For me, it was Toybiz X-Men Spy Wolverine. I remember as a kid thinking oh cool, another Wolverine action figure! Then I move his goggle type mask out of the way.... hmm okay. He looks like Sylvester Stallone with googly eyes. Good figure, bad head sculpt design.
Yes, that Luke was poorly sculpted, but back then Hasbro was making figures from specific scenes in the films. This Luke was clearly designed for the scene where he cuts Vader's hand off. If you used a Vader figure to grab the rail and kept Luke's arms behind the back it's good for a throne room diorama.

That JJJ figure was from another repaint, reuse set. The head was an alternate head for Chameleon. The body looks like a recast of Mister Fantastic. since there wasn't a good suited body to use, I guess somebody thought the cloth clothes was good enough. That person was wrong. Very wrong.
The Episode 2 were an interesting experiment really, if you were a kid you didn’t really ask much, and “on/off” action on the removable sabers was cool; if on the other hand you were an adult, you could easily collect the figures sealed keeping the shelves appeal, or put them in a diorama on your desk

I think a lot of Transformers fans would say G1 Broadside is the worst. Total brick in all modes, no features, bland color choices, and the scale is absurd. However, be prepared to hear a very long rant about Action Masters and/or gold plastic syndrome if you're brave enough to ask a TF fan about bad Transformers.
I'm surprised the TF Action Masters weren't listed. I read about how these figures arrived late in the line and took away the gimmick. TFs that couldn't TF, stuck in bot mode.
That Wolfsbane looks like it actually has a pretty decent sculpt but the hair is ridiculous. They probably should have gave her flocking similar to like what Playmates did for the original Movie Star Splinter from the 2nd Turtles movie which makes me wish Neca would release flocked variants for their Splinters.
Star Wars POTF2 released an atrocious R5-D4 that had a feature where it split open and shot a projectile. Even if those first few waves in 1995-96 had them too muscular (more or less following the trend of the times), this one was just ridiculous. Later Hasbro SW figures that were pre-posed, from the late Power of the Jedi through AOTC and the Saga line were just awful, too. There was one Obi-Wan that had a spin-kick feature and it had one leg that stayed perfectly straight and the figure pivoted to simulate a roundhouse kick...
All the main characters were buff on the Power of the Force relaunch line. Guess Luke got so bored on Tattooine, he started working out. Only Vader made sense since David Prowse had been a bodybuilder.
Ironically, the Meteorbs formula works better in the Ultraman toyline, in which they have egg-shaped toys that transform into any of the monsters and aliens Ultraman fought.
I tell you what I think is underrated, is knockoff figures that you would find in cheap stores, of wrestlers, transformers, etc etc. As there is a creative imagination to it that I find charming.

the Maximum Sweat figures are actually a lot of fun big massive chunks of plastic never used the water " sweat" gimmick but Road dogg is terrible looking
I have always thought the Aunt May figure looks like Gary Busey in Drag. Also I think the worst looking TMNT figs would have to be the mutating turtles and the pizza blasting turtles. They both have terrible sculpts and the mutating turtles have their back legs being used as front legs and it just looks bad
