This is really sage advice that you just can’t buy. It comes from experience. This young man is in the same situation that I was in at his age. Except by 23 I had already pulled the trigger on two hair transplants and by 23 all I was left with was scars in the donor region and scars along my hairline. The only part I would have qualified to audition for was Mary Shelley’s ‘Frankenstein!’ I then went on to wear a hair piece for 25 years. In this patient’s case as Joe has pointed out he can go about this the right way and consider a natural looking hair piece in life and in acting roles. It worked Sean Connery and many others today. If he feels that it’s not an option then he isn’t left with chasing the tiger by the tail all his life (as I did) and he will still be scar free. If there is one thing worse at 23 than the pain of losing hair it’s losing hair with scars on top of it. Often in life the hardest thing to do is the best thing to do. If I could go back in time I would have saved myself from hair transplants that didn’t achieve my goals (and leave me with scars). As hard as it may feel now, when you look back in years to come, you now have that comfort. You will know what I mean as time passes. Definitely the Hair System will give him the result and its instant. Your advice was spot on Joe. Being young and having Hairloss like this is nightmare that unfortunately you are just unlucky, but the good hair system they have these days is definitely a way to get around it as long as you are open with close friends that way, they wont make fun of you, and you dont have to be worried if every second person can tell.

I feel for him, but I definitely think that the flexibility of theatrical hair is the way to go for this guy.
Another good assessment, Joe. He definitely needs to give the Proscar/Propecia some more time to show full effects, perhaps he'll be able to maintain his current situation for while yet and still with the PRP and hair system options available to him.
I can definitely relate to this guy having had the same hair loss pattern at an even younger age. Firstly whether it's a hair system, hair transplant SMP or concealer -- The cure can't be worse than the disease. What I mean is if you are going to pursue any form of hair restoration and it is going to make you even more self conscious than being bald because someone might "Find out" then it is better to be bald. When I wore a hair system while I had some comical mishaps it did look good. I stopped wearing it because I was constantly worrying if people knew I was wearing one. On the other hand I don't care if people know I've had a hair transplant so as long as it doesn't look freakish (plugs or bad placement) it's no sweat for me. I agree his donor hair looks weak and that means a HT needs to be very carefully considered if at all. That belief is furthered by the fact hair loss drugs may have slowed or halted the progression but what remains is not robust. My donor hair is weak due to genetics and being over harvested, but at age 45 I can confidently say it has not gotten worse in over a decade so I can use what remains. I totally agree scalp hair when suitable should always be used first for a hair transplant. That said it would be interesting to see how much body and facial hair this person has. If he were a hairy beast he might be able to pull off using some scalp hair in the hairline region and filling in most of the rest with body hair. That way if his hairline receded he probably could touch it up with remaining scalp hair in the future since it's hard to have a good hairline with anything but scalp hair. Body hair grafts can be a life saver but are no miracle. They definitely do not grow as well as scalp nor look the same. Overall great analysis. No one likes to hear they are not a candidate for a hair transplant when it's become the last resort but always remember. ITS BETTER TO BE BALD THAN HAVE A BAD HAIR TRANSPLANT.

I had no choice but to go with a system or multiple revision and improvement surgeries. Luckily I have thick curly hair and donor is good. I finally got it right through you with Dr Arocha with number 4 surgery! Maybe a 5th total un my future if I want to do some fine tuning. If not I'm fine with that too. I'm 54 now. I would stear him towards BladCafe, maybe he would be convinced to just rock the bald head look and overcome the insecurities. I went bald a year and a half ago and it was the best decision I've ever made. Everyone has their prefference, and everyone should take the path that makes them the happiest, but this does seem rough, especially for someone so young. That being said, love your videos man; you're very real and knowlegable, it's a pleasure to watch.

I was glad he asked the question regarding the plan to first do FUT and then an FUE but you chose to skip it. It is easy to find dozens of people making the case that FUE is not an appropriate choice for people with extensive hairloss due to transection rate, graft growth rates, inefficient use of the donor area, harvesting from outside the safe donor area and you can find an equal number of people who argue that FUE today executed by a skilled experienced surgeon can have comparable results to FUT in terms of harvesting efficiency and graft growth rates. I would be interested in hearing Joe Tillman's nuanced view on the subject in 2021. Joe if you were starting your hair transplant journey today with a full donor area, no previous surgeries and the same hairloss that you had in the 1990s how would you approach your hair restoration. FUE or FUT, a combination of both. What are the risks and trade offs. This would be an excellent video. (If it exists and I just haven't watched it yet please point me to it)
I got a buddy who recently went to a doctor for a free consultation. He is 40 years old and they told him that it would be a waste of money because the hair on top of his head is too thin. I had a transplant done 3 years ago, fue and it was only 2500 grafts in the front hairline area and it came out great. I'm not familiar with the procedure too much so I don't know if they can just implant around all the thinning hair or if those doctors were correct, that he wouldn't be a good candidate for a transplant. I've seen some very bald dudes online that have had some excellent results from doctors all around the world. I'm hoping that my friend can get a doctor to tell him that it's possible for him to have a transplant with good results.
His hair loss is extensive for someone in their early 20's and ideally, if I were him, I would wait for Proscar result first, as you stated. The problem I feel, is he's only thinking about a short-term solution to his hair loss by considering having a hair transplant and not longer-term prospect, which most people seem to forget about at around this age?

