    Don't let the multiple sexual partners thing fool you. YOU only need to have had sex with ONE person, if that person has had sex with multiple partners! Stay careful out there people.

      Agreed. Homosexuals aren't at more risk than anybody else either. Once you have sexual intercourse, it just takes one encounter with someone else - kissing for example - to get you infected.

    The number of men I meet thru dating apps who genuinely do not believe in condoms or safe sex is APPALLING. So concerning... syphilis is on the rise!! 🙅🏼‍♀️🚫

    I’m a nurse practitioner and work at a wound care clinic. I got a hospital consult for someone with “cellulitis” but turned out he had a bad rash that was also on his palms and feet. I included syphilis as part of my workup and sure enough that’s what it was! He was there for a whole other reason and just happened to be “lucky” enough to have a rash while there.

      I remember learning about it in sex education and our school nurse filled in the gaps from it can also affect your brain, and I thought you were usually sick with it too.

    I remember seeing a patient many years ago with tertiary syphilis who had been raped by her father who had the disease and transmitted it to her as a child. To see what that disease had done to that woman’s brain impacted me so much.
    Thank you so much for all of your videos, but especially this one, as so many people are so cavalier about unprotected sex now. Now with such effective AIDS cocktails, there is far less awareness of the dangers of unprotected sex and the STI’s that can occur.

      Seriously, safer sex needs to continue to be talked about. HIV is not at all a fun disease to have, even if it doesn't automatically end people in droves anymore. Other diseases we used to say more about are also undesirable for very good reasons.

      I used to see Tertiary Syphillis when I first started my nursing career in the early 1990's, we'd see a few women whose husbands brought home Syphillis in WWII and they all were not caught until it was too late. They were in the end stages of the spirochete infection destroying their neurological symptom.

I know that hurt people hurt people, and that most people who sexually abuse others were sexually abused themselves, but that case hit me so hard that I still remember that unfortunate patient so many years later.

    Testing for syphilis was required before a marriage license would be issued in Florida until the late 1980’s.

      I think that was true in many states. I got married in Missouri in the ‘70s and it was a given that you would have to get your blood tests before getting a marriage license.

      @@xmx005 it’s so selfish! Even in couples who have an agreed upon “arrangement.” I know a couple where one partner wasn’t using protection 100% of the time and also had wayy more sexual partners. Now they both have herpes. Luckily, it’s type 1 so not as severe.
      This is why therapy needs to be normalized and easily accessible too, because that behavior was driven by underlying issues. However, they refused to seek help and just wanted to push it down and “power through.”

      It wasn't! But all these politicians are defending reproductive health clinics such as Planned Parenthood so people aren't getting affordable preventative care anymore. The rest of the health care systems can't manage the influx of that many patients and also patients are too ashamed to go to places not Planned Parenthood where they treat patients without shame.

      Practically no disease is truly irradicated. There is almost some very minute, geospecific place that harbors these often thought irradicated diseases.
      The data is there, you just have to look for it.

      Syphilis was never eradicated. It was managed by people going to planned Parenthood. Those are now closed in red states. Red states now seeing resurgence and at later stages and tragically hurting children.

There's so much "information overload" in our culture nowadays, that people rarely seek out additional information on something that they don't think is directly impacting them. I love biology, so I'm a nerd for this kind of information. . but I had no idea it was making a comeback.
      Very cool of Dr Dray to make a video on a less than desirable topic- just to get the word out to us! Probably doesn't benefit her channel, but she acknowledges that the information needs to be passed on, because she genuinely wants us to have the information we need in order to be healthy.     I'm happy you're talking about this. I'm a retired reproductive health nurse and syphilis is definitely on the rise in California. We treated average 15 clients per day for different stages of syphilis.

    You hit the nail on the head about letting up on education. Over a decade ago in health class, we were told not to worry about syphilis. Education is key for preventation.

    Thank you! I contracted syphillis in 2016. I was also a meth addict. It is very big with users even there are in denial.
    I blame the "sexual freedom" culture. That straightened me up real quick.

    A final note for the end of this wonderfully informative video...
    If you do ever contract syphilis and are successfully treated, you will from that moment on always test positive for it however it doesn't mean you still have the infection. As I understand it, it is measured in something called titres. Basically you're blood remembers that you have had the infection and when the titres are measured at a low level it means the infection is no longer present. However if it rises again it means you have been exposed to a new infection.

