His hand got blown off and his spine was severed when he got impaled by that metal rod. I imagine the clockwork heart probably had something to do with his blood drying up.

That must be some device that works like an artificial heart, connected to several other mechanisms (basically Kroenem's body works like a clock). In this case, he is giving his reflexes a boost in order to cut the statue in half.
Long story short: Kroenen has always been a schizophrenic surgeon and liked to modify himself in search of perfection. After his "death" in 1944, he came back to life thanks to black magic and he replaced parts of his body with mechanical ones to keep living and serving rasputin. In particular, he replaced his broken spine with metal, his lost hand with a mechanical one and his heart with a clockwork of his own design (have to assume that some of his organs have to be mechanical as well. When he turns the wheel, he becomes faster and with better reflexes. He can also turn it backwards to simulate his death.
Funny story; The leather and spikes that became all the rage with 80's metalheads was based, unknowingly from the fact that Halford was gay and dressed with that look already. Unknowing metalheads picked it up and it became all the rage. I'll bet that they felt silly when they found out. No one knew that he was gay until the 90's.
I wish I had his combative capabilities, although he is hideous behind the mask he is a true professional security guard. He managed to extend his life span and as a intelligent individual he realized how to make better use of himself and protected it, gotta give my man's credit for that although Hellboy kicked'em to the curve* 🙈

The woman gives the JAR to Rasputin in a secret location. Rasputin ressurects that thing......... But the woman went frik that . And went hammer time. Adn Rasputin was forced to appear
