Treating Floaters Not Usually Considered Floaters

  • I am using a YAG laser to treat a broad, diffuse vitreous membrane. This is the this 'sheet' usually attached to the retina in youth, and peels away from it in the event of a Posterior Vitreous Detachment. They are often not easily seen on examination, but this one is a doozy and pretty much spreads out as a diffusion membrane affecting the entire visual field. They are technically difficult to treat and require persistence, but I was able to improve the condition significantly and avoid the need for a more invasive surgical vitrectomy.

    That is how I feel my floaters are - a couple bigger “sheets” that cover much of my field of vision, rather than lots of smaller pieces. It’s so difficult to ignore.

      It is super common and even now I am only really starting to understand the important of treating these membrane sheets. They are very difficult to see or appreciate with typical eye exam methods which may explain why your local doctor doesn't seem to impressed by your eye exam findings.

    My floater looks really similar to that! I commented on another video about its shape, but it does change a lot (when i look the side or up is looks concave). its very dynamic and large... What would've caused this so suddenly at 22? No eye injuries, no uvitis recently... I'm a biochem student so if you have chemical explanations that'd be cool! Thanks for your time:)

      There isn't good science to explain floaters in younger, otherwise healthy people. It might be systemic inflammation/oxidation chemicals and cytokines that spill over from the retinal blood vessels into the vitreous changing that local and otherwise very simple chemistry.

      The newer, UltraQ Reflex has a cooling fan and is a workhorse. My older Ultra Q has much better optics. When I am struggling with the Reflex, I'll switch over and see stuff I didn't even know was there.

    Hi Doctor,
    I had an eye injury 2 months ago, small steel shard entered my cornea.
    Got that removed, but since I (just like this patient) see a haze that passes my peripheral.
    The only thing is: it passes my vision in the opposite direction.
    When looking left to right, it crosses my vision right to left, same goes for up & down.
    In front of the haze I see regular floaters too, those move with the eye movement.
    I kind of interpret them as a standing up circular form in my eye.
    The front part moves with my eye movement (small floaters) & the back part moves in the opposite direction (gaze).
    Is this even a thing with eye floaters?
    I visited 3 ophthalmologists, all say everything checks out... Yet I'm bothered by this blur/gaze that crosses my vision 24/7. 😒
    Is there some kinde of center point in the eye where you could experience the floaters moving in opposite direction?
    (Can't really find anything on this, only found very few people online with similar symptoms)

    Hello Doctor Johnson, I just want to ask if having a pvd always lead to ERM or they say macular pucker? Do all of your patients who have pvd has macular pucker having floaters or pvd after years of having it? Thank you

      PVD rarely leads to epiretinal membrane/pucker. That is not an expectation at all.

    Would this type of floater sink to the bottom of the eye eventually?
    As I have been told they will.

      Not likely. Think of this membrane as a really loose drum head. It remains attached at the vitreous 'base' way out in the periphery, but 360 degrees attachment.

    With a PVD, is the membrane sometimes still partially attached? Would a vitrectomy remove this?

      I have both the Ellex Ultra-Q and Ultra Q 'Reflex". I will do a video commentary on the differences between the lasers. That is very observant of you to notice. The newer Ultra-Q Reflex has better cooling, is quieter to work with and I'll often start with that laser. Truth be told, the original Ultra-Q has slightly better optics, and I find myself often switching to that for some of the more difficult -to-see floaters. They are complimentary to each other and have different strengths and weaknesses.

    Hello james I have a different eye problem please explain!That is my eye both vision good,I have some eye floaters from 2019..But last 6 months always seeing some like rainy lines or spider net on the vision,that time objects not shaking but the lines wobbling or shaking..when I see straight view that time under the eye making shake and shadow.. please let me know how to overcome this.

      Are you in India? If so you can find Low dose atropine 0.01% at the pharmacy. I have great success with that symptomatic treatment in younger patients like yourself.

    Does this laser treatment cause any pain in the eye?

      No. There is no damage to tissues and no inflammation in the way we would expect with other surgeries. I am delivering energy to mostly water and inert proteins.

      Had to look it up: "Promosm" = The term "promo sm" in a RUclips comment commonly stands for "promote so much" or "self-promotion," indicating that the commenter is trying to promote their own content, channel, or product. This type of comment is often considered spam and is typically discouraged within the RUclips community.
      That's a snarky first time complaint on my channel. If you read through all the comments on my You tube Channel, you will find overwhelming gratitude and appreciation for the information I provide that is not available elsewhere, and even especially when the floater-sufferer has been to their own doctor and only got dismissive platitudes of reassurance. I spend a lot of my time answering peoples question and the vast majority of that is not being monetized. I am sitting now at my home office on a Saturday answering almost every question in the comments section of some 40 You tube videos and I will spend hours doing so because people are worried and panicked and anxious about their floaters and don't know where else to get information.
      If you concern is about people promoting their businesses on You tube and feel compelled to comment on that, you have a Sisyphean task ahead of you. Since you are the only one here who does not appreciate my work, you might be better served to spend less time on You tube and instead work on some introspection.
