lol 3:28 the melody introduced in the piano is one of the themes in his f# maj op 27 piano sonata (also in the violin at 11:11 , if I had any doubt this melody wasn't from the sonata this longer snippet clears that :) )
I find his music very fascinating not the least after reading the commentars by Dimitri Alexeev! The composer has got a quarter of the blood in his vessels Swedish.
I feel the work is a bit too laid back, and the beginning sets the tone for the entire quintet. The work never superceded the lethargic beginning, and ultimately it falls flat in spite of Medtner's best attempts to the contrary

He worked on it for many years. Frankly, I adore this piece. Sorry you find it unimpressive. But we all travel different roads.
