    Trigeminal neuralgia is a relatively rare neurological condition involving compression of the trigeminal nerve. The compression of the trigeminal nerve leads to particular signs and symptoms, including characteristic facial pain. In this lesson, we discuss the pathophysiology as to why trigeminal neuralgia happens, the signs and symptoms, how it’s diagnosed and how it’s treated.

    Thank you so much I just found out I have this. I was in the emergency room in two states, my primary care, and oral surgeon. Now I have to go to a neurologist. They all think that’s the best place for me.

    Thank you, I have this, but I didn't know what it was, it just started and I saw my doctor and he couldn't figure it out. I'm over 50, and it started like 3-4 weeks ago. It happens at least once or twice a day now.

    I have been recently been put on Emgality, since the nerves that cause migraines and TN are similar, hoping this will help. We'll see

    A Sonicare toothbrush at least 1-1.5 minutes every day has saved my sanity. When using this toothbrush I can go a year without pain. Any episodes last a week or 2. It takes about two weeks of consistant use to get relief, so you have to power through the pain for that time. Diagnosed in my late 30s, stopped trying meds when I had a bad skin reaction to oxcarbazepine. Also they Autoimmune Protocol for nutrition and lifestyle to reduce overall inflammation in your body.
    Additional Note about toothbrush: for me, if I have a flare up, I use the toothbrush to massage my face (both sides, not just the side with TN pain), making sure to get the whole area from eye brow to jawline and nose to ear. No more than 1-2 min in am and pm. This has been working for me for almost 8 years.

      Benzocane based gel, (oral tooth gel) e.g. 20% oragel is very helpful, it doesn't last too long when you have realy long periods of pain, but putting it right at the back of the hinge of the jaw on the side that has an issue can cause short term relief for severe pain

      Thank you thank you thank you!!!! My face has been in throbbing pain the last 2 days worse than ever. I’ve had moments where I was in constant pain for 2 weeks but never this bad. I started tapping on my cheek after reading this and it took a lot a bunch of pain. It’s still there but it has taken the edge off. Tomorrow I’m buying a electric tooth brush so I can really massage the area properly. I’ve yet to see a doctor but at least now I know I can survive until I get a date and time to see one. Thank you so much for sharing this information.

    My experience of TN involved pain when I ate, talked and blew my nose. I was diagnosed with TN about three years ago by my gp. Specialists at the hospital thought it might be Temperomandibular Joint Disfunction (TMD or TMJ) though. One of them recently tested this and anaesthetised the nerve by injecting into my gums on the side of the face where the problem was. As soon as he did this, the tingling/little electric shock sensations I was experiencing when talking to him went away. I've now been prescribed Tegretol (carbamazapine). I'm not too happy about taking the tablets, but will do so for now until I make further enquiries about any surgical procedures that may be available. I live in the UK.

      @@ianjoe1678 Mine comes and goes and hopefully yours will too. I've come off the tablets now, and, to be honest, I don't think I will ever take them again because I don't like the side effects. It's a bit sore this morning, but there's no tingling or little electric shocks. I'm finding the cold or damp in the air can make it worse. If you can cope with the discomfort until it goes away, you might be okay.

      Mine happens suddenly when I'm sleeping. I get a cramp in my neck then before I even have time to react it feels as if the right side of my face has been smashed in with a baseball bat. I've been told many times that the nerves in my neck aren't connected to the nerves in my face so I don't understand why my neck triggers my TN. When I massage my neck, if I hit that nerve on the right side just under the base of my skull it cases a TN flare up.

      i’m glad you got diagnosed. welcome to the club lmao. i got diagnosed with MS when i was 16 and i found out, i have a higher chance of developing this. i’m 20 and i have TN as well. all of it sucks

    I have Trigeminal Neuralgia the pain is so awful. I've found extreme heat from a heating pad helps and upping my neuropathy meds. Thank you for the info the compression is very interesting because I have vasculitis. It's been always assumed related to my Lupus, RA, neuropathy. Thank you.

      @@raniqueblackman8096 the best way I've always explained the pain is to imagine the worst ear ache, toothache in every tooth on one side and a bad sinus infection all at the same time. The pain around my eye and sinus is always the last thing that starts but I can control that a little better because I start with upping my Lyrica and Cymbalta and the heating pad before it starts or gets to bad.

