    That story of how Barbara asked to compete for the Shark Tank role shows the tenacity that made her successful in business. But what I enjoyed most is seeing Mike respectfully disagree with her multiple times and then explain why in a rational way - she has the strong personality and confidence of being a Shark, but Mike didn’t softpedal or try to avoid disagreement. He is demonstrating the art of having a real conversation, not a celebrity interview.

    I love how Dr. MIKE absolutely will NOT support something he doesn't believe in.
    I might not agree with everything he believes in, nor do I disbelieve in everything he doesn't think works.
    But his absolute integrity to stand for what he believes in, makes him worth listening to and considering because he does what he truly believes in.

    That said, this dude's pretty much guaranteed to have longevity on here if he wants to. Most channels have limited appeal, but people can always stand to learn more about health..and Mike does it in a way that's honest and down to earth

    I found it interesting how this episode showed an example of how much people in traditional media undermine the monetary value of online media. She understood the reach of online media but she failed to initially see how much the top percent of creators can make on the platform. Matpat has done some episodes talking about this kind of stuff. Overall, it was really interesting hearing Barbra’s unfiltered opinions on things

      Is he making 8 figs a year from RUclips, or from the collective body of work (the boxing events, the appearances on other show, the live show, etc). I can't imagine he's making over 5 mil straight from content creation on YT/TikTok/etc (but what do I know), even including sponsors, given what I know a sponsor pays for a channel with 1M subs.
      I also don't know how far down the rabbit hole of performance metrics (view, engagement, etc) YT advertisers go. Because in that case, I may be vastly underestimating what he's getting for those ad reads.

      I would say a personality, and looks like Mike would kill it on any platform.
      Question is whether he'd make more or less.
      Yet like Mike says. He has more control, and I think that is worth something.
      I do wonder how much it costs him to work at a hospital, and why he chooses to do that. Is that a legal thing? Surely he could open his own practice.
      Anyway it is a shame the medical system can't be broken, and rebuilt. Keep the old one running while a new system is built or something.

      @@fairygirl626 No way to know for certain. But I know for a fact, high paying advertisers are more willing to pay more for ads on a health focused channel that is advertising safe, compared to a gaming channel. Same applies to in video sponsors as well.

    I like Barbara because when she makes a deal, they usually close, unlike Kevin, especially. I hate it when the person has multiple bids, picks one, and then the shark bails on the deal when the other shark could have helped them. Barbara might not make a lot of deals on the show, but when she does, they actually close, so she probably closes as many deals as the other sharks. I love how honest she is, from almost losing $400,000 to her multiple face-lifts lol. That's a breath of fresh air in the celebrity world. We never get that from actresses.

    😂damn Barbara 47?! If I had to guess why she thought Mike was older I would assume it’s because of the way he carries himself. Mike comes off as very secure, confident, and mature so one would naturally assume he can’t be in his early 30s and be that put together. Loving these podcasts!!!

    You know, I really love that Dr. Mike knows who he is, what he knows and where his boundaries are. He's really good at remaining in control of his energy and respectfully controlling the conversation. This results in a lot of respect in his opinion and building trust. He's very natural and not arrogant. He's a good person doing this work for the right reasons and it shows! I would 100% feel seen when speaking with him.

    I just want to say thank you for mentioning how the health care system is broken, including in Canada. I'm from Alberta, and I've been dealing with horrible abdominal pain for the better part of a decade with no answers, and ER visits just result in being given drugs and sent home. Doctors visits take months, or even years to see specialists, and some days I feel like I can't live like this anymore. I can't work, I'm on benefits that only give me a few hundred dollars a month, and I can't get help. It's truly awful.

    When Dr Mike was talking about how crazy ambulance pricing was, I can totally relate to that. My then 15 year old daughter spent 12 days at Children's getting medically stabilized while she waited for admission to an Eating Disorder clinic. She had to be taken via medical flight halfway across the country and it cost $400k... I had no idea it would be that much! But, luckily everything worked out, our insurance paid $250k and left our responsibility $69k, and the flight company stayed true to their promise to not balance bill, so we did not have to pay the $69k. It was expensive as hell, but it was the only way to get my daughter the treatment she was literally dying to receive.

      Forgot the state but a private company got sued for starting their own. It was cheaper and faster making it more effective. Once again though it was about money vs saving lives.

    I 100% agree on ER's. I had pain in my left side that felt like broken rib or possibly a blood clot. 19 hours in the ER before they told me it was neither and they did not know what it was. Gave me an ibprofen patch and sent me on my way. :( But on the plus side they noticed I had enlarged lymphnodes and told me to have my doctor check it out. Turns out I have MCL (Lymphmoma). Had I not gone to the ER would have never known till it was too late.

