stimulates cortisol it also stimulates DHEA production. Those two hormones are almost coupled together. You can't have one without the other. So the more stress you present with, the more DHEA-s production you're going to get. So the first step in figuring out if this is something that's relevant to your situation is to check your cortisol levels. You already know your DHEA-s is high but why is it high? If your cortisol levels are elevated this suggests it could be at least part or one of the causes
    Now one cortisol test is not going to tell you definitively that that's the cause. However if you have multiple samples that are showing you are above the normal or close to the normal upper end of the reference range, this does suggest it is the cause. There are different ways to check cortisol too. There is morning am cortisol. There is random serum cortisol and there is 24 hour urine tests. There are also salivary tests, but I don't use these very much so I won't comment much on them.
    If your cortisol is in the upper range, that does suggest this is a contributing factor. You should check multiple times because cortisol can be quite variable and can change from day to day depending on what's going on, what you've eaten etc. Serum cortisol levels for most labs around 16 or higher would be in the upper range with 20 mg/dl being high. So now that you have a high test, what are we going to do about it?
    Lower DHEA-s By Lowering Cortisol
    Keep in mind, if your cortisol is high based on testing that's one thing. If you don't know your cortisol is high, these suggestions may actually hinder your progress. Don't go off of a feeling of stress and assume you have high cortisol. You should check to know and also check with your doctor to make sure none of these things are going to interfere with anything. Generally these things are very safe. The first one to mention that can lower cortisol mention is Ashwagandha. Ashwagandha is in a category of herbs called adrenal adaptogens. There's multiple herbs that are in this category but Ashwagandha seems to be quite favorable. People respond well to it and do not have problems with it. Adaptogenic herbs work by balancing out the activity that they work on. In this case it is the adrenal glands and cortisol.

    Very informative video. My age is 25 and DHEA S Level is 567 alongwith serum cortisol (3-5 pm) level 24.7. My testosterone level is 0.5 and FSH is 5.7 and LH is 4.6
    Kindly suggest, if this report indicates some disorder.

    My DHEA (not sulfate) is 4200 on my dutch test. It's not supposed to be above 3000....my cortisol is also low. My DHEAS are normal but on the higher end of normal. I'm scared AF.

    I can tell you one thing doc anxiety attacks are super uncomfortable with these cortisol spikes so to speak

    Thanks so much for your videos! I have high DHEA-S but low AM cortisol levels and other cortisol levels throughout the day are normal, I did a 24 hour comprehensive urine test. My ACTH levels are also normal, CT scan on my adrenal glands came back normal, and my STIM test came back normal. I have no idea what’s causing my high DHEA-S levels but I want to learn how to correct it as I’m having negative symptoms. Any advice you can recommend would be much appreciated. Thank you!

      Have you figured this out yet? I have the same exact thing for so many years and no one knows. Not a single doctor that I’ve been to and it’s frustrating. I just want to get back in a healthy balance

    I'm 39 and just found out my DHEAS was too high. I do not have PCOS because my AMH is normal (1.2 = average or good for my age) and so is FSH (7.6), estradiol, LH, prolactin and so on are also all normal. Why is my DHEAS high? It's not new I think because I've always been hairy all over (white, hardly visible hairs though) and have always had problems with acne. I eat healthy and take supplements and have no deficiencies.

      We have different numbers here in Sweden because we measure it in umol/L. Normal level DHEA-S for my age (39, turning 40 this year) is 0.8-7.3 umol/L. Mine is 9.6 umol/L.

    I am a HRT patient with elevated DHEA 858 Cleared of tumors cancer Cushing’s and Addison’s my end of things that I have CAH but he’s not experienced enough to tackle it

      @@swintegrative - I see an Endocrinologist monthly, I also get injected weekly with Testosterone. Im a cash payment patient so you can imagine my of pocket cost just to feel some what normal on a daily basis. Physically I look like a shredded gym rat mentally and internally I feel crummy a lot of the time. My Endocrinologist has already told me that he has never seen a male with CAH in the 18 years he has been a doctor. Im currently in the process of trying to find a doctor in Texas who has experience in treating CAH in males. I had do to a bunch of testing and blood work back in January I tested negative for Cushings, Addisons, Tumors and Cancer.

    I'm wondering if you can you take DHEA with 5HTP?
    I saw that it's not reccomended to take DHEA with SSRI as it can increase seratonin to very high levels, but what about with 5HTP?

      Yes you can. DHEA does not have direct serotonin effect.

      Hi thanks for your comments. It is hard to say but there is no direct link of high cortisol and breast cancer. There is of course the link between stress and cancer but that is too general to be useful.

    Thank you very much for videos... I have one question...Can high DHEA level cause androgenic alopecia (woman)? my testosterone is at normal but I suffer from hairfall so I am worried... cortisol from blood was 758,5 nmol/l... Have a nice day

    Hello Dr, Thanks for your video. Generally it's a female problem but what is the scenario for men?
    i have elevated DHEA-SO4 levels(624) and i am suffering from hair loss, oily scalp and other thyroidish problems for more than 10 years.. I'm 33 years old MAN. There are lots of info about for low DHEA-S but not high DHEA.. What do you recommend for my case? Thanks in advance..

      The same things that cause high dhea-s in female can do this in males too, cortisol,/stress, increased enzyme activity. You have to check the various hormone levels to find out which enzymes are involved you might need to be screened for adrenal hyperplasia.

    is L theaine good for lowering cortisol? also can stress lead to estrogen dominance?

      Hi Alec, I imagine that L theanine might lower cortisol but Have not used it that way. High stress can increase the overall estrogen in system but this probably would not be enough to cause estrogen dominance. However, it is likely that estrogen dominance causes high DHEA.

    Whats diagnosis for high dhea and low cortisol while breastfeeding?

      Hi, there is not a definitive explanation for this based those numbers alone but I would expect your hormones to be different while breastfeeding. possibly 3 beta hydroxysteroid dehydrogenase enzyme is down-regulated.

    Thank you so much for this, it’s very eye opening and interesting to know. I have extremely high DHEAS levels but I have not been tested for cortisol levels I’m not sure why. Would extreme hair loss be a factor in high cortisol levels?

      Yes you can but birth control can make cortisol higher than it otherwise would be.

      It’s hard to know for sure without checking the other hormones and knowing the actual values. However, one possibility is down regulation in 3 beta hydroxysteroid dehydrogenase. This will keep more dhea-s around. Keep in mind these things are usually multifactorial with lots of layer that influence other layers.

      It is probably coming from your adrenal glands. It really depends on your unique situation. I would have to look at your whole health history and lab values.

    Any idea why a man would have high dhes, prolactin, androstenedione however low cortisol and testosterone? This is currently stumping my endocrinologist.

      it would depnd on the actual numbers... possibly high estrogens estrone and estradiol.

      @@swintegrative Ya that's the trick here. The estradiol wasn't elevated. It was 19.7 (range 10.0-42.0 pg/mL)

      Never heard of this but cholesterol synthesis is needed for hormones. Not sure I would use a stain just to lower dhea but depends on the situation.

    Hi doc, I was told I have high testosterone which I believe now its all related to also having a High DHEA-SULFATE. So, now im trying to lower my testosterone & DHEA. I'm assuming that by controlling my High DHEA-SULFATE, that would automatically lower my testosterone? Also, can you make a video on how to lower high testosterone on females.
    Another thing, i feel like I might have a insulin resistance.. maybe thats why I have high testosterone or even high DHEA-SULFATE?? I haven't been told I have PCOS but with everything I've been told so far, and having irregular periods, etc it definitely related to PCOS.