    I also had kind of simptom. I'm using Carbamazepine and it make my body to a state of relax. My condition is ok for now but I can still feal the pain just for a moment when I'm eating or when it's getting cold.

      I have pain from the top of my head around both foreheads Jaws chin, collarbone. This pain last 3 hours or longer. At least I know there's an end or I don't know what I would do. I have other variables like a benign tumor at the end of my tongue that might be pressing on a nerve... Fibromyalgia, at dinner time death in my neck, and I guess cranial on pain

    See a neurologist for TN. I was put on gabepentin & prednisone if your primary dr can help with medication.
    It started with a chipped infected tooth that was pulled. My dentist took x-rays found nothing.
    I have a numb left face, twitches, eye pain coming from my neck stenosis. Physical therapy was suggested. Pinched inflamed nerve from my neck.
    Improving after 4 months.

      I hope you feel better than before becareful of gabapentin and Prednisone steroids, it will get better try to eat better and slow down jaw and neck , what helps me is an antibiotic not amox or amoxicillin clav something stronger , it does has something to do with the mouth and there are a lot of nerves in around face that teeth and gums do , I use a hand massager and massage my face head neck every where massage it helps a lot alot , and not chew as much I have tmj it's terrible, so I try to not overdo jaw movements by chewing tough meats or fruit veggies, also tendonitis is terrible too the Nevers get affected, I hope u are doing ok

    I find it intriguing that you didn’t mention chiropractic or biofeedback as a therapy. I found this information on an MD site, even though these two modalities were listed at the very bottom. For me, and I’m not suggesting anything to anyone, I have found that chiropractic works best for my condition. Apparently the cause for me was a structural problem which could be alleviated by such, although I’m not recommending it.

      @@Chs3__1234 I have scoliosis and was in a motorcycle accident at a young age, landing on the side of my face. I tried biofeedback and it didn’t work either. Only when I have a chiro adjustment as well as laser therapy does the problem resolve.

    Carbamazepine had no effect. I'm on Gabapentin now. It took a month to kick in, but seems to be working . . . at the moment !

      You need medications or the very last, an operation. The pain will not stop and will start appearing more frequently. Check it out with a Neurolog and he might suggest doing an MRI.

    My brother has had TM for the last 14 years. He is suffering from pain all day and night. He had the procedure where they suck something from his face, put the camera in the nerve from the face. And give electric shock. It's a very painful procedure. Also he has been taking medicine ( tegoral, gabix or lalap ) for almost 7 years but he still suffers from pain. We are very worried about him.

      @@Thisguy2736he should be fine? Have you tried living your normal life with severe sudden onset pain? It’s excruciating and can debilitate your entire life. I’ve been dealing with something very similar for the last 4 months. 35 years old, healthy, worked out, ate healthy, etc. All the sudden I started having extreme pains in different areas of my body and most recently jolts of extreme nerve pain into random teeth. Unless you have been there don’t say it’s not a big deal bc it is. A lot of the ways to get rid of the pain are really bad for you like using opioids and stuff like that.

    Have you thought that chronic sinusitis causes this? Inflamation in sinuses? Solty water drops cleaning of the sinuses is one of the best handlings I tried, comparable to no other in its effectiveness and without use of drugs, also cheap and always available

    I have pain in my right side face when I go in cold temperatures like fan air AC outside cold temperatures..pain started...but when I get worm air from any heater then I feel okay...I don't know what is the problem

    I live in a small village in the northern part of Sweden. And got this knife stabbing pain and was absolutely sure it was a tooth.
    Two weeks prior to this, I had my tooths checked by a dentist, so I called her and got an appointment. She examined me again and took some new x-ray images but ruled out problems with my tooth. She was almost sure it was neurological and told me to get an appointment at our health care centre in our village. The doctor was almost sur it was Trigeminal neuralgia so I got Pregabalin (what it's called in Sweden) and it helped. But after a while, I had to increase the dose, and then when I had my blood tested, they said that I couldn't increase the dose anymore. So I was sent to a neuro specialist who gave me 3 options.
    1 continue with medication (which I didn't want. By this time, occasionally, my fingers would twitch because of the medication)
    2 they could push a needle to the trigeminus nerve and with a balloon squeeze the nerv so it more or less died. This could fix the pain for a couple of weeks to up to 10 years.
    3 They could make a microvascular decompression. Where they drilled a 1 Inc hole in my neck and then put some Teflon padding between the nerve and a blood vessel.
    I went for the third option. And after that, I've been out of pain for 4 years.
    That surgery did cost me something like 70 usd (or in Swedish 700 kronor), but that was not for my medical treatment it was for the cost of having a bed and the food while I was at the hospital.
    Yes, we have high taxes here but I won't complain about that.

