There's a good chance that for every gold restoration you do, it'll outlast a kid who goes to college, gets through dental school, places a crown on a patient, and then that crown fails years later. I guess that's me! I have a gold occlusal inlay on #19 placed in S. Korea at age 7. I still have it in my mouth at age 25 and I am going into my D3 year in the states! I'll be seeing patients in the clinic next month. It's sad that gold restoration is looked down upon (among my colleagues and professors) but mine's doing great and I don't plan to replace it per some dentists' advice. It shows when I open wide but back in time my parents wanted the best for me and went with the doctor's recommendation.
I placed 3 gold crowns and prepared another gold onlay on a dentist this past week. If the restorations are on molar teeth, who can see the gold, and, in my 40 years of clinical experience, gold margins are the best and gold is the most durable material. I have had to replace 2 e-max crowns on molar teeth in the past month. Any tooth colored material, including zirconia, can be broken if a patient bites something hard on it. With a few modifications, you can have gold inlays that fit so well they look" painted on" . I am glad to see that you are aware of the issues with the thermal coefficient of expansion and contraction, which is roughly 13 times that of tooth structure ,calling to question the value of any composite based restoration.
Only small composites are OK in my opinion for the reasons I stated. Large ones do not hold up, leak, etc. I try not to perform any procedures I cannot stand behind.
I really like where you go into detail about your reasoning for doing things the way you do them. Given that gold is getting more and more expensive, would you considering doing ceramic MO/DO inlays? I have started doing second impresions for when its more than single unit, but I do reversible hydrocolloid + alginate as tray material, seems to give accurate impressions easily!
Great. The thing I like about gold, if aesthetics is not an issue, you cannot break a gold restoration and the fit is incredibly accurate. I have patients in my practice who have had gold inlays and onlays for 50 years. Anything tooth colored can break. Gold cemented with resin cement is unbeatable.

I have used reversible hydrocolloid and polyether with custom tray for over 40 years. Look it up on Google. You must have canceled trays, a hose for water and tempering baths, but once you have those items the material itself is inexpensive and, according to the studies, reversible hydrocolloid and polyether with custom tray are the most accurate impressions. If a client would like high noble yellow crowns for #’s 29 &30 but have amalgam in the upper molar # 3 would galvanism be a factor due to the two different metals.

You did the gold inlay really well, but why in the first place did you treat these teeth? Both had just a shallow decalcification. With all the knowledge about preventive dentistry we have these days, non invasive treatment would be in place. Instruction on how to properly clean interdental, advise on a healthy diet, combined with a decent monitoring and local fluoride application can be very efficient and should always be in my opinion the first 'treatment'.
As you know, only about 1/3 - 1/2 of the decay present is apparent on a radiograph. This patient had significant decay on other teeth, some requiring endo and full crowns. How many times we can replace the inlay after it gets wear out on same tooth?.. Does every time of replacement more and more tooth structure is drilled?(like normal fillings) Or the old tooth structure is completely conserved and only new inlay is replaced at the place of old?..
Gold inlays are so durable you will never replace 99% of them. The only real reason for replacement I can think of is a fractured tooth or decay. If you place the patient on a good home care, diet program, hopefully those thing will not occur. You are correct, it could have been either tooth. Access plays a part in the decision which tooth to restore with the inlay and which to restore with the one surface composite. I normally restore the tooth with the most interproximal decay with the inlay. I do love gold inlays when there is plenty of tooth structure remaining so that the cusps do not have to be covered to prevent tooth fracture.

Possibly. If a tooth groove is stained but there is no evidence of decay, I would clean out the groove with a tiny mosquito diamond burr and place a sealant. I will have a video on sealant placement posted shortly.
It's normal, even if your teeth have no holes. Just keep cleaning with sensodyne toothpaste. Of course, if you think you have holes in your teeth, you should get them filled.

In my opinion, gold restorations are the strongest, longest lasting restorations dentistry has ever developed. I saw a patient in my practice today on hygiene recall I had placed 8 gold inlays and onlays 33 years ago and they looked as good now as they did then.

I agree with your assessment of hydrocolloid, I used it for many years, excellent material but not great for all situations so I made the switch to Impergum polyether when dealing with implants starting in the early 1990's. I am STRONGLY disappointed that you would say you are using a rubber dam when in fact while using a split RD not covering the papilla you are not really getting isolation when doing a (poor) composite. Without the proper technique (using a RD correctly placed covering all gingival tissue you don't get isolation ) Using vasoconstrictor to stop cravicular fluid is lazy and ineffectual. I would also suggest you consider watching Stevenson Dental Solutions on how to finish and polish gold. I have a whole mouth full of gold restorations and I did countless on my patients for 35 years.

Correct. Onlays of conservative full crowns for MODs to prevent tooth splitting.

Update what? Gold is still the best restoration if the tooth is not in the aesthetic zone. I have patients in my practice who have had gold inlays, onlays or crowns for 50+ years. Unfortunately, tooth colored materials are not that long lived.
