Tony B makes a fat joke of Tony, (like when they were kids) the boss, everyone is uncomfortable, then Tony laughs and sort of grabs him in a passive aggressive manner, (trying to save face despite being dealt a blow to his massive ego) while everyone nervously chuckles.
I never knew who Van Gleason the III was so I searched it up on YT and found the song. This became even funnier because in the song he says: Mboy are you flat. Tony B changed to to Mboy are you fat. He was a funny guy Tony B 😄🤣

When Tony came back from the hospital after he got shot, he still had the title of "Boss" yet something was off, he was perceived as being weak by his organization and he felt that they didn't give him the respect that he truly deserve (Nobody was disrespecting him but the element of fear was missing therefore he did what he had to do to show that this junkyard dog can still bite)
Probably because it works on so many levels, and it tells you all you need to know about the characters and scene. If you'd never seen the show before and you start watching here, first you'll laugh, then you'll say, "okay, I'm all caught up." 😁

Tony B knows this, which is why he's prodding Tony. Tony B was on a suicide mission from the second he got out.
The joke is that he has been in prison for 15 years (1989 because season 5 takes place in 2004), and Miami Vice Sonny Crocket-style suits were in style in the '80s. That's all Blundetto had to wear.
Everyone just went straight to neutral silent mode waiting for Tony's reaction. Even Silvio's observing from the background. Just so they could go along with his response to the insult.
In an alternate universe, Tony didn't have a single slice of gabagool before this scene, so when Blundetto said "m'boy are you fat" Tony fucking punched him to death in front of everyone