    If you call an ambulance you can ask them to take you to a hospital you prefer if it’s close. If it’s an emergency and a life threatening situation they will take you to the closest hospital. My dads blood sugar dropped one morning years ago and my mom called the ambulance because she didn’t know what happened. She asked them to take him to the hospital his doctor is at and they told her ok because by then they knew what was wrong and he was alert. Such a great video Mike!💜

      I think it depends. EMTs and fire departments around here (N. Illinois/S. Wisconsin) have contracts(agreements?) to take patients to certain hospitals. For example, my company had an employee with some special needs, and in case of emergency needed to see their doctor 45 minutes away. The fire department said they couldn't do that and would only bring them to a local hospital they had a contract with, and from there the ED doctor will determine if they should be transported to this other hospital.

      I'm a emt if life threatening you need to go to a level 1 hospital but if the hospital you want is not that much further or your physician is in a different hospital you can go to that one

    I really, truly respect Dr. Mike for his consistency when it comes to misinformation. It makes me trust him a lot that he turns down insane amounts of money, just because of the possibility of having to to spread misinformation. He truly is here to educate and not to become famous and rich.

    I disagree with Dr. Mike that the primary reason for elevated healthcare cost is administrators. The primary reason for the inflated prices is the insurance industry.

      @@AG-iu9lv outside of the management and executive levels most administrative positions don't receive high wages. Even average annual salary for clinic managers is less than that of nurses. With value based care on the rise there is a need for people like care coordinators and preventative health coordinators. Even these people rarely make over $20 and hour. Insurance companies are the ones that create complexity to the system in how they negotiate rates (they are price makers). The healthcare systems are price takers. This is what inflates the price. We are basically paying for a middle man (being the insurance industry). Ideally we would cut out the middle man but do we trust the government to run that job properly? Idk that's each person's individual call.

      @@sabel146 this is all true, it's very much multifactorial. It could be argued that the primary catalyst for many of these factors is the inherently ingrained in the culture created through the insurance "fee for service" model which encouraged treatment over prevention.

      Its a combination of many factors. However, Insurance industry does have a big role in this. The less they pay the doctors, the more they get to keep. At the end of each year, CEOs get millon dollar bonus because they "saved" the insurance lots of money. Well, they saved lots of money by collecting all the premiums and money from the goverment and then not paying the providers at all because they dont think its medically necesary or paying but taking the money back. Each year the payments goes down. If a hernia surgery pays 500 dollars to the sugeon in 2021 by now it has gone down to 300 and each year keeps going down.

    OMG Barbara had a question that I can most definitely answer *insert fangirl energy*. If you call an ambulance, do they have to take you to the nearest hospital? Speaking as a seasoned paramedic, the answer is it depends on the situation and how critical the patient is. The decision has nothing to do with insurance and everything to do with the condition of the patient. If the patient is stable/not critical they may request to be transported to a hospital of their choice (provided it is within reasonable distance). If the patient is in a critical condition they will be transported to the nearest APPROPRIATE hospital; for example, if the patient is presenting with stroke-like symptoms they'll be taken to the nearest stroke center even if that means bypassing other hospitals along the way. The idea is that it's better for us to take just a few extra minutes to get these patients to a facility that offers the specialized services they need than to delay care by taking them to the nearest hospital which will then turn around and send the patient to a specialized center which will take much longer. If you're in a rural area we may even call in a helicopter. Another factor we have to consider is the status of the hospital; sometimes a hospital, or more specifically the ED, will go on diversion for many different reasons, and then we have to take the patient to the next best/appropriate facility.
    That answer was way longer than I expected and I hope it made sense 😁

      This is exactly how it works in our publicized system up here in Alberta, Canada. I'm still new to the industry, just an EMR (8 month part time education, roughly equivalent to an EMT-B in most states) and waiting for PCP school. But after assisting a few flight medics with medevacs from the US, it's cool to know how similar our systems and scopes are (albeit with different scope names)..

    Toward the end of life, my mom had several trips to the ER and we figured out that if we took her in, we had to wait many many hours but if the ambulance took her in, she got right in.

    My helicopter ride to the hospital was $32,000.00. Insurance generally does not cover it in NC, because the service providers are private and almost always out of network. My insurance only covered it because the insurance company deemed "She was going to die before getting to the hospital w/o this specific intervention". It magically becomes in-network if you'll die w/o it.