    As a nurse I worked on several dementia units. We had a new resident come in, very sweet, but in time she started having behaviors that we were having difficulty dealing with. Eventually her blood work showed she had Syphilis. This was so tragic because it meant 2 things- for it to eat away at her brain and cause her dementia, she had to have had it for many years. Number 2- she most likely got it from her husband ,and he never told her he had it. Men usually have symptoms. Of course there could have been other scenarios, but the family thought them unlikely. So tragic. My other experience with Syphillis was on Career day in High School. I had chosen to tour the State Hospital- our local mental institution- where one of the female patients was admitted with it. I wasn't allowed to know her story, but she was totally out of touch with reality and unable to care for herself. This is from having sex people, and not disclosing your indiscretions to your current, or future partner. It's tragic.

      I know a guy and his x gf told me about her grey discharge in 2021, I warned him but he continued all over the place and with his new gf who I believe and have been telling them both that they have syphilis......... she's turning into Howard Hughes crazeeeeeeeeeeeee behavior and their skin stuffffffff.

    Many people with end stage disease without a diagnosis. tertiary syphilis is long term syphilis, as a Registered Nurse in Florida, seeing more and more in elderly and immigrants.

    Dr dray, thank you. A friend of mine was recently accidentally diagnosed in the er. Without the ER, he would not have been treated. Very dangerous and no one talks about it. Very important topic, thanks for educating the followers ❤❤❤

      My ex went to the ER because he had a rash on his foot and they told him it was syphilis he jumped my butt when he got home thinking I gave him syphilis. I stopped and asked him, did they do any blood work or tests of any kind? No. Guess what?!? It was an allergic reaction to the laundry detergent!

    My friends son died of syphillus , TN , 15 years ago. He kept being misdiagnosed as schizophrenic and wouldn’t take the prescribed meds because they didn’t help his brain that was eaten away like Swiss cheese by the time he was seeking medical help. All mental health hospitals should test for this and hopefully prevent , or at least track and trace an infected persons partners and get them medical intervention. I didn’t know that syphyllis was still a problem in our country or anywhere . Thanks for educating everyone as most don’t realize it’s out there and can damage and kill you .

    I have a friend who used to work in public health and she said that syphillis is becoming antibiotic resistant. It is wise for you and your partner to get STD tests before sexual activity.

    I have the impression a few years back HIV served as a driver to catalyse everybody's attention around safer sex practices and testing. Unfortunately in recent times HIV is not a focus anymore in the public debate and other infections such as syphilis - which as u say is a bacterial infection and not a virus - tend to go unnoticed.

      People are much less alarmed by HIV these days, and I don't know why. I just heard someone say this past weekend... nowadays HIV+ just take a few pills per day and they live a good long life. 😮

      @@pamelarodriguez5674 yes there are multiple reasons in fact that span from the absence of campaigns (and related budget), sexual education in schools, under financing of health services (in the countries where welfare provides for your health needs such as in my country)... also the way some concepts have been mainstreamed. See post exposure prophylaxis for example: an infected person with undetectable viral load is not infective in terms of the hit virus but could still be a vehicle of any other bacterial infection. It is a natural relapse from the years of terror in the 80s and 90s but at the same time this natural relapse should be managed properly while people have largely been abandoned to themselves or to some opinionated RUclipsr with no medical background. Especially in countries like the USA where the costs of testing and therapy are super high this is quite dangerous.

      @@themiasmastream I believe it is right and desirable to finally dismantle the association between sex and disease in terms of principles.. it's the way we are doing it that is a little sloppy: we are not going towards sexual education investing money in preventions, schooling, free healthcare for all but we are in fact regressing to pre HIV silence. Also the state invites people to test but at the same time, once you test and you happen to infect anyone, treats you like a serial killer on a killing spree while nobody ever questions the "victim" or the state itself that is ultimately - at least in most cases - the one that failed both parties not providing proper education and tools/free and equitable medical assistance etc etc. This is very contradictory and does not help in my opinion.

    this is a great PSA. it’s sad to see so many of my peers engaging in such frequent unsafe sex, often out of feeling pressured. your health and wellbeing (both mental and physical) are nothing to play with. in addition, anyone who doesn’t want to practice safe sex clearly doesn’t value you or have your best interests in mind- so you should probably rethink letting them gamble with your health. it’s also important to get tested if you are sexually active even if you are in a relationship or married. many people believe testing is only for those engaging in high risk behaviors but unfortunately there are rising cases of women being infected with syphilis and hiv from their SOs. i wish everyone reading this good health and ability to advocate for your wellbeing and boundaries surrounding safe sex 💪🏼

      Amen! You have to put your own health first 👌🏼 I'm 15+ years married and I have to demand my STI screening at the obgyn and they say but you're monogamous and I'm like sure I am but I'm not willing to gamble my own health on another person's behavior 🤷🏼‍♀️

    Retired nurse here. Years ago (60’s - ?). Every hospital patient had syphilis test as part of routine admission. Had a male patient about 10 yrs ago who was 101 yrs old. Tested positive - got a huge dose of penicillin in the nursing home.

