Romans 2:29 explicitly states, " But he is a Jew, which is one inwardly; and circumcision is that of the heart, in the spirit, and not in the letter; whose praise is not of men, but of God." So it would appear that the references in Jeremiah are to Christ's church and not just people outwardly proclaiming to be the children of Israel. People can convert to Judaism. People can become Christian or Muslim. But that does not necessarily bring them into the inheritance of God.
