  • Doppler velocimetry of the Middle Cerebral Artery (MCA) is a method to assess impedance / resistance to flow in the fetal brain circulation. Fetal middle cerebral arterial (MCA) Doppler assessment is an important part of assessing Fetal cardiovascular distress, Fetal anemia, Fetal hypoxia. In the appropriate situation it is a very useful adjunct to umbilical artery Doppler assessment. It is also used in the additional work up of intra-uterine growth restriction (IUGR), twin to twin transfusion syndrome (TTTS), twin anemia polycythemia sequence (TAPS)
    The middle cerebral artery is examined close to its origin from the internal carotid artery. By using color Doppler flow imaging the middle cerebral artery can be seen as a major lateral branch of Circle of Willis, running anterolateral at the borderline between anterior and middle cerebral fossae. The MCA Doppler is done to assess resistance in fetal brain vasculature.

    Mca psv is 67cm/sec with boderline higher than 1.5 times of median.... adv follow up what is this meaning mam please you can explain this....

    Ma’am only dr highlight MCa PI 1.05 ? Normal delivery possible? I’m 36 weeks 3days . And dr impression is abnormal fetal mca Doppler indices . Plz reply ma’am TIA

      Normal PI of middle Cerebral Artery is in the range of 0.1-2.45.
      But in Middle Cerebral artery we check S/D ratio and compare it with Umbilical artery S/D ration .
      MCA S/D ratio should be higher than UA S/D ratio.
      Reduced S/D ratio indicate underlying fetal abnormality.
      So teel me about your Gestational age, The S/D ratio of both MCA doppler as well as UA doppler and also tell about PSV.

    Ma'am, S/D ratio of right MCA is 339.6. What does it mean?

      S/D ratio of MCA doppler is normally 0.48 at 20th weeks of gestion and it decreases over time reaching to 0.18 at 37 weeks of gestation.
      It is always compred with Umblical Artery doppler and should be lower than the S/D ratio of Umblical Artery Doppler.

      @@AAUN I am 23 weeks pregnant now and S/d ratio of umbilical artery is 2.5 but S/D ratio of MCA is 339.6. How risker is the pregnancy? What shall I do?

      The normal MCA S/D ratio should be higher than Umblical ratery S/D ratio.
      Lower S/D ratio on MCA is abnormal.
      In that sense your Doppler report is normal but the value of 339.6 is unually high as compared to the units given by other doppler scan machine , probabaly there might be difference of the Unit used in that machine.
      Anyway higher S/D than Umblical artery S/D is normal.

      Yes , dear all the ultrasounds done in obstetrics including color doppler are completely safe for baby.
