      I agree. I retook my Uworld Priority questions last night after watching this video and killed it without having to study my notes I created when I took my first attempt at their Priority questions (which i got a 46% on). After watching this last night I got a 94%. This was my weak spot, and now I feel confident retaking my RN boards next week.

    Mark Klimek’s strategy for prioritization and delegation were the main reason why I passed the Nclex. It was never explicitly taught in my nursing program.

    For the last question, I would have picked "client's interacting with a visitor" because the normal is that they are withdrawn, and visiting with someone is the opposite of that.

    8-24-24 I passed the board exam on my first try. Three days prior to my exam, I don’t know what led me to Dr. Sharon’s channel. I found her videos for prioritization randomly and I watched it three times in a row. I got it and I got it. So I went to the practice questions and my grades were dramatically escalated.     For the last question, I totally would have chosen “client interacting with a visitor” because interaction to me means being sociable and they are typically withdrawn. Sleep disturbances is vague.

    I took nclex last 9/28/22. I watched 2 videos of u 15 min each coz I just found it a day before my nclex. I passed. I didn’t get questions on topics/systems I mastered. I got a lot of Peds/OB which I never studied but I passed due to using strategies including this. Thanks Dr Sharon

    I watched this last night; I went back into Uworld, retook all of the Prioritization questions,and absolutely killed it🙌🏾. This was without me studying the Uworld notes that I created during my first attempt at Prioritization (which I got a 46% on). My new score was a 90% without studying. This strategy works. I could never figure out how to answer priority questions, and you made it so clear and simple. Thank you so much.

      Same happened to me. I also found very helpful the atrial arrhythmias and ventricular arrhythmias videos, all these are a total game changers. If you find any other video that substantially impacted your grades, make a comment here. I'm not good in mental health and pediatrics.

    Rationale of the answer on the Last Question:
    For your guidance
    The most important behavior to report to the next shift is that the client was able to sleep from 2300 to 0600. This indicates that improvement in the symptoms of depression is occurring as a result of pharmacologic therapy. The nurse would expect to observe improvement in sleep, appetite, and psychomotor behavior first before improvement in cognitive symptoms. The client's flat affect is still a symptom of depression. The fact that the client had a visitor is not as important as changes in the client's behavior. Spending the evening in her room is a continuation of the client's withdrawn behavior and is important to report but not as important as the improvement in sleep.

    I did my NCLEX and passed at 85 questions. I tell you if you forget everything remember her voice on prioritization. I got more than 50% questions on priority. I just have her and Mark to thank.

    This was so helpful. But the last question threw me off. I honestly picked 2 because it was a change to me that she went from being withdrawn to now interacting but then I can see how sleeping 7 hours straight compared to disturbances is reportable as well. Wow I really pray I pass. This is an amazing review.

    I guess I have one questions based on the Depression question.. "I have started losing my hair" - could be happening due to the mal-nutrition and loss of 10 lbs. I would have chosen answer 2 "I have lost 10 lbs since I started feeling so bad." Although this is expected it creates a problem for malnourishment and many malnourished patients lose hair. This finding I see as more concerning and if worked on can inhibit hair loss from occuring.

    I so respect you and Mark Klimek you all are BRILIANT. However on the question about the schizophrenic patient is it not the priority based on a safety precaution to ALWAYS question whether the voices are instructing the client to hurt himself or others. I first leaned toward the "crackles" answers as this was an unexpected symptom. But understanding that safety is the priority with Nclex I went back to the "voices" choice because safety and immediate harm take precedence over mild respiratory concerns. Or should I have disregarded the "voice" mandate" because the question didn't mention depression or antisocial tendencies? Help!!! I'm confused???

      In the video she mentions not adding outside context to questions. Nothing in the answer choice indicated that there was a safety issue with hearing voices at that time. If the patient hearing voices becomes the priority, you have to start asking more questions like what are the voices saying, are they telling the patient to harm him/herself, etc. Once you do that you're no longer in the question. You're in a scenario you've created yourself, and your answer is probably going to be incorrect. This is how anxiety wears you out during a test. I haven't taken the NCLEX yet but I've taken similar exams (NREMT, ASWB LMSW) where similar logic applies.
      EDIT: I scrolled down a bit right after I posted and someone else already said this.

      We have to consider psychosocial and physiological needs with this question. And the physiological need is always prioritized. All the other choices are based on psychosocial needs.

    I felt like I was having a stroke watching this. There are so many ways that can depression can lead to hair loss. In the same exact question you mention that patients can experience extreme weight loss, a common cause of hair loss.
    Also, I am so tired of NCLEX prep saying to not assume and not read too much into a question, to then be met with a question with a rationale that tells you to assume things and dig deeper. There's so much conflicting advice.

      Anything can lead to hair loss…. But you have to think GENERALLY when taking NCLEX.

    And also for the patient with major depression concern about losing her hair because I never learn of hair loss being a symptom with major depression couldn't the questioner have just as easily said as is common in Kaplan that because major dep4ression and hair loss are totally unrelated to eliminate that answer immediately as this was only a "Distractor"? Where does the madness end?

      Hey Andre, I understand what you mean but the issue of hearing voices explains hallucinations which is not life threatening. However, If the answer had said "hearing voices to harm other people or himself", that would have been a safety issue/concern. I remember Dr. Sharon always saying, "do not read into the question or add to it but read understand and know what the question is asking you specifically". I hope this helps, Thanks!

    What's frustrating is that depression can cause telogen effluvium, but we'd still test to make sure that it wasn't due to hypothyroidism or some other cause, since there are other causes for hair loss. Still, I think we'd do that with weight loss too, so it's a bit of an annoying question imo.

      Lol, the NCLEX doesn’t test on those kind of exceptions. It tests on general principles
