Play endless and kill the boss on wave 5 then reset and do it again. Bosses give the most XP in a fast amount of time. It's tedious but it works and saves you a bunch of time.

I saved up for a pc just to play this game still. I maxed prestige on both consoles and they are so dead it sucks. This game was made for pc players.

Sucks how the team health add-on is a server plugin only and not individual mod. Would be so great if this game had the same modularity as L4D2.

No, I don't find it necessary. I've been playing so long without one it just feels natural without it.
