@@Tanman96 I did not experience pain but it is uncomfortable that you can't really move because the tubes where by my bikini area (one on each side). The removal was a little painful. I didn't wake up until 10 days after being intubated so I'm just sharing what I experienced after I woke up. My breathing wasn't good when I woke up but it progressively got better. I'm so thankful to be alive

This machine just saved my brothers life. My brothers organs were shutting down from the delta virus. There was no chance he could recover the doctors said. He went through two cprs, His kidneys were failing, heart was failing and Lungs. A miracle happened and my brothers lungs started to heal. Thank you ecmo for saving my brother. Thank you for this break down. I am a neuro ICU RN and because of COVID my facility is experiencing a high volume of VV ECMO pts and we don't have enough open heart RN'S to care for all the devices. Ask why hospitals are using the death protocol? Boot family out, give remdesivir, blast oxygen, bipap, precedex, vent, dnr. All other meds refused because they can be harmful? Even the WHO is against redesivir, why is it on the protocol?

Since the ecmo machine uses blood thinners, Thank God she was able to get the rest and recover Enough to hold her own. I was on ECMO for a week after going into cardiogenic shock after years of heart failure. Afterward I had an impella and then the lvad. Hospitalized for two months and inpatient rehab for 2 1/2 weeks. I still have some nerve damage and neuropathy but otherwise doing well and working towards a heart transplant after 2 years of therapies. Very good description!

That’s wonderful. I had the same. I was fit and healthy and a virus ruined my heart. I was on ecmo for a week in ICU until I GOT AN LVad. I spent a month in ICU and then went to a ward and waited three and a half months until I was fantastically lucky to get a transplant. I’m so grateful to all the doctors nurses and staff for saving my life. I’m now three years post transplant and baring complications, I am getting by fine.. ECMO survivor checking in 🙋 I was hit with an acute asthma attack at the end of October and was admitted to my local hospital. While they could get my O2 levels up, my lungs weren't expelling CO2 and I was in really bad shape. I was put into a coma and the ECMO team at Wythenshaw in Manchester were called and got me on their machine before they transferred me to their facility thirty miles away. The medical and nursing staff at all stages have been incredible, and this fabulous technology has given me a second chance at life 🙏
My sister spent a few weeks on ECMO and survived. She was an adult that had lung damage from a fire. She entered the hospital June 17th, 2017, placed on ECMO on June 21st, 2017, and removed from ECMO on July 5th 2017.

One year ago today, my husband (age 63)was on ECMO for severe Cardiogenic shock-post MI, He had right vent. failure - which led to some Left vent. failure - had a right pump which failed - then a left pump failure - with stent placement X 3 with no improvement. Cardiac arrest X 3 - Surgery 4 times within 24 hour time frame - was placed on ECMO in Huntington, WV - transported via helicopter to Ruby Memorial WVU Hospital in Morgantown, WV. Excellent group of ECMO technicians and medical staff - placed on VA ECMO - then to RVAD - and the rest is golden. Spent 1 month in hospital, then to in patient rehab, and finally cardiac rehab. No previous cardiac problems and is still alive and doing very, very well. Keep on teaching the wonders of this life-saving technology. It has taken me this long to really research what he survived. My daughter was placed on VV ECMO using a dual cannula when she was 24 hours old due to pulmonary hypertension from Maconium aspiration. Her lungs recovered and she was off the machine in 5 days. She is doing well and expected to be discharged from hospital in a week. You have beautifully explained the procedure. It makes a big difference for parents like us. Thank you !
Hi Eddie! The weirdest thing happened - I watched your ECMO videos this afternoon, and several hours later, I happened to care for a patient who fit the v-v ECMO indications to a T! She's on her way to the ECMO center as we speak! Thanks for giving me the knowledge to be confident in my assessment that "she needs ECMO".
Thats so cool! Well, not that she needed ECMO, but hopefully it'll help her, but more so that you recognized the potential need. One of the best things we can do for our patients is to advocate for them!
I'm 7 months post heart and kidney transplant and ended up going on echmo during surgery.It saved my life but give me nerve damage on my left thigh.Felt like my leg was lit on fire.Grateful to be alive.

