The last one can be fixed making it that you die if you keep the weapon equiped for a set amount of time. And not keeping them in the hot bar.sin e hot bar would be a tool belt and the inventory a backpack

Is just something that you really, REALLY don't want to have a prejudice against, because there is no worst feeling than be in the shoes of the type of person you looked down on.

Just continuous pain and suffering but you can’t escape it because even when you die you’re still old and your dementia is still getting worse. Eventually your consciousness will be nothing but mush yet it will continue on regardless

The dementia mechanic would've been way better if it teleported you to the same biome/structure you were in when you had the episode, but like 1000 blocks away. So, on the surface it looks the same, but everything is a little bit different.

These days you don't even need to be old to have dementia. That's been my struggle since I was a kid. It turns out that apparently there's a vitamin d deficiency that mimics the symptoms of dementia.

Make it so dementia has a decent chance to give vicodin, at least 1-2. This will prevent being softlocked by it as easily. Could also add a villager trade to the cleric. Could make the nether remove the slowness debuff since, like Florida, the heat helps. Lmao.

-insanely weak (5 hearts of health, 1/2 of normal attack damage, & ofc the infamous heart attack mechanic)

-What is a sleep schedule? (The timeframe of which you can sleep will be random; sometimes it will be normal, other times not so much and you can only sleep when all daily academic responsibilities has been satisfied)
-academic responsibilities (must do side tasks as coursework to maintain good grades or face getting disowned by your parents aka insta death)
-Low energy (If your saturation is empty, you cant run and will get mining fatigue and you can only fill your saturation with energy drinks however this will cause temporary nausea)
Bonus: tech reliant (you cannot craft from memory, you can only use minecraft’s craftingtable built in crafting handbook to make items.)
Challange idea: prime inventory challange. You get 2 dirt when you spawn for the 1st time. If the number of items in your inventory is not a prime number at any point, you will lose your world permanently. (NOTE: items in your 4 crafting slots DOESN'T count, but items in your offhand, your armor and items in any other slot DOES count)

By the way, if you are going to do this, I think using backups to undo your mistakes is fair game considering how hard the challange is.