    When would surgery be indicated if patient has been suffering for many years off and on. And my episodes last several days in length. Upping my Lyrica and Cymbalta seem to help but mostly I'm stuck laying on a heating pad in bed a minute or two off and pain comes immediately back.

      i started visiting dentists and doctors, 2 specialists diagnosed neuralgia trigeminus.
      i received a cocktail trough a needle right under/inside my chekbone (i was desperate becuase the pain was constant)
      one day after that an abscess formed above the tooth i treated right before i went on holiday
      turns out the dentist infected my rootcanal during the tretment, i let them clean everything and the pain went away.

    I have trigeminal neuralgia due to a botched surgery at age 18 to remove an eye. The surgeon hit the trigeminal nerve and it is excruciating. Only one medication has ever helped and if I don’t get it I cannot think or function. It’s from the left orbit working out towards the nasal area, and then the head itself.

    My mother suffered from this and I fear I might suffer from this as well... My pain tolerance is extremely high, or moreso, my perception of pain is somewhat distorted, so I'm not sure though. It's hard to find if its actually a hereditary disease

      I have tn. The pain is on another level. It's worse then having a combination of the worse tooth ache ever with sinus burning and pressure in and around my ear and face. This episode has lasted 6 weeks this time and I'm taking prednisolone for it. It stops the pain being unbearable but still pretty shocking to be honest. I'm managing to work but only just. My mum and dad didn't have it but my mum did suffer terrible migraines when I was a child. I belive mine came from a previous injury as a teenager although it never raised its head till I was 43. Good luck and don't worry because its rare and very unlikely hereditary

    i turned 14 march of this year and i had this horrible pain since June of 2021, it’s on the left side of my face and sometimes goes and comes. i’m so scared because i don’t know how to fix this, no meds help me and when an mri was done, they found nothing. it hurts most when i’m in a mask for a long time because of the heat. this is rlly a suicide disease

      i hope you all feel better, i’ve noticed that stress actually could take a toll on this pain which rlly sucks. idk if you’ve tried this but try using a towel with hot or cold water on it and see if it relieves the pain, for me cold water worked best but for some it’s different. but be careful because it might trigger it more. also try to not touch and massage the area too much as it sort of awakes the beast😭 massages definitely feel relieving but i’m afraid that it could actually cause more pain. this might sound kinda dumb but if u know the movie venom, i kind of accepted this thing as a venom in me. the pain right now for me is veryyy minimal and i’m really grateful, for me it was just to be patient and wait. i really wish you the best results in your treatment because i rlly think no one should go through this 😕

    IMO it is stemming from Cervical Instabilities. The Atlas /Axis Vertebrae pinching the Vagus Nerve will Yield Havoc to the branch nerves . TMJ , Tinnitus, Lightheadedness, Sinusitis... and lots more . When my cervical issues from disc herniations, Military Neck from Forward Head Posture (commuted by car for 48 Years), DDD in the discs, and C4-C7 issues inflames the Occipital areas of the neck into the face.     I have severe bilateral dengerative jaw joints disorder . And I'm 100% sure it is causing this. The pain is severe and absolutely renders me. My maxillofacial surgeons don't understand this and I'm still only just taking paracetamol ! I'm not on any heavy duty pain meds. I work and have a 7 year old and when these attacks happen (I had one this morning ) i genuinely feel like jumping out of a window . Does anyone have any recommendations for natural pain meds ?! I used heat if needs be but that's not doing as well as before and it leaves my face feeling numb . I'm struggling a lot :( my maxillofacial is refusing a double jaw joint replacement due to mw only being 29 so I'm out of options I think :(

    I have this. It’s a allergic reaction. It’s caused by mast cell activation disease. I take lyrica, Zyrtec, Montelukast, and Metformin. It takes up to 8 weeks for mast cells to turn over. My reactions are decreasing in severity. I don’t have a blood vessel on top of any nerves.

    Got pain on left jaw, spreading all over left face. I got no issues with chewing or talking, but pressure just stays there n about. I experience shooting pain few times a day. There are no signs of inflammation.
    I had root canal few months back n pain started few days back.