I love it!!! I'm a CVOR Surgical Tech and this has truly helped me understand ECMO even more and to helps me to explain more about it to orientees when they come through!!!
I went into cardiac arrest 3 times last year and was put on ECMO twice. It fried a nerve in my left quad and 4 of my toes are in the process of self-amputating because of it, but it saved my life.
In really enjoyed seeing how ECMO has progressed over the past 30 years. I was a cardio-pulmonary tech @ WHMC from 1973-1990. Drs. Robert DeLemos and Don Null were some of the early neonatal pioneers in using ECMO. The typical patient was a premature newborn, usually weighing less than 2000 grams and delivered because they were in fetal distress. When that happened, the baby would release it's meconium plug (essentially inutero stool.) It often had the consistency of tar. The baby would inhale this while still in mom's tummy and require ECMO. Wilford Hall's ECMO device looked somewhat like a Rube Goldberg device, but it worked fantastically well. When I retired in 1990, WHMC had the only ECMO that a neonatal patient could be placed on and flown across the country. The reason- at that time the standard aero-vac plane was some version of a Lear Jet. Not a lot of room inside for all the personnel needed. Thankfully, Uncle Sam had a large fleet of C9 air evac aircraft that could carry ECMO device, patient, 3 RN's and 2 technicians and a respiratory therapist.
How cool! Wow, thank you for sharing. Its always amazing to see how things progress over time and to hear about how things were before. Now-a-days, it has become such a small "manageable" device. Moving around with it is so much easier than even the setup we used just before the new Cardihelp pump, which is quite small and very portable. Add to that the increased availability and usage across many populations and it has become such an incredible tool to save so many people. I'm always in awe of the technology and it will be interesting to see how it advances in the future!
My loved one is currently on ECMO. Our family has been on pins and needles because we are not familiar with this process and our loved one is a 2 mos old baby. Thank you for this informative video. My anxiety about ECMO has been calmed down.

Loved the video. I was placed on ecmo. I was awake when the canulas where placed. I seriously didn’t think I was going to make it. Then when I was woken after I’d never felt so thankful to be alive. This machine and the drs who use it are definitely miracles to people fighting for there lives

I had never heard of Ecmo until my daughter used it as soon as I was born. I just have to say that it really is a miracle. I am grateful to that machine.
Really excellent presentation and a very understandable breakdown of the ECMO components and processes. I was curious about how ECMO worked and when it was indicated since the use of ECMO appears to be so critical in the tx. of advanced CV. I'm not a doctor, but I teach future RN's and Doctors at a High School pre-med program and the students like to throw random questions my way concerning current med issues and tx. so I just want to be prepared! I'm an xray tech at a hospital in Florida. We have been using ecmo for our covid patients. We have had patients flown in from all over central Florida to go on ecmo. We do fluoroscopy on these patients with c arm to help place the cannulas and chest and abdominal xrays afterwards to ensure the placement is correct and to monitor the lungs. I found this video extremely informative and very important, and plan on sharing it so we will have a more detailed idea of what we are seeing in the ICU and O.R on a daily basis.

I have been aware of ECMO for decades. It is an amazing technology but technically difficult, expensive and not widely available ( in the sense that not every hospital has it, or has multiple machines, or techs that can run the machine). Glad we have it, but better to avoid Covid infection. Retired RN
This explains plenty. Especially when you state it's not going to fix our patient. My brother age 58, was placed on ECMO. His heart was under great duress, the first heart attack happened in July. In December he was placed on the list for a heart transplant. After 12 hours he was removed from ECMO,3 minutes later he died.
We almost exclusively use central VA ECMO in my hospital. It is used post op for children who had any complications during open heart surgery. I believe that if it's decided that they shouldn't come straight off the bypass machine used in surgery they swap them straight onto ECMO in ICU and they might stay on that for anywhere from 3 days to 3 weeks typically. I work in a different dept and I've never been able to get much of an explanation as to why some patients go on ECMO and some don't beyond "to give their heart a rest while it recovers", so this video's been helpful, although the indications in the video don't really seem to completely line up with what we do.
I am an Electrical & Computer Engineer with Biomedical background. I was just curious to find the details of ECMO since my brother who is a doctor in India had a heart attack and they did angioplasty to insert a stent. Since they were not satisfied with his conditions, they connected him to an ECMO equipment. Your video has given me an awesome explanation of ECMO and thank you very much for the same. Our professor at an Iraqi university from Karbala at Al-Zahra University told us today about the ECMO device and said that he had not seen this device due to its high cost, even in his dreams, and in reality it is a device that is widely spread on social media sites.