He needs to wait longer to see how(6 months is not a lot to see good results). I think mine were impressive and after 9 months I started to have strong results that continued to improve until the end of the second year (where I am now), and since his hair loss is diffused I think we may see a huge improvement in the next months and years: If you want i can send you some photos about my case. Now I am 28 (but started losing hair when I was 19 and noticing it when I was 21). At 25 I was bald and then started taking finasteride. Now i only have a bit of thinning on my crown that doesn't bother me at all and the rest of the hair is really strong.
He may need dutastride at proscar and use some ketoconazole shampoo. It also seems he has a sensitivity to testostrone too. He may need to consider fluridil or cb 03 01 for protection and maybe ru58841 on the more daring side. But things look pretty bad. The hair system route should have no stigma.
I had a lot less hair than that at 19. Simply not blessed in the hair genetics at all. It's not my thing but given this patient still has strong temple points, a hair system would suit him well.
Hello Joe, we have found out that dutasteride in microdoses have shorter half lives while doses above 0.1 mg is very potent. We are applying it topically to surpass 0.1 mg on scalp to get longer half life and more potency while applying weekly to make sure that the systemic leakage is kept under control to affect serum DHT minimally. Have you heard anyone applying topical dutasteride weekly or anything similar? We are over 200 people in Discord and still figuring it out so curious if you have any input on that. Have a good one
I waited until I was 61 for a transplant. I had not realized how bad the front was getting as I was so focused on the crown. They did 3200 grafts and the front came out amazing. I was a little concerned the top middle had thin areas and the crown still bothered me. I just had another procedure where they put 12 on the crown, 1600 on top and 400 around hairline. I think it will look amazing......oh, they took 1000 from my beard this time
I guess it depends on what you use as a metric for peak masculinity. If your metric is the strength of my Magnum PI moustache and the number of sharks I posed with, then yes. That was peak masculinity. It was also right before I lead an unofficial invasion of a Yemeni beach in a WWII era beach lander so we could have a beach party. True story.
For this kind of situation, would it possibly be a viable option for him to do some hairline and temple work and cover everything behind with a partial system? I assume this guy's main concern is looking youthful for work, and though the system would cover the top loss I don't know that there are options there for the temples. Even that could possibly be too taxing on the donor though.
He's too young for that as this approach requires absolute commitment and that can only come from wearing hair for several years to know if it is something he can live with. It also limits his options as an actor.
@@hairtransplantchannel It raises an interesting question about temple-point transplants - some of us who have lost temple hair hate the 'wide' head look that the loss of this relatively small patch of hair gives us - so a temple reconstruction of say 200-300 follicles on each side might give us a better hairline on the side of our heads that we could either wear with a 'bald' head on top, or use a hairsystem with. I generally think the bald 'horse-shoe' looks far better if someone has kept good temple points.
I'm so pleased that you told this kid to try a hair system. No way should this young man ever get a transplant. Not at 23, 25, 30....given how much he has already lost. However, he has temple points, so a good hair system would look great. Great advice Joe.
Joe, I’ve seen you talk before about technicians performing surgery instead of the doctor, and a clinic I’ve seen a lot from online is ASMED in Turkey. From what I’ve seen it looks like pretty much everything is done by technicians, but Dr Koray Erdogan is someone you’ve mentioned in other videos but not really talked about in depth. What are your thoughts on this clinic and what do you make of the fact that it is predominantly technicians performing the surgeries? Cheers.
I know the clinic well and I visited them in 2015 during a hair conference in Istanbul and I was impressed with the level of detail and their standards. They do utilize technicians, but I'm not against that in general. How could I be when my last surgery was performed by a technician?
Yeah beneath the age of 33, being bald is just not cool, I think he gave a valiant effort though with all the treatments, don't forget theres still actors who are bald and do ok, granted they usually compensate with being tough or jacked