      I think people see syphilis as rare and uncommon. I know I did, until watching this. I only think about it in connection with the Tuskegee Study.

    Great information here, thank you Dr. Dray. Also, I’ve been coming across quiet a few videos by tiktokers and influencers regarding “Sephora kids”. These are the preteen and younger children buying skincare and cosmetics at Sephora. When I was growing up my mother taught me about appropriate skincare at the appropriate age. She explained why some products could actually harm my skin if I used them on my little girl face and body. Could you make a video on this. Some of these kids are buying retinols, chemical exfoliates, etc… very little supervision from their parents. Basically they let the child pop whatever they want in their basket, hand over a card at the register and let these kids go to town. Some parents need to learn ignorance is NOT bliss when it comes to unhealthy skincare habits that are forming very early.

    I was dating somebody last summer and we always use protection and he kept on going on and on about not wanting to use protection anymore and promised he was clean. I finally convinced him to go and get tested and lo and behold he had syphilis. I went and got tested as soon as I found out and even though the test came back negative I still asked to go on the treatment regimen just in case. Can't be too careful out there

      I grew up in Alberta. I remember going to several remote towns for field trips and seeing community health warnings everywhere for Syphilis. Fort Macmurray, slave lake, high level….it blew my mind. Remote areas seem to be the most hard hit.

    Thanks for this important video. I am a syphilis investigator at the University of Washington. This infection is far from being a disease of the past, but it is on the rise in high-income countries and still endemic in low- and middle-income countries. If left untreated, syphilis can progress to affect a patient`s cardiovascular and central nervous systems, potentially leading to death. The most significant death toll due to syphilis, however, is due to the ability of its causative agent, the spirochete Treponema pallidum subsp. pallidum, to cross the placenta and infect the fetus. Congenital syphilis is the most common infection associated with fetal loss or stillbirth in low-income settings, with an estimated ~1.4 million pregnant women infected every year in the world, resulting in 305,000 cases of fetal loss or stillbirth, and 215,000 infants born prematurely and/or with clinical syphilis. In the US, syphilis still affects primarily men who have sex with men, but the recent increase in cases among women of reproductive age has resulted in 2,855 cases of congenital syphilis in 2021 versus the 334 reported in 2012. Lastly, there is evidence that symptomatic syphilis increases 2- to 5-fold the risk of HIV acquisition and transmission. A great sexual life is a must-have, but be safe everyone!

    Back in the 90s i had a bout of vertigo. My PCP told me all the lab work he was going to do, then apologized he was testing for syphilis. I had no problem with it. Don't be ashamed or be in denial.

      Guys are very lax about protection and most girls know this. So im always shocked when I hear someone say they had unprotected sex with a dating app match. Like 😳 you realize if they did that with you…they’ve prob done that with a bunch of other girls too??… (all my friends are straight women so that’s the only perspective I can speak on).
      But a lot of that behavior also stems from low self-esteem and being self-destructive.

      This is actually an epidemic among older people as well-because pregnancy is not longer a concern they are less likely to use prophylactics.

    I’m a traveling cytotech and I was shocked by the number of patients with Syphilis. I’ve mentioned it to a few peers and they didn’t seem as surprised or concerned. Thank you for speaking on this.

    Something to think about when you find out your partner has been cheating on you when you're supposed to be in a monogamous relationship. Not only is it emotional abuse, but you could get a deadly disease.

      They may have contracted syphilis before they even met you and not know it. Always ask a new sexual partner to do a full panel STI test and show you the results on paper. That’s the loving and romantic thing to do.

    Yes, this is why screening is important. I hope you do one on gonorrhea as well, which, as Dan Savage's podcast pointed out recently, needs to be tested in various places of the body.

      Back in 1982 my then husband had Gonorrhea. He had difficulty urinating and I sent him to the doctor. Turned out he had Gonorrhea! I was 8.months pregnant at the time and they tested me but thankfully I didn't have it. He tried to gaslight me into thinking I gave it to him! Dude! I'm 8 months pregnant with your baby! I divorced him and never remarried.