I was in med surg III last summer (2021) and I think I was in either IMC or COU (cardiac observation unit) and my clinical instructor was coming out of the ICU with an excited look on her face and she said, “You guys, quickly, who hasn’t gone in ICU yet?” I rose my hand with another student out of 6 while other two students were shadowing in ICU. My clinical instructor brought us in the icu to the first room you could access and the door was slightly opened but my instructor said inside there is a patient using an ECMO machine. There is an ICU nurse in there 1:1 at all times charting every 15 minutes. There might have been 2 nurses, but there was a mandatory nurse in the room at all times. My instructor said, “This patient is the most sickest patient in the entire hospital. COVID is nothing compared to this”.

I have worked in my PICU as CA since I was 18 as a way to put me through school and get experience for med school. I’m in my gap year and am using this time to study what I see in my unit. We almost always have at least one child on ECMO, almost always VA ECMO. I would say that we don’t follow probably half of the contraindications you listed. I very regularly have kids on CRRT and ECMO. I very regularly have kids that have been intubated their whole life come back from the OR on ECMO. I once had a teenager intubated for a week, transitioned to an oscillator for a few days, and eventually go on VV ECMO. I understand that an oscillator is going to lower some of that pressure, but wouldn’t ECMO still technically be contraindicated? My understanding has always been that peds ECMO is incredibly different because of things like changed hemodynamics and sometimes anatomy. I’m curious about why these things don’t seem to matter the same?
Two close people that I knew had Covid, both of then were on ECMO, unfortunately both of them died few days later. I don’t have good memories with that machine but I’m glad to hear that it saved others lives ❤️

My friend was on ECMO for 31 days. She died. She had a congenital heart defect. Her mitral valve was damaged. She had 3 previous procedures. The went to have a 4 procedure to install a mechanical valve. Due to scar tissue they were unable to put it in so they inserted a pig valve. After her procedure she had a heart attack. They did a single CBG. She arrested but they got her back and they put her on ECMO.
Praying for my aunt on ECMO now I wanted to understand the way the ECMO works. This is such a game changer Blessings to everyone caring for patients on the machine & all ECMO survivors 🤍🎁
I was on ecmo when i was 6 months or even earler as i was born at 26 weeks i was 3 months premature i died 5 times i was supost to be born in December but i was born in September i had to live with a oxeygen for the first 3 years of my life i had to be fed thought a peg till i was 5 as i kept pergectial vomiting and so i am really a surviour i am now 21 i wouldnt be here if it wasnt for the nhs servies in the uk i am very greatful for the job they do

6 years ago I’ve had pneumonia and sepsis. Ended up on ECMO with multi organ failure. I can’t believe how lucky I am to be alive

I was on ECMO for a week after going into cardiogenic shock after years of heart failure. Afterward I had an impella and then the lvad. Hospitalized for two months and inpatient rehab for 2 1/2 weeks. I still have some nerve damage and neuropathy but otherwise doing well and working towards a heart transplant after 2 years of therapies. Very good description!

That’s wonderful. I had the same. I was fit and healthy and a virus ruined my heart. I was on ecmo for a week in ICU until I GOT AN LVad. I spent a month in ICU and then went to a ward and waited three and a half months until I was fantastically lucky to get a transplant. I’m so grateful to all the doctors nurses and staff for saving my life. I’m now three years post transplant and baring complications, I am getting by fine.. I’m so glad you are doing good too.