      See a neurologist for TN. I was put on gabepentin & prednisone if your primary dr can help with medication.
      It started with a chipped infected tooth that was pulled. My dentist took x-rays found nothing.
      I have a numb left face, twitches, eye pain coming from my neck stenosis. Physical therapy was suggested. Pinched inflamed nerve. Improving after 4 months.

    I’ve been diagnosed this kind of illness about 6 weeks ago and the doctor prescribed tegretol 200mg twice a day to me but the point is that medicine didn’t help with the pain at all and it makes my symptoms get more worse! I’ve told my doctor about that problem but he still ask me to continue using that medicine. I’m so stressed because every time I take that medicine my pain, numbness and tingling get more worse. 😢 should I go to find the treatment with another doctor?       there are also some solutions by operation, although for some people not permanent, it can be pain free for a couple of years at least. Depending on the operation method.

      Is like you are passing the same situation iam into. I have tried many doctors but still no solution. It was a friend pharmacist that recommended me Alprazolam(xanar) 1 mg a day. The medication help me to calm the pain and you feel normal like a human. I take 1mg everyday and it control the pain the whole day. Now Ian reducing to 0.5 mg. A doctor told me is not a long term use medication bit i really don't care provided it is given Mr pain relieve that I want. I take green tea to detox my body. When people don't know the gravity of pain people who have this condition pass through, they will tell you all sort of things. Don't use this , use this, avoid this. I am now interested on what give me pain free any day I took it than what doctors said. If doctors can't find the solution, then i should go for what helps. I have used lyrica, pregabalin 150mg and it makes my situation worst. But this Alprazolam is given me relieved everyday. How I calmed my nerve i don't know but it does

I was 2 months in with cluster headaches, 3 hour episodes and about 4-6 times a day. He told me that he saw an episode of drugs inc, you can actually find it on RUclips “drugs inc magic mushrooms cluster headaches”. I saw the video and felt “well what do i have to lose” I bought an 8th for $30. Split it with my friend and tripped for 6hrs while my gf at the time watched over me.

      Hey fellas, how is you experience with this? I’m worried that I have it because yesterday out of nowhere got pain as it is described. Electric pain in the face. Like in the jaw below the ear.

      @@dejectedsnow4127 it's likely your wisdom teeth!! That's what it ended up being for me and haven't got them out yet but the feeling left after one week

    I'm suffering at the moment it came on suddenly today feels like getting hit hard, with an ice pick just in one spot in front of my ear, its been on and off for hours it comes on suddenly of course there's an OOUCH or an OH S***. Its not funny and its not a headache. No wonder its called the "suicide head pain". ❤

    Can anyone in these comments help me plz I got an apointment on jan 20th but im in so much pain every strong meds ain't touching me and im on the brink of suicide.

      There is some evidence that Vitamin B-12 can reduce the pain of Trigeminal Neuralgia in some people. (Search on Google Scholar.) Generally, the preferred form of Vitamin B-12 is Methylcobalamin in sublingual Lozenge form. (Not cyanocobalamin). I use KAL brand, a combination of Methylcobalamin and Adenosylcobalamin, at a dosage of 2,000 micrograms per Lozenge. The Lozenge is very small. (Other brands are fine too.) Vitamin B-12 helps to repair the myelin sheath of nerves. It will take weeks, at least, to see improvement (if it does help). Meanwhile, consider calling a nurseline (if your insurance provides one) or, if things are unbearable, visiting the ER. If there's any chance your pain could be from an infected tooth, don't ignore that. I'm Not a medical professional and this isn't medical advice.

    Hello everyone, I am a language teacher and one of my triggers for my TN is speaking quickly or for more than 10 minutes at a time. I feel that I am unable to work until I find a way to manage my attacks? Is anyone having trouble get a doctor's note for work? I've never been on medical leave and my PC doc only takes me off for 2 weeks and expects me to work and talk all day. Anyone have any suggestions? My PC doc doesn't help me at all or give me treatment options. She doesn't seem to know much about my condition.

      Keep strong, I deal with this since I was 23, now I'm 29, every and every day despite the pain is worth living. I found some alternatives as well through body and mind control and now I hardly have crisis.

    I have facial pain..cheek pain. Jaw hang. Pain around eyes. Forehead pain. Eye squinting.
    I have chronic sinus too. What specialist doctor should I go to???
