  • CJ McCollum of the NBA Portland Trail Blazers suffered an injury to his left knee against the San Antonio Spurs and we now know the injury is a popliteus muscle strain.
    In this video we'll go over the anatomy and function of this often poorly understood muscle and then review the injury mechanism to explain how this could happen!

    If this is your first time hearing about this muscle, the popliteus, don't worry, it's not commonly injured and isn't a common topic in the world of knee injuries. Hopefully this helps you learn something new and understand why it was injured!

      I injured my knee almost 2 years ago and never got it checked. It always feels loose and every once in a while I manage to hurt it again by jumping or turning. I always thought it was something to do with the the meniscus but I wonder if it could be this muscle (never heard of it before until now).

      I tore my popliteus and my lateral meniscus last December. Doctor advised me to get a PRP. Is this the way to go?

    Great video! Could you do one on Brandon Roy, played on Portland in the mid to late 2000s. He was predicted to be in the debate of the best shooting guard in the league at a time where Kobe and Wade were playing at some of their highest levels but had to retire before he hit his prime because of a degenerative knee condition

    This channel is awesome. The mainstream news stories don't give enough insight into what's going on with injuries to get a feel for how long a player will be out, so this helps tremendously. Plus, it's just interesting info on its own.

    I came across an athlete at my footy club (aussie rules football) that had a popliteus strain. Took me a bit of differentiating to rule out LCL, mensical and/or other structures but, based on his history of presenting condition (HOPC) and subjective assessment it was hypothesised that the damage was localised to the popliteus. With that said, prescribing exercise beyond the initial /acute RICE protocol was really tough (so I can empathise with your point). I decided to prescribe mainly functional things like squats and deadlifts with emphasis on good biomechanics (along with some soft tissue manual work) and the athlete returned to play within 2weeks

    Thanks doc. Finally found an explanation for my knee pain at the lcl and wrapping around to the meniscus side, which follows the path of the popliteus. Hurt it deadlifting with toes pointing out.

    Very helpful , I’ve been out for 1 year with a meniscus and popliteus tear 😭 my knee kept swelling but now I’m good , I just rested and did a lot of running without playing football as I’m a pro soccer player and that I guess helped to strengthen back the muscles 💪🏿

    I remember back in the 90s that a popliteus issue is what began the chronic knee problems that derailed Anfernee “Penny” Hardaway ‘s career.

    so while my MRI shows very little other than inflammation, my PT/Massage person felt it was my Polpliteus and Plantaris, both affected by something I did transitioning from running to walkingin the gym. Most pain I've had in a long time for an "unremarkable" scan. Great explanation of a muscle I was unfamiliar with. Thanks, I think I am on a long road fo it all two loosen up. At two weeks already.

      Did you ever heal? I’m at 9 months with pain in back of my knee and upper calf and still don’t know what’s going on. Got an mri on my knee but they said they didn’t see anything.

    Super topical - I had a client that had a PLC injury earlier this week, now confirmed via MRI. Hers injury wasn't due to an awkward landing event but rather repeated lifting events at work, which is probably why her PCL wasn't torn (just her popliteus). CJ is fairly lucky to not have a PCL strain as part of his injury - roughly 75% of PLC injuries involve the PCL.
    Edit: there is a test called the "Dial Test" which can detect PLC injuries without MRI, but a MRI will be beneficial after conducting a Dial Test to find exactly which structures are damaged. Rehab for PLC injuries involve lots of progressive sled pulls, and a lot of work on your hip abductor / external rotator complex (which will now have to work harder to compensate for lax/torn structures)

    I’m very confident that this is what I just tore last night running. My foot externally, rotates fine, and I have no pain in the muscle, but as soon as I go to internally rotate the tibia area, it is absolutely excruciating. Hoping for a quicker recovery, just using ice and ibuprofen. Thanks for the video.

      For future reference, ice and ibuprofen is a no go. Ice slows healing (as of 2015, the guy who “invented” it retracted his research). And pain is like yellow construction tape [do not enter this zone] of the body. Ibuprofen allows you to cross the yellow tape (cause you don’t feel it) where the fibers/tendons are repairing, possibly slowing down recovery or injuring it further. Be safe.

    I got this exact injury playing handball last week. Yes, an isolated popliteus strain. This video was really useful! I was so afraid that I was out for the rest of the season, but with a little rest I’ll hopefully be back in a few weeks time.

    I train in the gym and about 3 months ago, I did a hard leg day with no issues, but the next day it felt like my whole right leg was lit up. My gastroc was really sore where it inserts behind the knee, the lateral anterior portion of my knee was sore, my hamstrings felt very tight and my quads and sartorius were tight. I rehabbed a couple of weeks to let everything calm down and in the interim, had my knee x-rayed and an MRI, which shows degeneration from a medial meniscectomy 19 years ago. The majority of my symptoms have calmed down, but I have an intermittent dull throbbing that travels from the lateral top part of my ankle up the tibialis anterior to wear it inserts at the knee (this occurs primarily at night sleeping!). Also, I continue to have an intermittent tightening feeling behind my knee on the lateral side. Also tightening behind the entire back of the knee, which led me to your video regarding the popliteus. I’m trying to figure out if 1 thing is causing the others or if I had simultaneous injuries. Could the popliteus cause pain in the tibialis anterior, since the structures are all and close proximity? Also, I’ve noticed when everything’s calm down my knee is not sore, so I’m assuming the knee is not the primary cause of surrounding pain. It has been frustrating to try to sort this out and know how to properly rehab without aggravating everything. Thanks.