I was hit with an acute asthma attack at the end of October and was admitted to my local hospital. While they could get my O2 levels up, my lungs weren't expelling CO2 and I was in really bad shape. I was put into a coma and the ECMO team at Wythenshaw in Manchester were called and got me on their machine before they transferred me to their facility thirty miles away.
The medical and nursing staff at all stages have been incredible, and this fabulous technology has given me a second chance at life 🙏
My sister spent a few weeks on ECMO and survived. She was an adult that had lung damage from a fire. She entered the hospital June 17th, 2017, placed on ECMO on June 21st, 2017, and removed from ECMO on July 5th 2017.

One year ago today, my husband (age 63)was on ECMO for severe Cardiogenic shock-post MI, He had right vent. failure - which led to some Left vent. failure - had a right pump which failed - then a left pump failure - with stent placement X 3 with no improvement. Cardiac arrest X 3 - Surgery 4 times within 24 hour time frame - was placed on ECMO in Huntington, WV - transported via helicopter to Ruby Memorial WVU Hospital in Morgantown, WV. Excellent group of ECMO technicians and medical staff - placed on VA ECMO - then to RVAD - and the rest is golden. Spent 1 month in hospital, then to in patient rehab, and finally cardiac rehab. No previous cardiac problems and is still alive and doing very, very well. Keep on teaching the wonders of this life-saving technology. It has taken me this long to really research what he survived. My daughter was placed on VV ECMO using a dual cannula when she was 24 hours old due to pulmonary hypertension from Maconium aspiration. Her lungs recovered and she was off the machine in 5 days. She is doing well and expected to be discharged from hospital in a week. You have beautifully explained the procedure. It makes a big difference for parents like us. Thank you !

@@treeceysfatcatenterstheroo5622 Thanks. Yeah my purpose in this part was to explain the how behind the gas exchange. Many staff new to ECMO are coming from CRRT management and understand the concept of fiber use for flow and diffusion, though that is blood in the fiber for CRRT.
I’m not a medical professional but I have a body with both respiratory and cardiac glitches. It’s very good to have this explained so thoroughly.

I went into cardiac arrest 3 times last year and was put on ECMO twice. It fried a nerve in my left quad and 4 of my toes are in the process of self-amputating because of it, but it saved my life.
Glad to hear you made it through! Unfortunately sometimes there are long term effects, but it does save lives!
In really enjoyed seeing how ECMO has progressed over the past 30 years. I was a cardio-pulmonary tech @ WHMC from 1973-1990. Drs. Robert DeLemos and Don Null were some of the early neonatal pioneers in using ECMO. The typical patient was a premature newborn, usually weighing less than 2000 grams and delivered because they were in fetal distress. When that happened, the baby would release it's meconium plug (essentially inutero stool.) It often had the consistency of tar. The baby would inhale this while still in mom's tummy and require ECMO. Wilford Hall's ECMO device looked somewhat like a Rube Goldberg device, but it worked fantastically well. When I retired in 1990, WHMC had the only ECMO that a neonatal patient could be placed on and flown across the country. The reason- at that time the standard aero-vac plane was some version of a Lear Jet. Not a lot of room inside for all the personnel needed. Thankfully, Uncle Sam had a large fleet of C9 air evac aircraft that could carry ECMO device, patient, 3 RN's and 2 technicians and a respiratory therapist.
How cool! Wow, thank you for sharing. Its always amazing to see how things progress over time and to hear about how things were before. Now-a-days, it has become such a small "manageable" device. Moving around with it is so much easier than even the setup we used just before the new Cardihelp pump, which is quite small and very portable. Add to that the increased availability and usage across many populations and it has become such an incredible tool to save so many people. I'm always in awe of the technology and it will be interesting to see how it advances in the future!

Really excellent presentation and a very understandable breakdown of the ECMO components and processes. I was curious about how ECMO worked and when it was indicated since the use of ECMO appears to be so critical in the tx. of advanced CV. I'm an xray tech at a hospital in Florida. We have been using ecmo for our covid patients. We have had patients flown in from all over central Florida to go on ecmo. We do fluoroscopy on these patients with c arm to help place the cannulas and chest and abdominal xrays afterwards to ensure the placement is correct and to monitor the lungs. I found this video extremely informative and very important, and plan on sharing it so we will have a more detailed idea of what we are seeing in the ICU and O.R on a daily basis.

