Non-HA Biostimulators For The Face And Body: UK 2023 Edition

  • Biostimulators are products that get to work deep down in the dermis layer of the skin and unlike dermal fillers which simply inject volume, biostimulators work by stimulating new tissue growth, helping the skin to naturally produce its own volume.
    There are a few different types of biostimulator compounds such as Sculptra and Lanluma which are made from poly-L-lactic acid. Radiesse and HarmonyCa which are made from Calcium Hydroxyapatite and Ellansé and Gouri which are made from polycaprolactone.
    All of these biostimulators have different 'personalities' and I use them for many different things. In today's video I'm going to be looking at these products solely in their biostimulatory benefits (as opposed to their volumising properties!)

    I am a Korean dermatologist. PDLLAs ( Lenisna and Juvelook) have occupied almost the whole market share in Korea. Gouri is a K-product, but don't get KFDA yet.

      I wonder why Gouri and Ellanse arent good enough for Korea the skincare capital of the world And not approved yet?

      @@Benrhythm as a person who knows Korean it is still in the process of KFDA. America for example takes around It might take 10 to 15 years or more to complete all 3 phases of clinical trials before the licensing stage. But this time span varies a lot. There are many factors that affect how long it takes for a drug to be licensed.

      @@Tannertheinjector fascinating on the Dexvelo site it stated it hope for approval by 2022. I wonder if it didn't pass clinical trials. It got me wondering if popular Human placenta extract are KFDA approved? I looked and it doesn't seem to be.

    Wow Sarah, you look incredibe. I can't put my finger on what's different, although you do look slimmer, but being slimmer isn't necessarily the reason for you looking so good. How would you feel about doing a video explaining what you've had done and why? Thanks for the information on bio-stimulators. It's fascinating. What's your view on the efficacy of topical PLLA like SculPLLA serum?

    I love this content so much! I think biostimulators are the way aesthetics are going. I'm 53 and I've just started with radiesse and sculptra... I hope I'm right!! :)

      Love a bit of radiesse and sculptra. Interestingly most people here prefer a hyaluronic acid or polynucleotides

      @@DrSarahTonksI recently lost weight and looked to Gouri to regain some lost volume. I did take diphenhydramine before the procedure. My face is very swollen. I don’t know if taking a different type of antihistamine would have been beneficial and maybe consistently for a few days post injection? I want to finish the full treatment plan but want to take whatever steps necessary to eliminate the risk of swelling. I know you said you reacted as well, interested in seeing if you have stumbled upon any tricks.

    As a USA injector this was awesome Dr. Tonks. I have used Korean and UK products beyond the US FDA approved ones. This was a awesome video thank you. I never heard of Gouri and I know korean but I found it through my supplier and i am obsessed! I am 100% going to do this too myself

    Hi ! You’re the first person I have come across who mentions Ellanse. I was recommended this product over Sculptra on my face, she injected (with a cannula) in a fan shape over my side facial area , I’m very happy the results were instant. I was told it would last for 2 year approx .She injected regular fillers in my sunken side temple area, and she said that would last a long time as that area of the face was fairly static . I would like any further details re Ellanse, curious about why you don’t use it? Regards Julie

      I just never really saw the need for it, and I’ve got more experience with Radiesse and Sculptra which to me are predictable, so I didn’t really want to add something new to my portfolio when the risk/reward wasn’t quite there for me

    I use that same type of toothpaste . Thank you for sharing your knowledge and experience for people who choose to or feel they need to not to age naturally for personal reasons. I will be that lined weak chinned person in the corner with the sparkly blue eyes.😅

      There was one I used called Boka but at the moment I am using Ruken that I bought in Japan. Tbh I prefer the Boka

    I have done Gouri last month and heard of the risks of swelling and bruising. Thank goodness I didn’t encounter any and I attribute it to my skilled injector who is a Gouri trainer. Other doctors may charge cheaper and I’m tempted at times .. but I’d rather not have any bruises!

