I have sentient spear that was created when a charlitan made a wish to an Arch Fey to become the greatest hero in the world. The spear forces a character to perform feats of heroism. It's +2. You are immune to fear but cannot retreat from battle unless you pass a wisdom saving throws.
I recommend checking out Creed's Codex: Arcane Secrets of Summoners. It is a 5e supplement with a Summoner class, but it also has sentient symbiotic gear, like weapons and armour.
