You will need a certified copy which you should be able to obtain either online or through your local government office. Google your state name and birth certificate for specific instructions
Normally you would include a signed and notarized DS-3053 if the other parent cannot be there. If the mom is only on the Birth certificate then only the mom has to be there.

My wife updated her last name after our wedding. Her maiden name is on our daughter's birth certificate. Which name should we use for the passport application? Her driver's license has her married name. Will this cause any issues with verification? Thanks you
You should not have any issues if you bring your marriage certificate to the in-person appointment. Here is a quote from the travel.state.gov website: "You and your child may have different last names, as long as the document showing your relationship to your child lists your full name. If your name is different than the one on the document showing your relationship to your child, submit proof of your legal name change."

Thanks for this breakdown! It's crazy that we still need to send checks to a government agency for this. There are more secure ways to take our money. Efficiency is definitely not the strong suit of US gov agencies.

The physical items are to show the person at the in-person appointment and the photocopies are sent with the application

It might take up to 8 weeks for the state department to send back the proof of citizenship documents.

Usually, women apply for their's baby's SSCard when they are in a maternity hospital.
