- If you have congenital (by birth) ptosis, you would wonder if surgery would make it perfect. Prospective brides and grooms worry if the in-laws would be able discover their defect once corrected by surgery. Others want to know if surgery can help them successfully tide through the eye fitness exam in competitive jobs.
The simple answer is: if your ptosis is 'congenital', surgery cannot make it perfect. We can only make it better. Certain limitations of surgery are explained here, so that patients understand their inevitable nature, and stop blaming their surgeon/procedure.

I have very slight ptosis on my right eye since I was 18 and now im 20. Will this ptosis stay like this or will it worsen over time as I age and drop more and more? What can I do to prevent it from worsening? I dont want to go through surgery considering the side effects and also because it doesn’t affect my vision just yet. After congenital ptosis surgery, any chance that the ptosis may reappear ?? I mean the outcome of surgery remains permanent or not ?
If the ptosis is severe, it will 100% recur.
If it is moderate, it may or may not recur
If it is mild, the results are mostly permanent.
I had sever ptosis i did surgery in 2019 but since last 2 years it’s started reappearing

Sir, mene 1 month 12 days pehle ye surgery kar waya tha. ab mere eye normally blink nehi hota, sote waqt eye thoda sa khuli rehti hai or niche dekhne se eye weird lagti hai. Doctor ne kaha 3 months wait karo, phir bhi thik nehi hoyi to aakh ki andar jo hai wo remove kar dene ki try karenge, kya ye possible hai doctor?? Remove karne se kya mere eye pehle jesa ho jayegi kya,koyi problem to nehi hogi?
The description you have written are inevitable side-effects of any ptosis surgery performed for severe ptosis that is present since birth. These side-effects are not dependent on the quality of surgeon, but will occur is all patients who have this condition. If you wish to look good in straight gaze, you have to accept these side-effects. They donot disappear after 3 months, they will stay as long as the effect of surgery stays (usually 2-4 years).

Hi Doctor, I am not sure if you’ll see my comment since the video was made 2 years ago and I just found about you.
I have a congenital Ptosis on my left eye and it’s pretty different in size compared to my right eye .. I am unable to look upwards without tilting my head high so I am considering a surgery but have a hard time selecting the best Surgeon in this field for assurance to a successful surgery . If you don’t mind, may I know where your hospital is located and how much money do I have to prepare ? Thank you
The more important issue isn't about the cost or which surgeon, but about being able to fully understand the limitations of ptosis surgery when performed for congenital ptosis. Many patients have the mis conception that it will blink and move like the other eye. Unfortunately, surgery can only increase its static position in straight gaze. In down and u-gaze, it will look odd or asymmetric. Also, surgery for severe ptosis has a limited life, and requires revisions. As long as you understand this, the surgeon does not matter much, as results are comparable when you choose an eye plastic surgeon.

I'm 40 years old and I had congenital ptosis on one eyelid... an operation was performed when I was 3 years old (surgical shortening of the muscle - a method with a scar on the eyelid).. the eyelid behaves differently after operation... it opens when i sleeping , I have to fit this eyelid, so that it stays closed for night......when i looking down ,my eyelid is more open than the healthy eye...years later my operated eyelid not looking good
All the findings you have described are classic post operative expected outcomes and are limitations of this surgery. It is wrong to believe that surgery will make your eyelid like the other eye. It does NOT. It only improves symmetry in straight gaze and side gaze. In downgaze, it will look odd. So the surgery does not solve your problem, it simply changes it. This has to be discussed in details before surgery by each surgeon.

Here are some of the reasons why you face delays at LV Prasad Eye Institute:
1) At LVPEI, we perform a complete examination of both eyes, and not just spectacle prescription, or cataract. Complete examination takes time.
2) The back part of the eye (retina) can only be examined after the eye aperture (pupil) is widened with specific eye drops. This can take 20-40 minutes, adding the wait period. If this step is skipped, nearly 4/5th of your eye is left out without a check up.
3) At LVPEI, we have over 20 specialists on any given day in the clinic, who specialize in different sub-sections of the eye. So if you are found to have an abnormal finding in any of them, you get an opinion from the best in that field, on the same day, at no additional cost (this is called cross consult).
4) LVPEI does not tightly control appointments like a visa office. This allows more patients to have access to eye care, but in return can add inevitable delays.
5) LVPEI is one of the few eye hospitals where eye emergencies from all over the country are referred. The doctors have to divert some of their time to these unpredictable additions, and hence the delay.
6) Finally LVPEI is a teaching hospital.Training is imparted to young ophthalmologists, and this takes time due to interactions. While this may add to the delay, we are making sure that your children and grandchildren continue to have good eye doctors long before you and I are gone.
So yes, we do have delays, but they are all for the right reasons, most of which I am proud of. We always work toward minimizing delays, but I tried to give you a broader picture of this fact. Hope this helps.