    My mum works at a small gynecology practitioner in Southern Germany and more and more cases with Syphilis come in.
    It is mandatory to test for all the sexually transmitted diseases in Germany when you’re pregnant. They diagnosed a young women who just got married and must have gotten it from her husband.
    So it’s not just in the US, it’s a similar rise in Europe.

    A friend of mine, a prosthodontal professor, told me that he recently saw a bad case of oral syphilis in a patient who came in for dental work. I would hope that dentists, as well as dermatologists and FMP’s/GP’s , would be very cognizant of the symptoms and stages.

      U.S. dentist here. Yes, we are trained to include in our tentative diagnosis and frequent c.e. courses in oral pathology keeps my skills honed. Have seen two cases in the past year and I'm a pediatric dentist on call to an urgent care clinic. And recently syphilis was the answer to a case study in JADA. Oh yeah, we are looking!

    Great video! Good job on educating people. One of my favorite pianists died of syphilis and I’m pretty sure he caught it from his mother (during birth), who worked in a brothel in the early 1900’s.
    Scott Joplin was his name. We still have his music but he was taken way too soon.

    FYI Neurosyphilis can happen at any stage, as well as syphilis of the eye. So def be aware of that, as I had it even tho was at secondary syphilis stage. Now treated and fully recovered

    As a public health worker in communicable disease, I SO appreciate this! Especially with congenital syphilis on the rise in pregnant women. Women are often not routinely screened for syphilis until pregnant or at delivery. If you’re sexually actively at all, get tested 💜

    I remember seeing the pictures of syphilis for the first time during biomedical lectures and was horrified! You just never forget the damage it does. It’s forever etched in my brain. Thank god we have treatments for it now.

    I remember learning this in grade school… I remember “spirochetes” and how these organisms gather in the brain and cause people to “go mad”.

      I have a somewhat similar memory it's been a few years I definitely remember the information about going mad and it getting the brain and I don't remember if it was syphilis or gonorrhea that effects your central nervous system and that was awful. Still a firm believer in a love glove too.

    It’s very sad we have enough to stress about. Thank goodness it’s treatable. Thanks Dr Dray for talking about things that can help people and reduce shame which hopefully reduces stigma and which reduced infection. In general we don’t see the education and outreach isn’t there like it was in the 1990s/early 2000s

    I had it before. Only reaction was throat issues that felt like the flu, and an ear infection. I'm sure you can deduce how I got it in my throat. Treatment was 3 shots of bicillin over 3 weeks.

    Thanks sm Andrea! You inspired me to get a full blood panel appointment. Thanks also for mentioning that it could be unsafe to contact a past partner and resources for that AND that lack of access is part of the problem. I’m in california and IEHP is a great service, but getting it, was a super difficult process, and I’m about to lose it. Before I qualified I had private insurance through my job, but it was so expensive I couldn’t even afford to use it. I hope one day we can all just have universal health care without worrying about losing it or not being able to afford it. Appreciate you.

    It is really important to talk and aware everyone about such crucial matters such as syphilis. It is no shame by all means doctors are here to diagnose and treat. Also there is no fear or any sort of negative emotions about such matters our health comes first and bypasses these meaningless obstacles of fear and criticism. We should enjoy every bit of our lives and be sad (of course some moments are dark and depressing but as i always say the sun will rise again tomorrow bringing a new fresh day full with endless possibilities and if they is not a sun tomorrow remember it's still there behind the clouds so do let your hopes fall down 😊❤) stand up and cease the opportunities that are given to you. Thank you so much Doctor Andrea for enlightening us on such serious matters. As i know the full title is board certified dermatologist and venereologist 👍.

    I love you so much. We are learning about syphilis in nursing school right now and we talked about the increasing rates of congenital and how detrimental it can be to the fetus/newborn.

    I was shocked with the numbers of new syphilis cases during my work in a health clinic. The treatment is cheap, the public power must do something to increase testing and treatment. I am in favour of active search with health care providers visiting people and providing guidance. Here in Brazil we have this for free but some areas are still underserved. If not cured in time, syphilis can be severely damaging for the brain

    Thanks doc, for this video. Unfortunately my mother almost died of it, and she was diagnosed with neurosyphilis, in the tertiary phase, and she had tabes dorsalis. This was less than 2 months ago. One year ago, she had “back problems” that made it painful for her to walk, but was otherwise not very different from the mom I always knew, and as of 2 months ago, she can no longer take care of herself :(