    What are your thoughts on animal collagen fillers? Specifically Atelocollagen?

      I have never tried it. I would tend to stay away from products like this due to potential for immune reaction and need for skin testing however it looks like there is at least one product for which this is not necessary. They also don’t seem to last long. Collagen went out of fashion around the same time I came into aesthetics which is 17 years ago now, so perhaps there have been advances. But when there are so many well known and safe products out there do we really need any more?

    Hi, thank you very much for the informative video.
    I have a question: I would like to tighten (not make volume) the skin of my lower face and would like to use Sculptra. So I can understand that I should ask for hyper diluted Sculptra. But my question is: How deep is it injected into the skin?
    I saw a video (I can't find it anymore), where the injector drew little squares all over the cheeks and put a little drop of Sculptra using a vertical pinprick in each of the squares and then massaged it all afterwards. Would that be the best way to do it?

      Yes you can do it like this or you can use a cannula, would be injector preference really. With needle you might be able to get into more areas

      But how deep would it have to be injected? Just in the upper layer of the skin or at the muscle level?
      I would also like to ask: Does Sculptra stimulate fat cells (make fat cells grow/multiply)? Or does it only stimulate collagen production? Do you know that? No one seems to be able to answer this question and it is important for me to know, because I have had fat transfer to the cheeks and I would not want that fat to grow.

    I’ve tried Ellanse M twice before 6 years back and I must say I’m now too chicken to take the risks anymore (being non-reversible).
    However I m doing Gouri now which is also irreversible? But not as scary I feel

      Well at least with Gouri you’re not going to end up able to see it as it’s not a “filler” like Ellanse is, even in worst case scenario

    I got a bruise from an PCL product which left a stain that has been hanging around for 4 months now. I am having co2 laser now in order to see if it will help to get rid of it

      Try PRP and or pico laser dine professionally. I’ve heard of a few others this has worked. Where is yours?

      @@bethgleicher2362 Thank you. I have tried picolaser on it which did nothing which is why I switched to co2 laser. I have heard of another person where PRP was succesful so that might very well be the next treatment to try. It is a browm stain under my eye. I have had it longer than 4 months .

      @@bethgleicher2362 i used the neatcell picolaser which is a homedevice. The co2 laser is one of the strongest lasers as far as I know I having this done professionally. It does take up to 3 months to see the final result of that one but I am not impressed. I am sure our bruises will fade away but they are persistent I will give you that. I want to speed things up because I have great skin besides the bruise. I will discuss PRP with my provider on my next appointment.

      @@bethgleicher2362 what we have is called hemosiderin staining. Hemosiderin staining occurs when red blood cells are broken down, causing hemoglobin to be stored as hemosiderin. Best option for treatment is laser.

    I have bruising from gouri and miracle L. One bruise from gouri over 4 months so far and the miracle L 1 month. What would you suggest to get rid of bruising? It's on my face and not under the eyes. I'm very upset about this.

      Yep this is a thing that can happen with PCL. If you get a bruise it’s a really big problem because it takes a very very long time to go, maybe 6 months

      Best to inject with a cannula and stop if there is any sign of a bruise

      @@DrSarahTonks hi Dr thank you. I’m so worried that’s it’s permanent! Would you suggest PRP or a laser tx to help get rid of it? It happened after I had PDO threads done right afterwords. I have a small one under my cheekbone from beg of April and a big one from over a month ago.

      @@bethgleicher2362 I have this issue too. So annoying. Nothing so far has gotten rid of it. I have tried microneedling, TCA peel, IPL light. I have just had co2 laser on the stain (only one treatment so far) which did not do much. Please let me know if you find something that works 🙏

      Tbh you’ll be lucky to get much in the way of inches, if you want a large difference then it has to be fat transfer or implants

      A week is not long I would give it a wee while longer. If you are concerned then go back to your original injector
