How To Stretch The Piriformis Muscle When Feeling Pain: Do's And Don'ts

  • Stretching the piriformis muscle can help reduce pain to the buttocks, the pelvic and the hip region. It can also help relieve compression of the sciatic nerve. But how should you stretch when you're in pain? Can you go all the way?
    No, you need to take it easy. The stretching exercise shouldn't be perfect, because it will create more pain.
    I will show you the different versions of the piriformis stretch exercise: lying down, sitting on the floor or sitting on a chair. We will look at the differences, and see why I don't recommend the stretching exercises done in a sitting posture: they can cause lower back pain and there might be side effects to the stretch.

    Many thanks . I have the same but very sharp pain in the glute..but this pain only occurs at night just before I fall asleep. I am fine all day with no pain. Is this pyriforus scatia depression symptoms

    What benefit is derived stretching a muscle already so tight it is compressing the sciatic nerve causing the pain in fir place?. Please clarify for those of us who are not fully trained in physiotherapy.

      Good question, Charles. A muscle can be shortened and / or hypertonic. In both cases, this can lead to compressing the "pipes" that go through it (be it blood vessels or nerves). Stretching is one of the possibilities to reduce muscle tonus at rest, but also to lengthen it. However, this will suppose that you stretch without inducing pain, otherwise you will have a contraction reflex that produces the opposite effect. If stretching does not work, other solutions can be tested: EMS, heat, relaxation exercises, etc. Does this answer your question?

      ​@@OlivierGirard All valid info - going to try as much of these options as I can muster. Going for Orthopedic recommend physiotherapy. Need to address muscle imbalances and some necessary biomechancs posture corrections. Got my hands full on this one. Trying to stay mobile at over 80 yrs young...

    I have cysts in my right hip and groin area. They’re just small but they make it impossible to bring my knee to my chest. There’s really no flexion at all in that area. And there’s nothing else I can do to help it.

      Correct... I understand. Then, I would do 2 things. #1 is buy a chair like a Hag Capisco, so that you can sit with a hip angle more than 90 degrees. #2 is check out what electrostimulation can do for you as far as piriformis is concerned: ruclips.net/video/5cv77BH4jK8/видео.html (I use the Beurer EM49)

    Hello sir, do you have a video for piriformis stretch for novices who can’t pull their leg at the extent you did in this video? Should we just pull as much as it feels comfortable? Because from what I understand, we need to feel the stretch in the buttocks for the stretch to have a point.

      Hey dear! You need to feel something, but that shouldn't be pain. So indeed, you should use a belt or something to stretch only to such extent that you remain comfortable...

      @@iamtheone9242You mean the spots of piriformis, or of gluteus minimus? If you mean the spots of piriformis, it can be... But it's not always easy to locate the pain points precisely, and these one are located under your buttock muscles. So there are certain postures that allow you to go through the first layer so as to palpate the piriformis, whilst others keep the first layer contracted, which prevents your pressure from reaching the piriformis. Manual therapy is really an art which takes a lot of skill (that why I'm an hands-off therapist 🤣!). You can also try the test that I suggest at 1'04'' in the other video on the piriformis: ruclips.net/video/PHXmQ8Pt0Nc/видео.html, that will give you some more insight.
      Actually, the same goes for glueteus minimus: it lies under gluteus maximus and gluteus medius...