    I worked in a 39-bed ED, 550 bed hospital in a larger city for two years and had a chance to talk with the medical director of Emergency Medicine on a few occasions. He said larger, flagship hospitals are designed to operate at 90-95% capacity at baseline. Meaning during summer months, when fewer people are sick, the hospital would be nearly full of patients. During flu season, admissions increase by as much as 30%, and ED visits by nearly 50%. (This was all before Covid.)
    When this happens, the ED gets backed up by admitted patients who are just waiting for a room upstairs. We had patients spending 70+ hours waiting in an ED room, when the average time for admitted patients to wait is supposed to be 4 hours. If we can't get patients upstairs, we don't have any rooms available to start seeing new patients.
    Covid made everything so much worse, for both people with Covid as well as patients with chronic medical conditions. Hospitals became swamped, even during "normal" seasons, and you had patients who needed to be seen emergently waiting hours, slowly getting worse.

    This was wonderful. The contrasts with financials, availability, understandings and the difference it is between a Dr and a patient. I'm not trying to put to much emphasis on this subject... But the drastic differences that she has with medical, far out ways anything that others may have. love Dr Mike's perspective on this.

    I love the age guess!! LOL I guessed 34/35 based on what I know with length of college/med school/ residency. Then add a few years to get established in the field and RUclips! :)
    But I adore Barbara! She is so uplifting and kind in the tank! I have watched Shark Tank religiously and have bought several products that I have seen on the show, and LOVE most of them! But listening to the way Dr. Mike interacts with her is so awesome! Open and honest! Laughter is the best medicine, love the meme reviews when I feel under the weather! Because your laugh is contagious, but in a good way! LOL
    A good pharmacist will see the denial and the why, and then call the Dr. and have them re-send the script the way it WILL be covered!

    10:27 i’m canadian, Dr Mike is 100% right. and our health leaders are much more upfront about the RSV/covid epidemic. They told the press we will not be able to manage that amount of hospitalizations.

    Such a good conversation! Every time I hear doctor Mike talking about women’s health I’m glad that I live in a country with universal healthcare, I had a painful uti and had to get antibiotics when I was only working a part time job, can’t imagine being in a situation where I can’t get that if I can’t afford it, healthcare should be a human right!

    I had a rude awakening about PPO insurance the last time I had to get sent to a hospital for an injury. I obviously wasn’t fully coherent, but apparently the EMTs asked if the hospital 2 miles away was fine. I said yes, but after the entire experience how was I supposed to know the hospital was in Network, while the doctor who stitched me up wasn’t? And someone else that I can’t remember wasn’t in network either, so I paid way more than I had thought I would. These things are out of a patient’s control, so what can we do to fix this? Especially in an emergency we can’t check who is in or out of network. That was also the most expensive two mile trip for me.

      That happened to me too! Went to the ER life or death and then I get a bill that one of the doctors was out of Network! Like whyyyyy?

      They passed a bill in January 2022 about no surprise bills in the hospital.

    Really interesting to hear about the US healthcare system and how broken it is. Talking about how it is so badly stressed and that one surge of cases can easily cripple the entire system and then hearing how it's so expensive that patients even when sick refuse to go in fear of going into massive debt... It doesn't really make sense to me.
    In Australia, we don't have the most perfect system but annually we pay a small fee as part of our tax, $200 maybe towards healthcare. Then that's it, nothing more to pay for 99% of medical procedures. The only real reason to get health insurance in Australia is if you would like to stay at a private hospital over a public, you get a nicer room, bigger bed, quieter environment. It's not like you get a higher quality of physicians either, most of them work in the public system as well as having priveledges at private hospitals. Another advantage for our private system is you can pretty much have an operation whenever you want, whereas the public system you have to wait for a doctor or surgeon to be available for a procedure, and it's usually down to the severity of your case.
    Ambulances are about $1200 for childeren and adults up to around 60 and then it becomes free, most states subsidise ambulances, private health insurance also covers ambulance as pretty much standard.
    What I'm saying is, why is it so expensive in the US? and why are people infinitely more money for services that are inadequate? Surely if it costs multiple times what other systems cost, it should a good deal better than them?

    Wow, really loved that interview, absolutely wonderful! Listening to her talk about eye doctors reminded me of how my (since retired) eye doctor caught my seasonal tree allergy during an eye exam once that I didn’t even realize I’d developed at the time and chalked up the symptoms to just being a cold even though I experienced it with regularity every year and the tell tale sign was the fact that there was never any phlegm involved and my eyes were way itchier than normal which should’ve totally clued me in but it never even occurred to me because I was wearing contacts at the time and just thought those were the cause instead. It’s amazing how interrelated so many things are when it comes to your health!

    I agree with Mike in his decision not to syndicate over the language/control issue. Credibility is much more important in the medical field vs products manufacturing. You lose your reputation in medicine, there's no public redemption like there is in other fields.