Wow, that's a lot of hair loss for a 23-year-old guy. And he's been on Rogaine for 4 years, Proscar for 6 months, and done PRP??? And his hair loss is still this advanced at 23? I don't want to be a Debbie Downer, but with such aggressive hair loss at such a young age despite all of the measures he's already taken, I fear he may be a lost cause. If he really still wants to fight, with such aggressive hair loss he should immediately go on 0.5mg of Dutasteride daily along with 2.5mg oral Minoxidil daily.
He's only been on FIN for 6 months, that isn't even long enough for it to do much. I'd say most of the loss occured before the FIN and now the FIN has stabilized it. I'd say if he stays on FIN, he shouldn't have that much more loss. He used other treatments like Minox, but the thing is without a DHT inhibitor, NO other treatments will stop his hairloss! they will only improve it cosmetically slightly. DHT will still be ruining his follicles the whole time.
@@akashduggal1634 I'd say his hairloss started 4 or 5 years ago, so he got on rogaine. He should have gotten on FIN back then, not just rogaine. Because I bet if he got on FIN 4 years ago, he wouldn't have that bald patch, he'd just be a Norwood 2 at most. This is why there needs to be more info out there - DHT inhibitors are the ONLY thing that will save your hair!! Everything else will just thicken it slightly but the damage will continue to happen! Like with my Rat Analogy, if your wooden fence has RATS - Minox is like painting the fence. Sure it looks better and can cover some of the damage, but the rats are still there and they will continue biting the wood. Eventually the damage will be so bad that paint (minox) will no longer cover it. What you need is to get rid of the Rats (DHT), by using Rat Traps (fin). This will stop the damage. Traps won't kill every rat, and FIN won't stop all DHT. But it will drastically slow the damage down.
@@mikesully110 well said. I think everyone should consider 5ar inhibitors asap when they notice any early signs of hair loss and minoxidil is something taken in addition but. Dut and fin along with minoxidil should be used indefinitely but yeah always start with fin or dut or both.
dont waste your money on all the extra BS. Take finasteride and get a hair transplant. you do all that other non sense red caps and BS you'll end up spending the same about as a FUT.

For this kind of situation, would it possibly be a viable option for him to do some hairline and temple work and cover everything behind with a partial system? I assume this guy's main concern is looking youthful for work, and though the system would cover the top loss I don't know that there are options there for the temples. Even that could possibly be too taxing on the donor though.
He's too young for that as this approach requires absolute commitment and that can only come from wearing hair for several years to know if it is something he can live with. It also limits his options as an actor.

@@hairtransplantchannel It raises an interesting question about temple-point transplants - some of us who have lost temple hair hate the 'wide' head look that the loss of this relatively small patch of hair gives us - so a temple reconstruction of say 200-300 follicles on each side might give us a better hairline on the side of our heads that we could either wear with a 'bald' head on top, or use a hairsystem with. I generally think the bald 'horse-shoe' looks far better if someone has kept good temple points.

I'm so pleased that you told this kid to try a hair system. No way should this young man ever get a transplant. Not at 23, 25, 30....given how much he has already lost. However, he has temple points, so a good hair system would look great. Great advice Joe.

Joe, I’ve seen you talk before about technicians performing surgery instead of the doctor, and a clinic I’ve seen a lot from online is ASMED in Turkey. From what I’ve seen it looks like pretty much everything is done by technicians, but Dr Koray Erdogan is someone you’ve mentioned in other videos but not really talked about in depth. What are your thoughts on this clinic and what do you make of the fact that it is predominantly technicians performing the surgeries? Cheers.
I know the clinic well and I visited them in 2015 during a hair conference in Istanbul and I was impressed with the level of detail and their standards. They do utilize technicians, but I'm not against that in general. How could I be when my last surgery was performed by a technician?

Wow, that's a lot of hair loss for a 23-year-old guy. And he's been on Rogaine for 4 years, Proscar for 6 months, and done PRP??? And his hair loss is still this advanced at 23? I don't want to be a Debbie Downer, but with such aggressive hair loss at such a young age despite all of the measures he's already taken, I fear he may be a lost cause. If he really still wants to fight, with such aggressive hair loss he should immediately go on 0.5mg of Dutasteride daily along with 2.5mg oral Minoxidil daily.

He's only been on FIN for 6 months, that isn't even long enough for it to do much. I'd say most of the loss occured before the FIN and now the FIN has stabilized it. I'd say if he stays on FIN, he shouldn't have that much more loss.
He used other treatments like Minox, but the thing is without a DHT inhibitor, NO other treatments will stop his hairloss! they will only improve it cosmetically slightly. DHT will still be ruining his follicles the whole time.
I mean even if he were on dutastride it would require and year to a year in a half. He may also have a sensitivity to testostrone.
@@akashduggal1634 I'd say his hairloss started 4 or 5 years ago, so he got on rogaine. He should have gotten on FIN back then, not just rogaine. Because I bet if he got on FIN 4 years ago, he wouldn't have that bald patch, he'd just be a Norwood 2 at most.
This is why there needs to be more info out there - DHT inhibitors are the ONLY thing that will save your hair!! Everything else will just thicken it slightly but the damage will continue to happen!
Like with my Rat Analogy, if your wooden fence has RATS - Minox is like painting the fence. Sure it looks better and can cover some of the damage, but the rats are still there and they will continue biting the wood. Eventually the damage will be so bad that paint (minox) will no longer cover it.
What you need is to get rid of the Rats (DHT), by using Rat Traps (fin). This will stop the damage. Traps won't kill every rat, and FIN won't stop all DHT. But it will drastically slow the damage down.
@@mikesully110 well said. I think everyone should consider 5ar inhibitors asap when they notice any early signs of hair loss and minoxidil is something taken in addition but. Dut and fin along with minoxidil should be used indefinitely but yeah always start with fin or dut or both.