You can try Upneeq eye drops. They are however only available in the USA. An alternative is Brimonidine eye drop, which may lift the eyelid for an hour or two. In case you plan to use it for an important social event, first try it out before that. Use it on your important day ONLY if the trial worked.
There is no non-surgical alternative.

If it is congenital and severe/moderate, you will have downsize lag and incomplete closure of the eye as long as the surgery is successful. In severe cases, a typical surgery lasts for 2-4 years only, and requires revision thereafter. So short answer: Do not expect 'full' recovery with exact symmetry to other eye. That is possible ONLY while looking straight.

Risk hai, lekin chota sa. 95% cases mein eye adjust ho jata hai. 5% cases mein dry spots ki wajah se additional drops ya surgery karna padta hai. Sau mein ek ka surgery reverse bhi karna pad sakta hai.

I tried to study about this disease as one of my close friend has this disease. Most studies have stated that in case of LPS resection, downgaze lag resolves within 3 months. Is this problem for lifetime or really resolves in most cases?
If your ptosis is since birth, the downgaze lag stays. Yes, it may improve a bit during the first 3 months of healing, but it can never match the normal eye.

If the ptosis is 'congenital', incomplete closure of the eye is an inevitable side-effect that has to be accepted. It is a give and take. Without this drawback, there is no surgery. However, this is only applicable to ptosis 'by birth', and the gap depends upon the severity of ptosis.
The best advice can be given by an eye plastic surgeon in actual consultation.

Legally, disability is decided by permanent vision loss, and ptosis does not come under the same. My advice would be to stop using ptosis as your disability, and consult an eye plastic surgeon to explore if it can be corrected.

There is no scientific basis for the advice you received. Ptosis surgery can be performed at anytime, or any age, for one of the following two reasons:
1) If it is reducing vision by blocking the view (usually relevant in first 8-10 years of life)
2) If the ptosis is cosmetically bothering the patient

For any ptosis from birth, after surgery you will definitely have two new problems:
1) Incomplete blinking during the day
2) Incomplete closure during the sleep.
The degree of this new problem will depend upon the severity of the ptosis.

Myasthenia gravis is treated medically with tablets. In majority of cases, surgery is not required. However, if your eyelid muscle is slipped from its normal place (disinsertion), you may be the rare myasthenic patient where a surgery may help. Only an actual check up can answer if you are the right candidate for surgery. Rarely, sling surgery may be done in cases where medications aren't helping.

The fact that you are talking about a sling means the patient has severe ptosis. And for severe ptosis, no matter which sling material you use, the ptosis is bound to recur. Expecting a permanent result in a sling is science fiction.

Mr Adiba, The issues you have described are part and parcel of any sling surgery done for severe ptosis, irrespective of the material used. The choice or material or the skill of the surgeon is not the cause of your concern here. What you missed out on was having a detailed discussion with the surgeon prior to the surgery.

Face wash with soap is possible, by gently closing the eyes so that there is no gap. Eye staying open has no effect on sleep.

Ptosis can be corrected even in a blind eye. If you are cosmetically concerned about the ptosis, it needs a check up and a surgery if possible.

Sir can we blink eye properly after ptosis surgery
If your ptosis is since birth, blink as well as closure at night will be incomplete. The more severe the ptosis, the more incomplete will be the blink, and higher will be the chance of drying and redness. So after surgery, you may look better, but may not necessarily feel better.

Sir mera bhi same problem hai but vision affect nhi hai toh kya surgery karana chahiye
If vision is not affected (which is the case in most patients), whether to get the surgery done or not is a personal choice. When the reason for surgery is cosmetic, the doctor will not advise it, but we can perform the surgery for those who are interested in it for cosmetic reason.