I have been aware of ECMO for decades. It is an amazing technology but technically difficult, expensive and not widely available ( in the sense that not every hospital has it, or has multiple machines, or techs that can run the machine). Glad we have it, but better to avoid Covid infection.

This explains plenty. Especially when you state it's not going to fix our patient. My brother age 58, was placed on ECMO. His heart was under great duress, the first heart attack happened in July. In December he was placed on the list for a heart transplant. After 12 hours he was removed from ECMO,3 minutes later he died.

We almost exclusively use central VA ECMO in my hospital. It is used post op for children who had any complications during open heart surgery. I believe that if it's decided that they shouldn't come straight off the bypass machine used in surgery they swap them straight onto ECMO in ICU and they might stay on that for anywhere from 3 days to 3 weeks typically.
I work in a different dept and I've never been able to get much of an explanation as to why some patients go on ECMO and some don't beyond "to give their heart a rest while it recovers", so this video's been helpful, although the indications in the video don't really seem to completely line up with what we do.

I am an Electrical & Computer Engineer with Biomedical background. I was just curious to find the details of ECMO since my brother who is a doctor in India had a heart attack and they did angioplasty to insert a stent. Since they were not satisfied with his conditions, they connected him to an ECMO equipment. Your video has given me an awesome explanation of ECMO and thank you very much for the same. Our professor at an Iraqi university from Karbala at Al-Zahra University told us today about the ECMO device and said that he had not seen this device due to its high cost, even in his dreams, and in reality it is a device that is widely spread on social media sites.

I was in med surg III last summer (2021) and I think I was in either IMC or COU (cardiac observation unit) and my clinical instructor was coming out of the ICU with an excited look on her face and she said, “You guys, quickly, who hasn’t gone in ICU yet?” I rose my hand with another student out of 6 while other two students were shadowing in ICU. My clinical instructor brought us in the icu to the first room you could access and the door was slightly opened but my instructor said inside there is a patient using an ECMO machine. There is an ICU nurse in there 1:1 at all times charting every 15 minutes. There might have been 2 nurses, but there was a mandatory nurse in the room at all times. My instructor said, “This patient is the most sickest patient in the entire hospital. COVID is nothing compared to this”.
I didn't know this existed. Believe it or not.. I was thinking about a machine like this. And here i get to know about it 👍👍👍 Thank you very much 👏👌👍✌💪

I have worked in my PICU as CA since I was 18 as a way to put me through school and get experience for med school. I’m in my gap year and am using this time to study what I see in my unit.
We almost always have at least one child on ECMO, almost always VA ECMO. I would say that we don’t follow probably half of the contraindications you listed. I very regularly have kids on CRRT and ECMO. I very regularly have kids that have been intubated their whole life come back from the OR on ECMO. I once had a teenager intubated for a week, transitioned to an oscillator for a few days, and eventually go on VV ECMO. I understand that an oscillator is going to lower some of that pressure, but wouldn’t ECMO still technically be contraindicated? My understanding has always been that peds ECMO is incredibly different because of things like changed hemodynamics and sometimes anatomy. I’m curious about why these things don’t seem to matter the same?
Two close people that I knew had Covid, both of then were on ECMO, unfortunately both of them died few days later. I don’t have good memories with that machine but I’m glad to hear that it saved others lives ❤️

My friend was on ECMO for 31 days. She died. She had a congenital heart defect. Her mitral valve was damaged. She had 3 previous procedures. The went to have a 4 procedure to install a mechanical valve. Due to scar tissue they were unable to put it in so they inserted a pig valve. After her procedure she had a heart attack. They did a single CBG. She arrested but they got her back and they put her on ECMO.

I was on ecmo when i was 6 months or even earler as i was born at 26 weeks i was 3 months premature i died 5 times i was supost to be born in December but i was born in September i had to live with a oxeygen for the first 3 years of my life i had to be fed thought a peg till i was 5 as i kept pergectial vomiting and so i am really a surviour i am now 21 i wouldnt be here if it wasnt for the nhs servies in the uk i am very greatful for the job they do
