  • Tinnitus, sometimes called “ringing in the ears,” involves hearing a sound that cannot be linked to an external stimulus. In this video, I discuss nervous system mechanisms that might underlie tinnitus.
    TRANSCRIPT:
    Tinnitus, sometimes called “ringing in the ears,” involves hearing a sound that cannot be linked to an external stimulus. The nature of the perceived sound can vary in complexity, but common manifestations include ringing, hissing, or sizzling. Prevalence estimates vary, but most studies suggest tinnitus occurs in somewhere between 10 and 15% of the population. Although most cases are manageable, severe cases of tinnitus can significantly impact quality of life. Tinnitus can occur as a symptom linked to a number of different conditions, and thus there is not a single mechanism that can explain all cases. The most common predisposing condition, however, is hearing loss, which is found in up to 90% of patients with tinnitus.
    The mechanisms underlying tinnitus are not well understood, but it’s thought that hearing loss might be linked to tinnitus in a number of ways. Damage to the cochlea, for example, may lead to increased signaling in other auditory structures to compensate for the reduction in auditory input from the cochlea, and the resultant amplification of neural noise may lead to tinnitus. Additionally, there may be a reorganization in the auditory cortex where neurons that normally respond to frequencies that are missing due to cochlea damage now respond to other frequencies and display abnormal levels of activity.
    Neuroimaging studies have revealed other potential abnormalities in tinnitus, such as increased activity in the auditory cortex and decreased activity in areas like the prefrontal cortex. The latter may be linked to a failure to inhibit increased auditory signaling. Other studies have found increased activity in regions involved with memory like the hippocampus and surrounding areas. These might be involved with retrieving a sound from memory that becomes the perceived sound in tinnitus. Activation of areas like the anterior cingulate cortex and insula, which are involved in identifying important stimuli, may prevent habituation to the perceived sound in tinnitus.

    ive had tinnitus for as long as i can remember, i still remember the night i discovered not everyone hears the sounds i hear. absolutely blew my mind i didnt know what silence was (still dont)

      Think about first time you ever heard. What kind of things happened then? How is the sound like? As a bell? Train signal or a animal or a machine sound? A voice?
      Did you get a horrible message from your family when you get a call? Try to remember the trauma exactly and repeat it in your mind as you se a film. When you you remember it and calm your mind that moment it’s pasted and you don’t need to be horrified then the tinnitus will quit. It never gets wrong.

    Good to see this problem is being intricately addressed. About 10 years ago there was an article in "The New Yorker" that said tinnitus was the most prevalent injury in soldiers coming back from the mid-East wars.

    I've had it since 1994. Worsened just over a month ago. I just live with it. Actually getting used to it. Many frustrating days of course though. ;)

      We have to ignore it and live with problem.
      My situation:-I had a sinus infection from AIR CONDITIONING in the ceiling, particles were breathed in at work. I have whooshing hissing beeping tones 4 over a year now. Blew nose hard and ears popped. Went deaf in right ear. Infection cleared with antibiotics(amoxicillin)from hospital. Right ear Hearing returns. Now this EMOTIONAL STRESS OF NOISE. Tried MASKING NOISES by headphones. 4 hours a day. Bit of a relief. Annoyingly Noise comes back.
      DR gave “brain blocker signal medication” didnt work, instead caused MEMORY PROBLEMS.
      THREW POISON TABLETS in THE BIN thats were they belong.
      Phoned hospital and was told Still on ENT waiting list. Appt due THIS Y E A R ?
      Lack of DR’s & regular strikes over pay. It seems the “almighty creator” has overlooked earnoise.

    I occasionally get a 13kHz sine wave in my right ear, it comes on suddenly, lasts about 15min and then goes. I get it a few times a week. It's happened for as long as I can remember, as early as 5 or 6, but nobody would believe me (or cared) when I told them as a child.

    "Natural" tinnitus can also be harnessed for spiritual purposes. There's a whole tradition called nada yoga that focuses on the "sound of silence", the inner sound that we can hear when we're in complete silence, which allows practitioners to achieve profound meditative states and mystical experiences.

      This makes sense because many areas of the brain, other than those associated with auditory phenomena, are associated with tinnitus. (Relevant article: www.bbc.com/news/science-environment-32414876 )
      The link between mystical experiences and the brain-wide network that's at play during tinnitus can be a very fruitful area of research.

    How to cure tinnitus?
    How is your tinnitus frequens sound like? Like a train signalling? Phone calls sound? A animal? A machinery sound? Did you get a horrible message from your family when you get a call? Try to remember when you get the tinnitus ? How was your mental health? When was you terrified last time? Feel angry about your life? Then remember the trauma you had exactly and repeat it in your mind as you se a film. When you remember your trauma and understanding it and calm your mind that the moment it’s already pasted and you don’t need to be horrified anymore then the tinnitus will quit. It never gets wrong.

    Does anyone who suffers have difficulty concentrating and difficulty focusing for any length of time? My self I can habituate to it pretty well however I notice I can't sit still for long, say to read or focus on something. My thoughts are a jumbled incoherent mess.I often wonder am I on the spectrum somewhere for ADHD or is tinnitus impacting my ability to concentrate? Maybe subconsciously my mind moves around so much as not to focus on the persistent ringing. It's quite perplexing.

    I have a question, totally not relevant to the video but i hope someone who has tinnitus as well could answer this for me. Can theme parks make tinnitus get worse? I'm going to one this weekend and I'm scared that the sound of the roller-coasters might be too loud or will it not affect me that much? Anyways hope someone can answer 😅

      If you're concerned about the sound, bring earplugs. But even if you decide to take a chance, it's unlikely that the sound will have a long-lasting impact. You might get tinnitus for a day or two, but it will probably go away quickly. In other words, don't worry about it too much; you can either prevent it or deal with it.

      Mindfullness is my way to deal with tinnitus. Try not to focus on the sound, because thats a way to train your brain to be more aware of the sound. Awareness of the sounds can cause more annoyedness and tiredness. When your less aware of the sound, it also will be less annoying. Its a process to be more mindfull with tinnitus. These are my own findings to deal with tinnitus.

      @Sems Sonmez just use onions,wipe around your ringing ear,and a little of its oil into the ear using with your index finger.I ever had this sicness for over a month and i used this method,it worked.Thanks God.The ringing was gone completely.

    No se desesperen, es normal caer en pánico, creo que a todos los que hemos pasado por esto nos pasó, pero con el tiempo uno se termina acostumbrando, recomiendo tomar Cloruro de Magnesio, ayuda demasiado al sistema nervioso y al ayudar al sistema nervioso aprendes a vivir con él. ¡ÁNIMO!