    Now I can understand why it hurts so much when I use my other foot to try and take off my running shoe on the other foot if I don't loosen up the laces. It puts a lot of pressure on the popliteus muscle.

    I have a pain in the back of the knee, but it is just when I flex it with a resistance, it doesn't bother me in Sport activity or running, i don't feel any pain when I jump or land, not even doing squats I find it painful...it's is just with that flexion against a resistance.

    How about 'overuse' during cycling?... with the foot clipped into pedals there is some 15 º outward 'float' built into the cleat. I have experienced this kind of injury following a number of sessions of offload climbing, long, smooth steep hills. The style of climbing involves being out of the saddle with the knees close to locking out at the top position of the relative crank rotation. At this position the power is delivered by pulling up ...and at times with that 15º outward float being incorporated. My background is from some 30yrs of long and ultra distance road running, cycling, Nordic skiing and rowing. So hopefully have a reasonable legacy of good biomechanics ?

    Hi Doc. New to this channel. I am recovering from knee surgery and I had two months long pain behind knee where it would give out. I went to PT but no one seems to know why or what the pain is. The popliteal region is where my pain is making it hard to flex knee.

    Doctor Sutterer,
    I have the same injury and after visiting several orthopedic doctors, I was diagnosed by a Doctor of Osteopathy that I have the inflammation/Strain of Left Popliteus Muscle which does not allow me out on pant while standing and cannot bend my leg more than 90 Degrees. It causes excruciating pain at the back of my knee. Where can I get the treatment as I have been unable to find and treatment in Austin, Texas.

    I have what is described as the poster lateral corner injury. I’ve been resting it with oral-anti inflammatory treatment and will return home to get imaging and a better diagnosis than I have been able t make for myself.

    Im currently having this popliteus issue after my post ACL surgery, now waiting for my next shockwave therphy, its really painful & cant predict when it can happen, its appear all of sudden once you walk/twist/rotate your movement😢

    Hi Dr Sutterer. I am a 45 year old basketball veteran that has played organized basketball for 35 years. I have had a lot of nagging injuries from overtraining but my knees have been healthy until recently. I partially tore (Md2) my ACL but I did not stop playing as I am already on a pain killer called Gabbapentin (2 herniated lumbar discs). I have adjusted my game to fewer change of pace and direction moves and I am careful to get a solid plant when i jump on that knee. Should I opt for surgery before it's too late or could it have healed in these 3 years without incident? I really want to play ball as long as I can.

    Hi doc. I am a dental hygienist and I have been researching this for the past week trying to figure out WHYYYYY I am having such excruciating pain of the posterior area of my left knee. I ended up at the ER unable to walk last friday. X-rays show no signs of any arthritis, and I am waiting on blood work to rule out any autoimmune issues. My exercise routine consists of walking 2-4 miles four to five times per week with my Doberman. I have had no trauma to the knee whatsoever. I am scheduled to see an orthopedic surgeon tomorrow for more evaluation. ANY insight would be greatly appreciated. At this point I am at a loss an Tylenol #3 doesn't even take the edge off of the pain. 🤷‍♀️🤷‍♀️🤷‍♀️

    I hurt my knee during soccer season and my physiotherapist told me I had a popliteus injury. I had a gigantic bruise on the back of my knee which I found strange, is the bruising common? is it a sign of tearing the muscle?

    I’m dealing with this now, such a bizarre injury. Was externally rotating my right leg while extending in full X guard In jiu jitsu. Look a long time to get an mri to show that it wasn’t a tear to the meniscus, but contusions to the popliteus tendon. Been a long slog with physio.

      @ took a while, and then it ended up being a misdiagnosis once the mri results came and I had torn my patella tendon. It healed before the mri, and took a specialist to see. So then two months off after that and now I roll no problem. Just still aware of lots of x is no good.

    I have a posterior medial meniscus root tear with extrusion. I have no pain or swelling and no mobility or flexibility problems. I get soft grinding that comes and goes spontaneously. Is there anything you could suggest that might help with the soft grinding. Thanks Dr

    Do you know how long it takes to be healed? because i think i injured my popliteus 2 weeks ago. I have been seeing a physiotherapist 4 times. The pain is already gone when I was walking but one time when I started running after 2 km, the pain came back. In total, I have been off from running about 2 weeks since the injury. Do you have any suggestions? Thank you doctor!

    Did i didn’t even know you could strain your popliteus I honestly thought it might be easier to tear your acl because the muscle wants to rotate and but anyway congrats on 50 k

    Is it possible to injure this muscle and if someone was decondioned, and they descended a large outdoor staircase? I think I did this. ....also would an MRI show this? Thank you!

    Can popliteus burst? Yesterday something happened when I took a step like something burst and a noice like something snap.
    Can't stretch out my leg now.
    Pain in the back of my knee and even in back bone upper leg muscles, can't stand on my leg.
    What should i do?
    Emergency doctors took an xray on skeleton, nothing showed at that. No mr-xray to see what happened with muscles.
    But something really happened that gives so mutch pain that it's even hard to relax.

    Yeah always while walkin.. and strain Also it can move to the hand and shoulder and chest ? I feel always pain on my left side of body sometimes I feel like that lump is moving as traveling 🙄😭

    pretty sure I injured only this tendon by wearing too tight support hose. Months of pain and swelling, saw a video on a stretch for this muscle and 80% better in 2 days. No more cane, no more limp.

    Wouldn’t external rotation of the tibia make the popliteus lax instead of tight and predispose it to a strain?

      Nope, it runs from latera femur to medial tibia. So if you hold femur in place and rotate tibia outward, uou pull that medial side even further forward and the popliteal muscle wraps further around knee so to speak
