I really liked this approach to loading based on current condition. I could only just manage tier 2 when I started and worked through to tier 4 over several months. I had a 15min routine each morning to start the day - the trick is consistency and patience. Worked a charm.

Good demonstration. So, I understood you right, that the program should recognize the patient's step of problem and generally start one step before. Is there any basic rcommendation, when you should switch to the next loading step. Is patient's complaint a matter of descision to step back?
I have a history of achilles tendinopathy. Few weeks ago doing calf raises I heard/felt like a slow paper tear but I didn't feel any pain. But I noticed after a few weeks my load tolerance dropped, when I play sports I am having flare ups. Did I partially tear my tendon lifting?
In theory, the eccentric heel drop should be done before you can do a normal concentric single leg full (concentric+eccentric) calf rise, so I would put it right before it interms of loading.

I used this for Achilles tendon rupture after wearing the boot, also works for that.

if you do the seated with weight make sure to have the plate vertical on your knee so the weight is distributed over tendon not throughout your thighs

No it is just exercises based on the amount of load on the Achilles tendon.
