  • Uterine Artery Doppler is a specific ultrasound which measures the velocity of blood flow in the uterine artery, which is the major blood supply to the uterus arising from the anterior division of internal iliac artery.
    The indications of Uterine artery Doppler include previous history of preeclampsia, previous child with intrauterine growth retardation, unexplained high maternal serum Alpha Fetoprotein level, high maternal HCG level.
    During systole, the blood flow is very fast and this depends upon the contractile ability of a woman’s heart. During Diastole, the flow pattern depends upon the terminal vessels vascular resistance mainly the spiral arteries.

    Hi mam, I am currently 32 weeks pregnant. At 29 weeks 6 days scan
    Right uterine pi = 1.21, ri = 0.65, s/d = 2.84
    PSV 106.4 cms/sec
    EDV 37.5 cms/sec
    Left uterine pi = 1.94, ri = 0.78, s/d = 4.51,
    PSV 68.1 cms/ sec
    EDV 15.1 cms/ sec
    Mean PI = 1.575
    MCA pi = 1.81, ri = 0.83 and s/d = 5.85, PSV = 67.9 cms/sec, EDV = 11.6 cms/sec (19 centiles)
    Umbilical artery pi = 0.94, ri = 0.62 and
    s/d = 2.66 (37 centiles)
    PSV = 40.9 cms/sec, EDV = 15.4 cms/sec
    CPR = 1.926
    EFW is 1223 +- 122.3 gms.
    Doc said fetal has anemic.
    Apart from this, there is any problem with Mean IP.
    After that, scan 31 weeks 2 days
    Umbilical artery PI =0.90
    MCA PI = 2.36
    MCA PSV = 66.14 cm/sec and doc said all parameters are below the 10th centiles for gestation s/o symmetrical IUGR
    EFW is 1399 gms
    Pls reply me mam. Where is problem? and how much risk?

    Hi please may i ask your advice? My pi seems to be high. It was 1.38 at 34 weeks, then 1.4 at 34+6. Next day at 35 it was 1.5. I was admitted and next day it came down to 1.2 and fetal movement returned to normal. I'm having doppler, fluid and heart trace 2x a week with a view to induce at 37 weeks. If the pi continues to rise, is this ok as long as no notches or reversed end diastolic flow happens? Or will they bring baby sooner if i go tomorrow or Monday and it's still high? Many thanks

      The main role of Uterine artery is at 21- 28 weeks of gestation when is helps in assessing and predicting certain fetal complications mentioned in this vide.
      Now as you are at 34 plus weeks of gestation, so in your case fisrt of all serial growth scans will tell us whether your baby developed IUGR or not. Moreover, your baseline investigations and BP record along with protein in urine test will help in detecting pre-eclampsia.
      In order to plan delivery, the UMBLICAL Artery Doppler indices and waveforms (normal, absent or reversed) will tell when the baby should be delivered.

    Is there anything wrong?

    What is the mean or percentile and mean PI in Doppler study
    At 20 weeks scan mean pi = 1.25 (66 percentile) is it normal?

      Although limited studies are available to check the effect of uterine fibroid on pulsatility index but few studies show that pulsatility index increase in such cases due to impedance in blood flow in uterine artery.

      The arteries of parous women assume a much more convoluted and tortuous course.
      Parity means number of pregnancies.
      With each pregnancy the tortuosity of uterine artery increase.
      So no need to get worried about it.
      Tell me S/D ratio and Pulsatility index. Those are important to know.

      I hope it will be normal but next time if you visit your obstetrician or radiologist ask about that values as well.
      For Protein C deficiency , Injection Clexane throughout pregnancy and stopping before delivery is found to be quite benificial and helps prevent different complication.

      The normal pulsatility index at this gestational age is about 1.07 +- 0.38.
      High pusatility index and persistent of notch at this satge index increased risk of developing certain conditions like pre eclampsia, fetal growth restriction, stillbirth.
      So in order to avoid complications in pregnancy, you need regular antenatal visits with regular record of BP along with urine test for proteinuria. Moreover, your essential tests are needed to be done and regular growth scans of the baby will give us an idea about biometry and liquor volume. In short as your pregnancy is at high risk so as compared to other patients, we need vigilant monitoring.

      Infact due to inadequate placentation in the first trimester the resistance to blood flow is high in placenta .
      Thats why ideally the doppler is done at 20-24 weeks bcoz by that time the trophoblastic invasion is almost complete.
      Somes tudies show that PI of more than 2.35 at 11 -14 weeks is predictive of FGR or pre-eclampsia.
      But I would recommend you to repeat your Uterine Artery Doppler at 222 weeks and tell me about PI at that time.

      Yes, dear Heparin and Aspirin have been found to have beneficial effects in ensuring the placental circulation.
      Although limited studies are available about Yoga but if done carefully it can be useful as well.

    Hello madam..my NT scan report shows right uterine pi is 2.4 and left uterine pi is 2.4 but mean is not mentioned...is that too high..or any complications will come ... please help me with your answer please

      First tell me the gestational age in weeks as uterine artery PI varies with gestation.
      The PI of 2.4 is quite high at every gestation.
      The mean UtA-PI values are about 1.84 , 1.07 and 0.78 in the first, second and third trimesters of pregnancy, respectively.
      For management you need serial growth scan and assessment of risk factors causing increased PI in uterine artery doppler by an obstetrician who would take appropriate history and examine you and baby thoroughly and plan management accordingly.

      As placentation is not compele till 18th weeks so at 12 weeks the uterine artery doppler is not reliable.
      Repeat it at 18th weeks , then it will be managed acoordingly.
      Hopefully it will be normal on 18th weeks.

    I’m currently 12 weeks . Right uterine pi is 3.27 , left is 1.71 and the mean pi is 2.5 . What are the complications and how can I rectify it ?

      As the placentation is not complete before 18 weeks , so I would advice you to repaet this doppler at 18- 20 weeks and then we will plan accordingly.
      The complication which is expected in case of derrnged doppler is fetal growth resriction and pre-eclampsia but again I would say repeat it at 18- 20 weeks . Most of the people get normal doppler latter on when the placenta completes its trophoblastic invasion process.

      Good question.
      Actually, the uterine artery doppler ultrasound measures the mother uterine artery blood flow velocity (not the fetus) And by assessing the maternal uterine artery blood flow pattern we identify the risk of developing growth restriction and other abnormalities in the fetus.
      So it doesn't matter whether the fetus is male or female.

      If radiologist has specifically mentioned that he/she is commenting on fetal uterine arteries, then that is just because he/she was intending to check blood flow of each and every individual blood vessel flow pattern of fetus.
      But in order to assess and predict the risk of developing fetal complications, the maternal uterine arteries parameters are analyzed. Your scan must have comment on maternal uterine arteries as well. If not, then you need to repeat your doppler scan.

    Hello Madam, my mean uterine PI is very low at 18 weeks , it is 0.68. There is no interpretation given for very low PI. Will it mean any complication? Kindly help

      The mean UtA-PI values should be below 1.84 , 1.07 and 0.78 in the first, second and third trimesters of pregnancy, respectively.
      Low PI means low resistance to blood flow.
      So 0.68 i satisfactory.
